MSCTP實(shí)驗(yàn)動(dòng)物模型研究

【摘要】:研究目的:探討不同劑量、流速對(duì)比劑對(duì)動(dòng)物門(mén)靜脈達(dá)峰時(shí)間、門(mén)靜脈強(qiáng)化峰值及門(mén)靜脈成像的影響。材料與方法1、不同注射劑量、流速時(shí)門(mén)靜脈達(dá)峰時(shí)間及強(qiáng)化峰值實(shí)驗(yàn)實(shí)驗(yàn)對(duì)象:選擇正常巴馬豬6頭,體重約20kg-40kg。CT掃描前實(shí)驗(yàn)動(dòng)物予以禁食10小時(shí)、禁水4小時(shí)及測(cè)量體重。其中4頭豬分別進(jìn)行不同劑量(2.0ml/kg,2.5ml/kg)和不同流速(2. Oml/s,2.5ml/s 3. Oml/s)配伍組合,掃描24次;另2頭豬只進(jìn)行3次上述不同劑量、不同流速配伍組合,掃描3次,共計(jì)掃描27次。同一頭豬相隔掃描的時(shí)間至少超過(guò)48小時(shí)。CT掃描:正常巴馬豬先行麻醉后,將其取俯臥位固定于自制紙板并放置于掃描床上,同時(shí)建立耳背靜脈注射通道。掃描設(shè)備為64排64層GE Light Speed VCT。先行全肝臟CT平掃,選擇層厚為5啪連續(xù)掃描。瀏覽圖像并選取門(mén)脈主干(發(fā)出第一支門(mén)靜脈右支層面或其向上2個(gè)層面)作為連續(xù)動(dòng)態(tài)增強(qiáng)掃描(灌注掃描)的靶層面。經(jīng)雙筒高壓注射器經(jīng)耳背靜脈團(tuán)注離子型對(duì)比劑60%泛影葡胺,注射對(duì)比劑2s之后開(kāi)始掃描,1層/次/秒,間隔1s,共掃描時(shí)間80s,獲得圖像40張。CT圖像處理:將動(dòng)態(tài)增強(qiáng)掃描數(shù)據(jù)傳入GE Advantage Window4.4工作站,在所獲得圖像上,于門(mén)靜脈主干放置ROI區(qū),利用Perfusion4軟件或手工測(cè)量并繪制門(mén)靜脈的時(shí)間-密度曲線(TDC),觀察門(mén)靜脈達(dá)峰時(shí)間及其強(qiáng)化峰值。2、門(mén)靜脈血管成像實(shí)驗(yàn)實(shí)驗(yàn)對(duì)象:上述實(shí)驗(yàn)中的4頭巴馬豬。實(shí)驗(yàn)方法:通過(guò)上述實(shí)驗(yàn)獲得的門(mén)靜脈達(dá)峰時(shí)間即最佳掃描延遲時(shí)間的基礎(chǔ)上,對(duì)同一頭巴馬豬至少相隔48h后,用相同的流速、造影劑及劑量計(jì)算方式再次進(jìn)行MSCT門(mén)靜脈期增強(qiáng)掃描。掃描范圍以包括整個(gè)肝實(shí)質(zhì)。共獲得24組MIP. VR圖像。圖像處理與分析:將掃描數(shù)據(jù)以1.25mmm薄層重建并傳入GEAdvantage Window4.4工作站進(jìn)行VR、MIP門(mén)靜脈血管成像后處理。獲得的門(mén)靜脈血管圖像進(jìn)行質(zhì)量評(píng)估,觀察血管顯示的分支情況、清晰度把門(mén)靜脈血管圖像分為A、B、C、D四個(gè)等級(jí)。所有計(jì)數(shù)指標(biāo)輸入SPSS16.0統(tǒng)計(jì)軟件包(析因設(shè)計(jì)方差分析)及Microsoft Office Excel 2003進(jìn)行統(tǒng)計(jì)學(xué)處理。結(jié)果:1、固定流速為2.0ml/s,劑量分別為2. Oml/kg及2.5ml/kg時(shí),Imax P分別為187.9±13.3Hu,215.7±9.8Hu;固定流速為2.5m1/s,劑量分別為2. Oml/kg及2.5ml/kg時(shí),Imax P分別為188.1±24.4Hu,227.3±36.7Hu;固定流速為3.0ml/s,劑量分別為2.0ml/kg及2.5ml/kg時(shí),Imax P分別為214.5±17.OHu,235.3±11.5Hu。2、劑量不同時(shí)門(mén)靜脈強(qiáng)化峰值有差別(P0.05)。在不考慮流速的前提下,劑量2.5ml/kg與2. Oml/kg相比,門(mén)靜脈強(qiáng)化峰值增加了29.28HU(P0.05)。流速不同時(shí)門(mén)靜脈強(qiáng)化峰值無(wú)統(tǒng)計(jì)學(xué)意義(P0.05)。3、固定流速為2.0ml/s,劑量分別為2.Oml/kg及2.5ml/kg時(shí),Tmax P分別約為43.4±8.2 s,49.0±8.2s;固定流速為2.5ml/s,劑量分別為2.Oml/kg及2.5ml/kg時(shí),Tmax P分別約為35.0±4s,39.5±6.4s;固定流速為3.0ml/s,以劑量分別為2. Oml/kg及2.5ml/kg時(shí),Tmax P分別約為31.0±5.9 s,38.0±1.Os。4、不同流速及劑量對(duì)門(mén)靜脈達(dá)峰時(shí)間均有影響(P0.05)。2.0ml/s與2.5m1/s及2.0ml/s與3.0ml/s之間有差別(P0.05),2.5ml/s與3.0ml/s之間無(wú)差別(p0.05)。在不考慮流速的前提下,劑量2.5m1/kg與2.0ml/kg相比,TmaxP延長(zhǎng)5.73s(P0.05),無(wú)統(tǒng)計(jì)學(xué)意義。5、在劑量為2. Oml/kg情況下,流速為2.0ml/s時(shí)獲得門(mén)脈評(píng)分為C級(jí)2例次,B級(jí)2例次;流速為2.5m1/s時(shí)獲得門(mén)脈評(píng)分為B級(jí)2例次,A級(jí)2例次;流速為3.0ml/s時(shí)獲得門(mén)脈評(píng)分為B級(jí)2例次,A級(jí)2例次。在劑量為2.5ml/kg情況下,流速為2.0ml/s時(shí)獲得門(mén)脈評(píng)分為A級(jí)1例次,B級(jí)3例次;流速為2.5m1/s時(shí)獲得門(mén)脈評(píng)分為A級(jí)3例次,B級(jí)1例次;流速為3.Oml/s時(shí)獲得門(mén)脈評(píng)分為A級(jí)3例次,B級(jí)1例次。結(jié)論1、對(duì)比劑劑量的增加對(duì)門(mén)靜脈強(qiáng)化峰值及門(mén)靜脈達(dá)峰時(shí)間均有影響。其他參數(shù)不變時(shí),隨著劑量的增加,門(mén)靜脈達(dá)峰時(shí)間延長(zhǎng),門(mén)靜脈強(qiáng)化峰值提高。2、對(duì)比劑流速的提高對(duì)門(mén)靜脈達(dá)峰時(shí)間有影響,對(duì)門(mén)靜脈強(qiáng)化峰值無(wú)影響。其他參數(shù)不變時(shí),隨著流速的提高,門(mén)靜脈達(dá)峰時(shí)間提前,門(mén)靜脈強(qiáng)化峰值提高。3、在相同劑量情況下,流速為2.5m1/s與3.Oml/s時(shí)的MSCTP圖像質(zhì)量等級(jí)一致;劑量為2.Oml/kg與2.5m1/kg時(shí),MSCTP圖像質(zhì)量大部分是B級(jí)以上。以達(dá)到診斷要求MSCTP的對(duì)比劑最低劑量、最低流速作為多層螺旋CT門(mén)靜脈期掃描的推薦參數(shù),本研究建議,對(duì)比劑劑量為2.0ml/kg、對(duì)比劑流速為2.5m1/s為佳。
