【摘要】：第—部分 內(nèi)窺鏡下經(jīng)口咽入路至中下斜坡應(yīng)用解剖學(xué)研究 目的 研究?jī)?nèi)窺鏡下經(jīng)口咽入路至中、下斜坡局部解剖學(xué)并測(cè)量相關(guān)解剖數(shù)據(jù)，為臨床手術(shù)提供形態(tài)學(xué)依據(jù)。方法 交替使用手術(shù)顯微鏡和0°、30°硬質(zhì)內(nèi)窺鏡研究10例成人尸頭，探討經(jīng)口咽至中下斜坡手術(shù)入路；測(cè)量與入路相關(guān)的60具成人顱骨標(biāo)本數(shù)據(jù)。結(jié)果 斜坡常規(guī)開窗20mm*30mm，插入內(nèi)窺鏡和各種手術(shù)器械，，研究對(duì)應(yīng)腦干側(cè)前方的解剖結(jié)構(gòu)；沿途“路標(biāo)”構(gòu)成的路線圖為：口咽→咽后壁軟組織→骨性標(biāo)志(枕骨大孔前緣→下斜坡→咽結(jié)節(jié)→中斜坡)→硬腦膜→非骨性標(biāo)志(Ⅵ-Ⅱ顱神經(jīng)、椎基底動(dòng)脈及其分支，以及其之間解剖毗鄰關(guān)系)；切牙孔、前鼻棘、后鼻棘、卵圓孔、破裂孔、頸動(dòng)脈外口及舌下神經(jīng)管外口的內(nèi)側(cè)緣至咽結(jié)節(jié)及后四項(xiàng)至中線的距離分別為(mm)：(71.0±4.1)、(78.3±4.3)、(33.7±4.3)、(27.2±2.1)、(13.2±2.0)、(25.7±2.3)、(19.7±1.3)、(23.3±2.1)、(10.5±2.0)mm、(25.2±3.7)、(17.2±2.7)。結(jié)論 應(yīng)用內(nèi)窺鏡經(jīng)口咽入路可完成中下斜坡腦干腹側(cè)部位病變的手術(shù)治療，以處理椎基底動(dòng)脈瘤及體積較小且血供一般的病變?yōu)榧选?br/>[Abstract]:Applied Anatomical study of part-part Endoscopic approach from oropharynx to Middle and Lower ClivusObjective to study the local anatomy of middle and inferior Clivus via oropharynx approach under endoscope and measure the relevant anatomical data to provide morphological basis for clinical operation.Methods 10 adult cadaveric heads were studied by using surgical microscope and 0 擄30 擄rigid endoscope.
