【摘要】：目的：調(diào)查惡性腫瘤患者的心理健康水平和生活質(zhì)量水平,探討心理干預(yù)對(duì)惡性腫瘤患者的作用并評(píng)價(jià)心理干預(yù)的效果。 方法：收集無(wú)錫市第四人民醫(yī)院腫瘤科2009年1月至2011年12月住院治療的惡性腫瘤患者共178名,并隨機(jī)分為心理干預(yù)組(89例)和對(duì)照組(89例)。入院一周內(nèi)兩組患者均填寫(xiě)癥狀自評(píng)量表(Symptom Checklist-90, SCL-90)及簡(jiǎn)明健康狀況調(diào)查表(36-item Short Form Health Survey, SF-36),以分別測(cè)評(píng)其心理健康水平和生活質(zhì)量水平；對(duì)照組給予常規(guī)護(hù)理,干預(yù)組在常規(guī)護(hù)理的基礎(chǔ)上給予支持性心理干預(yù)。治療一個(gè)月后,對(duì)患者及配偶心理健康水平及生活質(zhì)量進(jìn)行重新評(píng)價(jià),確定干預(yù)對(duì)惡性腫瘤患者的影響作用。 結(jié)果：①SCL-90量表測(cè)量結(jié)果提示,心理干預(yù)前干預(yù)組和對(duì)照組患者除軀體化和恐怖因子以外,其余各項(xiàng)均無(wú)顯著的統(tǒng)計(jì)學(xué)差異；心理干預(yù)后,干預(yù)組患者各項(xiàng)因子分均顯著低于對(duì)照組；干預(yù)組在心理干預(yù)后,除恐怖因子外,其余項(xiàng)目得分均顯著低于心理干預(yù)前：將心理干預(yù)前后對(duì)照組SCL-90評(píng)分情況進(jìn)行比較,干預(yù)前后有8項(xiàng)具有統(tǒng)計(jì)學(xué)差異。②SF-36量表結(jié)果提示,心理干預(yù)前干預(yù)組和對(duì)照組SF-36評(píng)分各維度均無(wú)統(tǒng)計(jì)學(xué)差異；心理干預(yù)后,干預(yù)組在6個(gè)維度的得分顯著高于對(duì)照組；支持性心理干預(yù)后,干預(yù)組在3個(gè)維度的得分顯著高于干預(yù)前；心理干預(yù)前后對(duì)照組SF-36得分除軀體疼痛得分顯著升高外,其余維度干預(yù)前后得分均無(wú)統(tǒng)計(jì)學(xué)差異。③SCL-90總分、陽(yáng)性癥狀均分、軀體化、抑郁和敵對(duì)五個(gè)項(xiàng)目的降低值與生活質(zhì)量總分增加值相關(guān)。 結(jié)論：心理干預(yù)能提高惡性腫瘤患者的心理健康水平和生活質(zhì)量水平；心理健康水平與生活質(zhì)量水平存在相關(guān)關(guān)系。
[Abstract]:Objective: to investigate the mental health and quality of life (QOL) of patients with malignant tumor, to explore the effect of psychological intervention on patients with malignant tumor and to evaluate the effect of psychological intervention. Methods: a total of 178 malignant tumor patients hospitalized from January 2009 to December 2011 in Department of Oncology of Wuxi 4th people's Hospital were collected. They were randomly divided into two groups: the psychological intervention group (89 cases) and the control group (89 cases). Within one week of admission, the two groups of patients filled out the symptom Checklist (SCL-90) and symptom Checklist-90. SCL-90 and 36 item Short Form Health Survey (SF-36). Their mental health and quality of life were measured respectively. The control group was given routine nursing, and the intervention group was given supportive psychological intervention on the basis of routine nursing. After one month of treatment, the mental health level and quality of life of the patients and their spouses were re-evaluated. To determine the effect of intervention on patients with malignant tumor. Results the results of the SCL-90 scale showed that there was no significant difference between the intervention group and the control group except somatization and phobic factors. After psychological intervention, the factor scores of the patients in the intervention group were significantly lower than those in the control group. After psychological intervention, the scores of other items in the intervention group were significantly lower than those before psychological intervention except for phobic factors. The SCL-90 scores of the control group before and after psychological intervention were compared. Before and after intervention, there were 8 items with statistical difference. 2SF-36 scale results showed that there was no significant difference in SF-36 scores between the intervention group and the control group before psychological intervention. After psychological intervention, the scores of six dimensions in the intervention group were significantly higher than those in the control group. After supportive psychological intervention, the scores of three dimensions in intervention group were significantly higher than those before intervention. Before and after psychological intervention, the scores of SF-36 in control group were significantly higher than those in control group except for somatic pain. There was no statistical difference in the scores of other dimensions before and after intervention. 3. The total score of SCL-90, the mean score of positive symptoms and somatization. Depression and hostility were associated with the increase in total quality of life (QOL) scores. Conclusion: psychological intervention can improve the level of mental health and quality of life of patients with malignant tumor, and there is a correlation between mental health and quality of life.
