SD大鼠腰2神經根切斷后對下肢運動功能影響的研究

【摘要】:目前由車禍等高能量損傷所致骨盆骨折等損傷是導致的骶叢根性撕脫傷的主要原因之一,其損傷容易導致一側下肢運動感覺功能障礙及膀胱、性功能障礙。針對骶叢根性撕脫傷的外科治療效果欠佳,因此骶叢撕脫傷的治療成為了目前骨科臨床治療的一大重點和難點問題。目前國內采取健側神經根移位治療骶叢撕脫傷已取得一定進展,但動力源神經不足成為限制修復效果的關鍵因素。為進一步探討骶叢撕脫傷的治療,尋找新的動力源神經以用于以為治療骶叢撕脫傷,課題組在對大鼠腰叢神經解剖參數觀測的基礎上,通過解剖觀察大鼠腰2神經的走行及主要支配肌肉,并已成功建立大鼠腰2神經根切斷的動物模型,通過觀察大鼠術后一般情況,雙側肌肉濕重對比,結合靶肌肉橫斷面積變化,以及相應周圍神經肌電圖的變化規(guī)律。找出切斷腰2神經根后對大鼠下肢運動功能的影響。為將腰2神經作為新的動力源神經用以修復骶叢撕脫傷提供了理論依據和實驗基礎。 第一部分SD大鼠腰2神經的解剖測量及腰叢電生理檢測 研究目的:在解剖分析大鼠腰2神經的組成及走行基礎上,結合肌電圖方法,確定動物模型大鼠腰2神經的走行及其支配的主要肌肉。 研究方法:選用10只成年SD大鼠(雌雄不限),暴露腰2神經根后向遠端逐漸分離至靶肌肉,確定大鼠腰2神經支配肌肉,并測量神經長度等數據。選用10只成年SD大鼠,通過不打開椎板方法,在椎間孔外暴露腰2~4神經根,并利用肌電圖記錄儀記錄雙側股四頭肌、股二頭肌及脛骨前肌刺激電位,記錄刺激結果。 結果:通過解剖觀察,腰2神經自椎間孔發(fā)出后,向腹側走行,穿過腰小肌到達腹膜后,沿髂腰肌下行后,匯入股神經;肌電圖示腰2神經參與股神經組成,參與支配股四頭肌的少部分。 結論:通過解剖測量及肌電圖檢測確定大鼠腰2神經走行及支配肌肉是可行的。 第二部分SD大鼠切斷腰2神根經后對下肢運動功能的影響 研究目的:研究單側切斷SD大鼠腰2神根經后對其下肢運動功能的影響 研究方法:選用體重250-300g的成年雄性SD大鼠共40只,隨機數表法分為2組,,采用不打開椎板的手術方法,暴露腰2神經根并切斷,建立腰2神經根切斷動物模型,右側為實驗側,左側為對照側(暴露腰2神經根但不切斷)。分別于損傷后4周,6周,8周,12周,16周取動物模型大鼠,進行大體行為學觀察,術后16周取動物模型大鼠,進行相應靶肌肉的肌電圖檢測和靶肌肉的HE染色切片和三色染色切片檢測。 結果:1、一般情況:術后38只大鼠存活,同等條件下補齊40只大鼠3.存活大鼠下肢BBB評分實驗組為20.3±0.90分,正常組為21分;3.20只實驗組SD大鼠均未在在雙側大腦皮層內記錄到SEP,按照SEP標準及解釋,說明實驗組造模成功;4.股四頭肌、股二頭肌、脛骨前肌HE染色切片及三色染色切片檢測未見相應肌細胞明顯萎縮情況。 結論:單側切斷SD大鼠腰2神根經前后其下肢運動功能未見顯著差異。
