激素聯(lián)合脂多糖誘導(dǎo)股骨頭缺血壞死模型兔的骨質(zhì)變化

【摘要】:背景:前期研究采用激素聯(lián)合脂多糖已成功構(gòu)建股骨頭缺血壞死兔模型。目的:進(jìn)一步觀察激素性股骨頭壞死早期骨質(zhì)的動(dòng)態(tài)變化。方法:成年新西蘭大白兔24只,隨機(jī)分為實(shí)驗(yàn)組(n=12)和對(duì)照組(n=12)。實(shí)驗(yàn)組采用激素聯(lián)合脂多糖改良技術(shù)建立激素性股骨頭缺血壞死兔模型,對(duì)照組相同部位注射等體積生理鹽水,飼養(yǎng)條件不變。建模完成后,分別于第5,10,15,20天進(jìn)行股骨頭多層螺旋CT、顯微CT及蘇木精-伊紅染色檢測(cè),觀察股骨頭結(jié)構(gòu),骨小梁形態(tài)、分布,測(cè)定骨質(zhì)密度及空骨陷窩率。結(jié)果與結(jié)論:(1)影像學(xué)結(jié)果顯示,實(shí)驗(yàn)組與對(duì)照組兔股骨頭形態(tài)完整,關(guān)節(jié)面光滑,第15,20天實(shí)驗(yàn)組股骨頭骨皮質(zhì)變薄,骨小梁分布稀疏,間距增寬,不連續(xù),骨礦物質(zhì)密度、組織礦物質(zhì)密度、骨體積分?jǐn)?shù)值均顯著低于對(duì)照組(P0.01);(2)病理學(xué)結(jié)果顯示,實(shí)驗(yàn)組空骨陷窩和脂肪細(xì)胞不斷增多,骨細(xì)胞和造血細(xì)胞逐漸減少,第15,20天空骨陷窩百分率顯著高于對(duì)照組(P0.01);(3)結(jié)果說明,激素性股骨頭壞死早期股骨頭內(nèi)骨細(xì)胞壞死增多,伴骨小梁微細(xì)骨折發(fā)生,導(dǎo)致骨質(zhì)密度減低,最終可造成骨重塑障礙。
[Abstract]:Background: a rabbit model of avascular necrosis of femoral head has been successfully constructed by hormone and lipopolysaccharide. Objective: to observe the dynamic changes of bone in early stage of steroid-induced femoral head necrosis. Methods: 24 adult New Zealand white rabbits were randomly divided into experimental group (n = 12) and control group (n = 12). The model of steroid-induced avascular necrosis of femoral head was established by hormone and lipopolysaccharide in the experimental group, and the same volume of saline was injected into the control group. After modeling, the multislice spiral CTs of femoral head, microscopic CT and hematoxylin eosin staining were performed on day 5, 10, 15 and 20, respectively. The structure of femoral head, the shape and distribution of trabecular bone, bone density and the rate of empty bone lacuna were observed. Results and conclusion the imaging results showed that the femoral head of the experimental group and the control group had a complete shape and smooth articular surface. On day 1520, the femoral head of the experimental group became thin, the bone trabecular distribution was sparse, the spacing widened, the bone mineral density was discontinuous. The tissue mineral density and bone volume fraction were significantly lower than those in the control group (P 0.01). The pathological results showed that the number of empty bone lacunae and adipocytes in the experimental group was increasing and the number of bone cells and hematopoietic cells was decreasing. The results showed that osteonecrosis of femoral head increased in the early stage of steroid-induced osteonecrosis, accompanied by bone trabecular microfracture, which resulted in the decrease of bone density, which resulted in bone remodeling disorder.
