【摘要】：唇腭裂是口腔頜面部最常見的先天性畸形,我國平均每生700個嬰兒中就有1個患唇腭裂。唇腭裂不僅嚴(yán)重影響面部美觀,還因口、鼻腔相通直接影響發(fā)育。嚴(yán)重的兔唇患兒會影響到吃奶,不能正常哺乳,會導(dǎo)致患兒營養(yǎng)不良�；純涸谏L發(fā)育期間學(xué)習(xí)說話時吐字不清,會影響到語言的發(fā)育。因此,探討唇腭裂的母源性高危因素,并以此指導(dǎo)唇腭裂發(fā)生的預(yù)防具有十分重要的意義。 本研究從實證的角度出發(fā),采用自編問卷、生活事件量表(LES)、特質(zhì)焦慮量表(T-AI)、特質(zhì)應(yīng)對方式問卷(NC)、C型行為量表(CB)和艾森克人格問卷,對唇腭裂發(fā)生的母源性高危因素進(jìn)行探討,為孕期保健指導(dǎo)提供依據(jù),以有效預(yù)防唇腭裂的發(fā)生。 結(jié)果:綜合測評結(jié)果顯示,研究組與對照組相比,生活事件量表負(fù)性事件緊張總值、C型行為量表的焦慮、抑郁因子均高于正常對照組,且差異性具有統(tǒng)計學(xué)意義(P0.05);而生活事件量表正性事件緊張總值、特質(zhì)焦慮量表和C型行為量表的理智、樂觀和社會支持因子得分均顯低于對照組,且具有顯著統(tǒng)計學(xué)差異(P0.05)。為探討懷孕期間誘發(fā)唇腭裂的有關(guān)危險因素,運用logistic逐步回歸分析對危險因素進(jìn)行篩選,最終進(jìn)入模型的危險因素有5個,分別是學(xué)歷、懷孕期間體重的變化、奶和豆的攝入量、保健品的服用及是否補(bǔ)充過葉酸。 結(jié)論:1.孕期婦女具有高程度的焦慮、抑郁,負(fù)性事件刺激量,消極應(yīng)對方式,人格方面神經(jīng)質(zhì)及掩飾性強(qiáng),可能引發(fā)唇腭裂。2.孕期婦女具有積極應(yīng)對方式,正性事件刺激量,理智,樂觀及高度的社會支持,則可能降低唇腭裂的發(fā)生率。3.孕期婦女的學(xué)歷、是否補(bǔ)充過葉酸、保健品的服用、奶和豆的攝入量及懷孕期間體重的變化均為產(chǎn)生唇腭裂的重要因素。
[Abstract]:Cleft lip and palate is the most common congenital malformation in oral and maxillofacial region. Cleft lip and palate not only seriously affect facial beauty, but also cause cleft lip and palate. Nasal communication directly affects the development. Severe children with harelip will affect milk, can not normal lactation, will lead to malnutrition. Children learn to speak in the growth and development of unclear words. Therefore, it is of great significance to explore the maternal high risk factors of cleft lip and palate and to guide the prevention of cleft lip and palate. From an empirical point of view, this study adopts a self-designed questionnaire, a life event scale, a trait anxiety scale and a trait coping style questionnaire. Type C behavior scale (CBB) and Eysenck Personality questionnaire (Eysenck Personality questionnaire) were used to investigate the maternal risk factors of cleft lip and palate, and to provide the basis for health care during pregnancy, in order to prevent cleft lip and palate effectively. Results: compared with the control group, the anxiety and depression factors of the life events scale were significantly higher in the study group than in the normal control group. The difference was statistically significant (P 0.05). The scores of positive event stress total, trait anxiety scale and C-type behavior scale were significantly lower than those of the control group. In order to investigate the risk factors of cleft lip and palate during pregnancy, logistic stepwise regression analysis was used to screen the risk factors. There were five risk factors for eventual entry into the model, including academic qualifications, changes in body weight during pregnancy, intake of milk and beans, use of health products and supplementation of folic acid. Conclusion 1. Pregnant women have high levels of anxiety, depression, negative event stimulation, negative coping style, and strong neuroticism and concealment in personality. May cause cleft lip and palate .2. pregnant women have positive coping style, positive event stimulation, rational, optimistic and high social support, may reduce the incidence of cleft lip and palate. Supplementation of folic acid, use of health products, intake of milk and beans, and changes in body weight during pregnancy were all important factors for cleft lip and palate.
