拇指腕掌關(guān)節(jié)韌帶的解剖力學(xué)分析及臨床意義

【摘要】:目的研究拇指腕掌關(guān)節(jié)背橈韌帶及前斜韌帶的解剖學(xué)及生物力學(xué)特點(diǎn),測(cè)量其彈性模量及延伸率,為傷病時(shí)韌帶修復(fù)提供參考。方法解剖成人男性尸體40例新鮮手標(biāo)本,制做成大多角骨-韌帶-第1掌骨標(biāo)本,20例保留背橈韌帶,20例保留前斜韌帶,在生物力學(xué)機(jī)上測(cè)試,測(cè)量和計(jì)算背橈韌帶及前斜韌帶的長(zhǎng)、寬、厚、最大載荷、彈性模量、延伸率。結(jié)果背橈韌帶、前斜韌帶的最大載荷分別為(213.5±72.4)、(168.7±35.2)N,彈性模量分別為(17.2±6.7)、(9.3±2.5)MPa,延伸率分別為(116.2±21.3)%、(92.7±22.4)%。背橈韌帶的最大載荷、彈性模量及延伸率均大于前斜韌帶。結(jié)論拇指腕掌關(guān)節(jié)囊內(nèi)韌帶中,背橈韌帶剛度較大,韌性較強(qiáng),對(duì)維持關(guān)節(jié)穩(wěn)定性起重要作用。前斜韌帶剛度較小,韌性較差,韌帶退變時(shí)容易損傷。拇指腕掌關(guān)節(jié)炎應(yīng)該首選重建前斜韌帶,且應(yīng)選用彈性模量及延伸率接近背橈韌帶的材料為佳。
[Abstract]:Objective to study the anatomical and biomechanical characteristics of the dorsal radial ligament and anterior oblique ligament of the wrist and metacarpal joint of the thumb, and to measure their modulus of elasticity and elongation so as to provide a reference for the repair of the ligaments during injury. Methods Forty fresh hand specimens of adult male cadavers were dissected. Most horn-ligaments-first metacarpal bone specimens were made. 20 cases of dorsal radial ligament and 20 cases of anterior oblique ligament were preserved. The length, width, thickness, maximum load and elastic modulus of dorsal radial ligament and anterior oblique ligament were measured and calculated by biomechanical machine. Results the maximum loads of the dorsal radial ligament and anterior oblique ligament were 213.5 鹵72.4 and 168.7 鹵35.2N respectively, the elastic modulus were 17.2 鹵6.7 鹵2.5mpa, and the elongation were 116.2 鹵21.3 鹵22.42.The maximum load of the dorsal radial ligament. Conclusion in the internal ligaments of carpometacarpal joint of thumb, the dorsalis radialis ligament plays an important role in maintaining the stability of the joint because of its large stiffness and strong toughness, and the anterior oblique ligament is less rigid and less tenacious than that of the anterior oblique ligament. The anterior oblique ligament should be reconstructed first and the materials with elastic modulus and elongation close to the dorsal radial ligament should be chosen as the best.
