寧波市耐三代頭孢菌素志賀菌攜帶β-內(nèi)酰胺酶與基因分型

【摘要】:目的了解本市耐三代頭孢菌素志賀菌攜帶ESBLs及其基因型,為疾病防控提供依據(jù)。方法采用直接和增菌培養(yǎng)法,分離患者標(biāo)本中志賀菌;藥敏采用K-B法,ESBLs志賀菌表型確證采用紙片法;CTX-M、OXA、TEM和SHV耐藥基因采用PCR法;耐藥基因分型采用核苷酸序列法,用BLAST分析比較確定基因型別。結(jié)果藥敏篩檢出69株耐三代頭孢菌素志賀菌,占ESBLs志賀菌的74.19%。檢出CTX-M(CTX-M-1群和CTX-M-9群)、OXA和TEM耐藥基因,檢出率分別為79.71%、79.01%和26.09%,未檢出CTX-M-2群和SHV耐藥基因。DNA序列比對(duì)CTX-M-1群以CTX-M-15型為主,還檢出7個(gè)其它型;CTX-M-9群則以CTX-M-14型多,其它型檢出6個(gè);49株OXA和18株TEM耐藥基因測(cè)序后均為1型(OXA-1型和TEM-1型)。攜帶2種以上耐藥基因的志賀菌21株,占30.43%。結(jié)論本市志賀菌對(duì)頭孢曲松等三代頭孢菌素耐藥率較高,檢出的ESBLs酶型種類(lèi)多,CTX-M(CTX-M-1群的CTX-M-15型和CTX-M-9群的CTX-M-14型)是主流酶型,且可同時(shí)攜帶多種耐藥基因,給疾病防控帶來(lái)困難。OXA-1型的高攜帶率,提示我們應(yīng)加強(qiáng)分析。B群志賀菌無(wú)論是表型的耐藥性,還是耐藥基因檢出率均高于D群志賀菌,該發(fā)現(xiàn)有助志賀菌的擴(kuò)散與流行研究。
[Abstract]:Objective to study the ESBLs and its genotypes of Shigella cephalosporin resistant to the third generation in order to provide the basis for disease prevention and control. Methods Shigella was isolated from patient samples by direct and bacteriostasis culture. Phenotypic confirmation of Shigella Shigella strains by K-B method. Tem and PCR were used for identification of CTX-MX OXAX Tem and SHV resistance genes, and nucleotide sequence method was used for genotyping of drug resistance genes. Results 69 strains of Shigella cephalosporin resistant to third generation were detected by BLAST analysis, which accounted for 74.19% of ESBLs Shigella. OXA and TEM resistant genes of CTX-M(CTX-M-1 group and CTX-M-9 group were detected. The detectable rate was 79.01% and 26.09%, respectively. There was no CTX-M-2 group and SHV resistance gene. The majority of CTX-M-1 group was CTX-M-15 type, and the other 7 other CTX-M-9 strains were CTX-M-14 type, and the positive rate was 79.01% and 26.09%, respectively, and the positive rate was 79.01% and 26.09%, respectively. Six strains of OXA and 18 strains of TEM resistance genes were identified as type 1, OXA-1 and TEM-1. 21 strains of Shigella with more than 2 resistant genes were found to be 30.43. Conclusion Shigella has a higher resistance rate to ceftriaxone and other three generations of cephalosporins. The CTX-M-15 type and CTX-M-14 type of CTX-MN CTX-M-1 group detected by ESBLs are mainstream enzyme types, and can carry multiple drug resistance genes at the same time, which makes it difficult to prevent and control diseases. OXA-1 type has a high carrying rate. It is suggested that we should strengthen the analysis of Shigella spp. B whether it is phenotypic resistance or the detection rate of resistance genes is higher than that of group D Shigella. This finding is helpful to the study of the spread and prevalence of Shigella.
