經(jīng)皮頸椎后路內(nèi)窺鏡術(shù)安全性的應(yīng)用解剖學(xué)

【摘要】:[目的]通過(guò)應(yīng)用解剖學(xué)方法探討經(jīng)皮頸椎后路內(nèi)窺鏡下頸椎間盤(pán)切除術(shù)的安全性因素,為臨床手術(shù)提供理論依據(jù)。[方法]選取5具成人尸體標(biāo)本,觀測(cè)側(cè)塊高度(a)、寬度(b);以關(guān)節(jié)突關(guān)節(jié)間隙后內(nèi)側(cè)緣交點(diǎn)(O點(diǎn))為中心,建立平面直角坐標(biāo)系,觀測(cè)O點(diǎn)與對(duì)應(yīng)節(jié)段神經(jīng)根下緣和硬脊膜外側(cè)緣交點(diǎn)(A點(diǎn))、下一節(jié)段神經(jīng)根上緣與硬脊膜外側(cè)緣交點(diǎn)(B點(diǎn))的位置關(guān)系及各象限重要結(jié)構(gòu)的毗鄰關(guān)系;O點(diǎn)與硬脊膜外側(cè)緣的水平距離(e)、與椎動(dòng)脈前壁的垂直距離(g)以及內(nèi)側(cè)壁的水平距離(j);椎動(dòng)脈內(nèi)側(cè)壁與硬脊膜外側(cè)緣的水平距離(d);神經(jīng)根與下關(guān)節(jié)突交點(diǎn)和硬脊膜外側(cè)緣的水平距離(c);上位椎弓根內(nèi)下緣(G點(diǎn))與下位椎弓根內(nèi)上緣(H點(diǎn))連線與O點(diǎn)的水平距離(f);AB間的距離(k)。所得數(shù)據(jù)行統(tǒng)計(jì)學(xué)分析。[結(jié)果]a為(10.17±0.66)mm~(11.69±0.97)mm;b為(10.30±0.89)mm~(12.29±0.47)mm;e為(5.66±0.51)mm~(7.57±0.44)mm;g為(3.79±0.44)mm~(4.29±0.47)mm;d為(5.73±0.47)mm~(8.29±0.70)mm;c為(6.90±0.60)mm~(8.27±0.44)mm;f為(1.69±0.87)mm~(4.83±0.96)mm;j為(4.81±0.90)mm~(7.40±0.79)mm;k為(18.12±1.11)mm~(20.87±1.19)mm。[結(jié)論]PPECD中O點(diǎn)是更具臨床意義的定位點(diǎn),可作為手術(shù)入點(diǎn)標(biāo)示開(kāi)骨窗安全范圍、準(zhǔn)確定位病變部位并避免損傷椎管內(nèi)外重要結(jié)構(gòu)。
[Abstract]:[objective] to explore the safety factors of percutaneous endoscopic cervical discectomy by anatomical method, and to provide theoretical basis for clinical operation. [methods] five adult cadavers were selected.