[Abstract]:Objective : To investigate the effect of different dose and flow rate contrast agent on peak time of portal vein , peak value of portal vein and portal vein imaging . Oml/s,2.5ml/s 3. Oml / s combination , scanning for 24 times ;
The results were as follows : 1 layer / time / sec , interval 1s , scanning time 80s , scanning device is 64 rows of 64 - slice GE Light Speed vct . Image processing and analysis : The image of portal vein was reconstructed and passed into GEAdvantage Window4.4 workstation for VR and MIP portal vein imaging after image processing and analysis . The obtained portal vein image was evaluated by quality evaluation . The branch of vascular display was observed . The results were as follows : 1 . The fixed flow rate was 2.0ml / s and the dose was 2 . At Oml / kg and 2.5ml / kg , the difference was 187.9 鹵 13.3Hu , 215.7 鹵 9.8Hu , respectively .
The fixed flow rate was 2.5ml / s and the dose was 2 . When Oml / kg and 2.5ml / kg , the difference was 188.1 鹵 24.4Hu , 227.3 鹵 36.7Hu , respectively .
At the fixed flow rate of 3.0 ml / s and the dosage of 2.0ml / kg and 2.5ml / kg , respectively , the peak values were 214 . 5 鹵 17 . OHu , 235 . 3 鹵 11.5 Hu . 2 , and the dose did not differ significantly ( P 0 . 05 ) at the same time , but the dose was 2.5 ml / kg and 2 without considering the flow rate . Compared with Oml / kg , the peak value of portal vein enhancement increased 29.28HU ( P0.05 ) . The peak value of portal vein enhancement was 2.0ml / s , the dose was 2.0ml / kg and 2.5ml / kg , respectively , Tmax was 43.4 鹵 8.2 s , 49.0 鹵 8.2s , respectively .
At the fixed flow rate of 2.5ml / s and the dosage of 2.Oml / kg and 2.5ml / kg , Tmax was 33.0 鹵 4s , 39.5 鹵 6.4s , respectively ;
The fixed flow rate was 3.0 ml / s with a dose of 2 . At Oml / kg and 2.5ml / kg , Tmax was 31.0 鹵 5.9 s , 38.0 鹵 1.Os respectively . There was no difference between 2.0ml / s and 2.5ml / s and 2.0ml / s and 3.0ml / s ( P0.05 ) . In Oml / kg , the portal vein score was grade C in 2 cases and grade B in 2 cases when the flow rate was 2.0ml / s .
When the flow rate was 2.5ml / s , the portal vein score was grade B in 2 cases and grade A in 2 cases .
When the flow rate was 3.0ml / s , the portal vein score was grade B in 2 cases and grade A in 2 cases . At the dose of 2.5ml / kg , the portal vein score was Grade A and grade B was 3 cases .
When the flow rate was 2.5ml / s , the portal vein score was Grade A , 3 times and grade B was 1 case ;
The peak time of portal vein was prolonged and the peak value of portal vein was increased .
When the dose was 2.Oml / kg and 2.5ml / kg , the image quality of MSCTP was mostly higher than grade B . In order to reach the diagnostic requirement of MSCTP , the lowest dose and the lowest flow rate were the recommended parameters of multi - slice spiral CT portal vein scan . The study suggested that the contrast agent dose was 2.0ml / kg and the flow rate of contrast agent was 2.5ml / s .
