上海地區(qū)獻(xiàn)血者IgA缺乏調(diào)查及CD36缺失表型的初步研究

【摘要】:免疫球蛋白A(immunoglobulin A, IgA)缺乏一般指血清中IgA含量少于5mg/dL或缺失,當(dāng)含量低至0.05mg/dL以下時(shí)則稱為IgA完全缺乏。在IgA缺乏的個(gè)體中會(huì)產(chǎn)生抗-IgA,當(dāng)輸注含IgA的血液成分時(shí)可增加輸血過敏反應(yīng)的發(fā)生風(fēng)險(xiǎn),嚴(yán)重者可致休克或死亡。IgA缺乏癥在不同種族中的發(fā)生頻率已有廣泛研究,且存在差異性。中國健康獻(xiàn)血者人群中IgA發(fā)生率仍未知,且對IgA缺乏者的輸血治療缺少針對性的輸血策略。本研究對來自上海及周邊地區(qū)漢族健康獻(xiàn)血者進(jìn)行IgA缺乏篩查,以獲得其發(fā)生頻率,并初步分析IgA輸血過敏反應(yīng)發(fā)生風(fēng)險(xiǎn),以保障輸血安全。通過建立一種雙夾心酶聯(lián)免疫分析(ELISA)方法初步檢測22609份健康獻(xiàn)血員血清中IgA含量,IgA缺乏者再用凝膠免疫法確認(rèn)之,同時(shí)對IgA缺乏標(biāo)本作抗-IgA的檢測。經(jīng)統(tǒng)計(jì),22 609份健康獻(xiàn)血者標(biāo)本中,發(fā)現(xiàn)14例IgA缺乏者,比例為0.062%(1:1615)。其中7例確認(rèn)為IgA含量0.05 mg/dL,由此上海地區(qū)獻(xiàn)血人群IgA完全缺乏比例為0.031%(1:3230)。在這14例IgA缺乏(IgA5 mg/dL)標(biāo)本中鑒定出2例含抗-IgA;IgA缺乏且含有抗-IgA者比例為0.009%(1:11305)。上海地區(qū)健康獻(xiàn)血者中IgA缺乏發(fā)生頻率介于日本人種與高加索人種之間,但對IgA缺乏者的輸血仍存在潛在的風(fēng)險(xiǎn),需要采取一定措施預(yù)防和規(guī)避輸血過敏反應(yīng)的發(fā)生
[Abstract]:Immunoglobulin A (immunoglobulin A, IgA) deficiency generally refers to the IgA content in the serum is less than or absent from 5mg/dL, and when the content is lower than 0.05mg/dL, it is called IgA deficiency. Anti-IgA is produced in individuals with IgA deficiency, which increases the risk of allergic reactions to blood transfusion when infused with blood components containing IgA, and the frequency of shock or death in severe cases in different ethnic groups has been extensively studied. And there are differences. The incidence of IgA in healthy blood donors in China is still unknown, and there is no specific transfusion strategy for patients with IgA deficiency. In order to obtain the frequency of IgA deficiency in healthy blood donors of Han nationality from Shanghai and its surrounding areas, the risk of allergic reaction of IgA transfusion was preliminarily analyzed in order to ensure the safety of blood transfusion. A double sandwich enzyme-linked immunosorbent assay (ELISA) was established to determine the level of IgA in serum of 22609 healthy blood donors and confirmed by gel immunosorbent assay (GIA). Meanwhile, anti-IgA was detected in the samples of IgA deficiency. Among 22 609 healthy blood donors, 14 cases of IgA deficiency were found, the proportion was 0.062% (1: 1615). Among them, 7 cases were confirmed as IgA content of 0. 05 mg / dL, and the proportion of total IgA deficiency was 0. 031% (1: 3230) in blood donors in Shanghai area. Two of the 14 IgA deficiency (IgA5 mg/dL) samples were identified as having anti-IgA deficiency and anti-IgA in 0.009% (1: 11305). The frequency of IgA deficiency in healthy blood donors in Shanghai is between Japanese and Caucasian people. However, there is still a potential risk of blood transfusion for IgA deficient people. Some measures should be taken to prevent and avoid the occurrence of allergic reaction to blood transfusion.
