慢性HBV感染患者心理干預(yù)效果評(píng)價(jià)研究

【摘要】:研究背景:我國(guó)是乙肝大國(guó)。慢性HBV感染者普遍存在抑郁、焦慮、悲觀、消沉等心理狀況。臨床上多表現(xiàn)為主觀癥狀較多,對(duì)患者預(yù)后產(chǎn)生了很大的影響,很大程度上影響了慢性HBV感染者的生活質(zhì)量。目前國(guó)內(nèi)外對(duì)于慢性HBV感染者的心理健康狀況分析的研究比較多,從不同角度闡述了慢性HBV感染者存在的心理問題,同時(shí)闡述了不同的心理狀況的應(yīng)對(duì)措施及相關(guān)的護(hù)理體會(huì)。但在臨床中進(jìn)行慢性乙型肝炎、乙型肝炎肝硬化的心理干預(yù)研究較少。 研究目的:對(duì)HBV感染者住院期間在常規(guī)保護(hù)肝臟、對(duì)癥治療基礎(chǔ)上,進(jìn)行心理干預(yù),比較分析心理干預(yù)前后患者心理健康狀況,為進(jìn)行HBV感染者心理干預(yù)提供參考依據(jù)。 資料與方法:采用癥狀自評(píng)量表(SCL-90)、焦慮自評(píng)量表(SAS)和抑郁自評(píng)量表(SDS)對(duì)包頭醫(yī)學(xué)院第一附屬醫(yī)院感染疾病科門診符合入選條件的接受住院治療的120名慢性HBV感染患者進(jìn)行心理干預(yù),評(píng)價(jià)干預(yù)效果。應(yīng)用SPSS11.5軟件包進(jìn)行統(tǒng)計(jì)分析處理。 主要結(jié)果:1)慢性HBV感染者心身癥狀改善慢性乙肝組干預(yù)2周時(shí)與對(duì)照組相比較,SCL-90心理因子得分,軀體化、焦慮、精神癥狀因子得分差異顯著;乙型肝炎肝硬化組干預(yù)2周時(shí),干預(yù)組軀體化、焦慮得分與對(duì)照組比較,差異有顯著性;兩組患者干預(yù)4周時(shí)各心理因子得分比較,差異具有顯著性。心理干預(yù)對(duì)HBV感染者心理癥狀的改善有顯著作用。2)SAS、SDS標(biāo)準(zhǔn)分比較慢性乙肝和乙肝肝硬化組患者入院時(shí)、2周、4周時(shí)SAS、SDS標(biāo)準(zhǔn)分比較,差異均有顯著性。焦慮、抑郁癥狀明顯改善。 結(jié)論與建議:心理干預(yù)可以改善兩組患者的心理健康狀況,建議臨床醫(yī)生在對(duì)慢性HBV感染者進(jìn)行藥物治療的同時(shí),開展心理干預(yù)工作。
[Abstract]:Background: China is a large country of hepatitis B. there are many psychological conditions such as depression, anxiety, pessimism, depression and so on in patients with chronic HBV infection. The clinical manifestations are more subjective symptoms, which have a great impact on the prognosis of patients. At present, there are many studies on the mental health status of chronic HBV infected people at home and abroad. This paper expounds the psychological problems of chronic HBV infected people from different angles. At the same time, the coping measures of different psychological status and related nursing experience were expounded, but the psychological intervention of chronic hepatitis B and hepatitis B cirrhosis in clinic was less. Objective: to carry out psychological intervention on the basis of routine liver protection and symptomatic treatment during the hospitalization of HBV infected persons, and to compare and analyze the mental health status of patients before and after psychological intervention so as to provide reference for psychological intervention of HBV infected persons. Materials and methods: using symptom Checklist 90 (SCL-90), Self-Rating anxiety scale (SAS) and Self-Rating Depression scale (SDS), 120 patients with chronic HBV who received inpatient treatment in the first affiliated Hospital of Baotou Medical College were selected. The patients were involved in psychological intervention. The effect of intervention was evaluated. SPSS11.5 software package was used for statistical analysis. Results the scores of SCL-90 psychological factors, somatization, anxiety and mental symptoms in patients with chronic HBV infection were significantly higher than those in patients with chronic hepatitis B at 2 weeks after intervention, while those in patients with cirrhosis of hepatitis B at 2 weeks after intervention were significantly higher than those in the control group. The scores of somatization and anxiety in the intervention group were significantly different from those in the control group. There was a significant difference between the two groups. Psychological intervention had significant effect on the improvement of mental symptoms of HBV infected patients. The standard scores of SAS were significantly higher in patients with chronic hepatitis B and cirrhosis of hepatitis B than in patients with chronic hepatitis B and cirrhosis of hepatitis B at 2 weeks and 4 weeks after admission, and there was significant difference in anxiety between patients with chronic hepatitis B and patients with cirrhosis of hepatitis B at 4 weeks after admission. The symptoms of depression were obviously improved. Conclusion and suggestion: psychological intervention can improve the mental health status of patients in both groups. It is suggested that clinicians should carry out psychological intervention while treating chronic HBV infection.
