【摘要】：目的探討回陽(yáng)生肌膏對(duì)慢性皮膚潰瘍的影響及可能作用機(jī)制。方法 150只BALB/c小鼠分為空白組、模型組、西藥組、益氣溫陽(yáng)組、活血通絡(luò)組,每組30只。采用免疫抑制聯(lián)合皮膚傷口的方法制備小鼠慢性皮膚潰瘍模型。益氣溫陽(yáng)組和活血通絡(luò)組分別給予益氣溫陽(yáng)方和活血通絡(luò)方各0.018 g,西藥組給予重組人堿性成纖維細(xì)胞生長(zhǎng)因子0.3 g,均制成含藥明膠海綿、無(wú)菌貼貼敷,隔天換藥,進(jìn)行瘡面拍照。測(cè)定瘡面面積并計(jì)算愈合率;創(chuàng)傷第3、7、14天取瘡面組織,HE、Masson和天狼星紅染色觀察瘡面新生肉芽組織及膠原分布等;免疫組織化學(xué)法檢測(cè)瘡面組織巨噬細(xì)胞數(shù)量;使用實(shí)時(shí)定量PCR檢測(cè)瘡面局部M1、M2型巨噬細(xì)胞分泌的細(xì)胞因子基因表達(dá)。結(jié)果損傷第7、14天,與模型組比較,西藥組、益氣溫陽(yáng)組、活血通絡(luò)組瘡面面積減小,愈合率升高(P0.05),且活血通絡(luò)組愈合率高于益氣溫陽(yáng)組(P0.05)。損傷第3天,空白組、西藥組、益氣溫陽(yáng)組白細(xì)胞介素6(IL-6)水平低于模型組,血管內(nèi)皮生長(zhǎng)因子(VEGF)、白細(xì)胞介素10(IL-10)水平高于模型組,活血通絡(luò)組VEGF、IL-10水平高于模型組,且活血通絡(luò)組VEGF水平高于益氣溫陽(yáng)組(P0.05)。損傷第7天,空白組、西藥組、益氣溫陽(yáng)組、活血通絡(luò)組皮膚M1型巨噬細(xì)胞數(shù)量、TNF-α水平明顯低于模型組,M2型巨噬細(xì)胞數(shù)量、VEGF、IL-10水平高于模型組(P0.05或P0.01),且活血通絡(luò)組VEGF、IL-10水平高于益氣溫陽(yáng)組(P0.05)。結(jié)論活血通絡(luò)方與益氣溫陽(yáng)方均可促進(jìn)小鼠皮膚慢性瘡面愈合,其機(jī)制可能是通過(guò)促進(jìn)巨噬細(xì)胞表型轉(zhuǎn)化,促使瘡面修復(fù)由炎癥期過(guò)渡到增殖期,并且活血通絡(luò)方對(duì)巨噬細(xì)胞表型轉(zhuǎn)化作用優(yōu)于益氣溫陽(yáng)方。
[Abstract]:Objective to investigate the effect of Huiyang Shengji ointment on chronic skin ulcer and its possible mechanism. Methods 150 BALB / c mice were divided into blank group, model group, western medicine group, warming air temperature group and activating blood circulation and Tongluo group with 30 mice in each group. The model of chronic skin ulcer in mice was established by immunosuppression combined with skin wound. The Yiqen-yang and Huoxue Tongluo groups were given 0.018 g of Yiqen-yang recipe and 0.018 g of Huoxue Tongluo recipe respectively, and the western medicine group were given recombinant human basic fibroblast growth factor 0.3g respectively. All of them were made into drug-containing gelatin sponge, aseptic paste, and changed the medicine every other day. Take a picture of the sore face. The area of the sore surface was measured and the healing rate was calculated, the tissue of the sore surface was stained with HEMasson and Sirius Red on the 3rd day of trauma and the distribution of new granulation tissue and collagen was observed, and the number of macrophages in the tissue of the sore surface was detected by immunohistochemical method. The gene expression of cytokines secreted by M _ 1 M _ 2 macrophages was detected by real-time quantitative PCR. Results on the 14th day of injury, compared with the model group, the area of the sore surface decreased and the healing rate increased in the western medicine group, the warming Yang group and the activating blood circulation and Tongluo group (P0.05), and the healing rate in the Huoxue Tongluo group was higher than that in the warming Yang group (P0.05). On the 3rd day of injury, the levels of IL-6, VEGF and IL-10 in the blank group, western medicine group and Yiyangyang group were lower than those in the model group, and the levels of vascular endothelial growth factor (VEGF) and interleukin-10 (IL-10) in the model group were higher than those in the model group, and the level of VEGF IL-10 in the Huoxue Tongluo group was higher than that in the model group. And the level of VEGF in the group of activating blood circulation and Tongluo was higher than that of the group of benefiting air temperature and yang (P0.05). On the 7th day of injury, blank group, western medicine group, warming air temperature group, The level of TNF- 偽 in skin M1 type macrophages in Huoxue Tongluo group was significantly lower than that in model group (P0.05 or P0.01), and the level of VEGFN- 10 in activating blood circulation Tongluo group was higher than that in warming Yang group (P0.05). Conclusion Huoxue Tongluo recipe and YYYYYYYZHAN Yang prescription can promote the healing of chronic sores in mice. The mechanism may be to promote the transformation of macrophage phenotype and promote the transition from inflammatory stage to proliferative stage. And the effect of Huoxue Tongluo recipe on phenotypic transformation of macrophages was better than Yiqian Yang recipe.
