ESR及CRP在兔脊柱結(jié)核模型構(gòu)建中的變化規(guī)律研究

【摘要】:目的觀察紅細(xì)胞沉降率(ESR)及C反應(yīng)蛋白(CRP)在新西蘭兔脊柱結(jié)核模型構(gòu)建中的變化規(guī)律。方法45只新西蘭兔隨機(jī)分為造模組(n=25)、對照組(n=20)。造模組于腰4、5椎間隙鉆孔,填入明膠海綿,在其中浸注H37Rv結(jié)核標(biāo)準(zhǔn)菌株混懸液0.1 ml;對照組浸注生理鹽水0.1 ml。觀察實(shí)驗(yàn)兔一般狀況,并通過影像學(xué)、病理學(xué)、微生物學(xué)評估造模情況;觀察兩組實(shí)驗(yàn)兔術(shù)前、術(shù)后(1、3、5、7、10、14、28、42、56 d)不同時間點(diǎn)ESR及CRP的變化規(guī)律;探討ESR及CRP與兔脊柱結(jié)核感染的相關(guān)性。結(jié)果術(shù)后56 d造模組21只實(shí)驗(yàn)兔最終完成實(shí)驗(yàn),對照組為18只。通過影像學(xué)、病理學(xué)、微生物學(xué)評估:造模組脊柱結(jié)核造模感染陽性率為76.19%(16/21),造模陽性兔均至少符合一項(xiàng)造模陽性判定標(biāo)準(zhǔn);造模組另5只造模陰性兔及對照組實(shí)驗(yàn)兔均無陽性發(fā)現(xiàn)。動態(tài)觀察兩組ESR、CRP的變化規(guī)律:造模組陽性兔體內(nèi)的ESR、CRP變化規(guī)律與造模組陰性兔明顯不同,而造模組陰性兔體內(nèi)的ESR、CRP變化規(guī)律與對照組相似;造模組陽性兔術(shù)后ESR及CRP迅速升高,ESR術(shù)后第7天達(dá)到高峰,CRP術(shù)后5 d達(dá)高峰,至術(shù)后42 d已趨于平穩(wěn),術(shù)后56 d處死動物時ESR及CRP仍維持在較高水平;造模組陰性兔及對照組實(shí)驗(yàn)兔ESR在術(shù)后第7天達(dá)高峰,此后逐漸下降,至術(shù)后42 d已降至術(shù)前水平,CRP術(shù)后第3天達(dá)高峰后逐漸下降,至術(shù)后28 d降至術(shù)前水平;造模組陽性兔ESR、CRP達(dá)高峰時所測值及實(shí)驗(yàn)結(jié)束時(56 d)所測值均高于造模組陰性兔和對照組同時間點(diǎn)所測值(P0.05);繪制術(shù)后56 d時ESR、CRP與結(jié)核造模感染相關(guān)性的ROC曲線,曲線下面積:ESR為0.953,CRP為0.927,均顯著大于0.5,以Youden指數(shù)最大的切點(diǎn)為最佳篩選點(diǎn)計(jì)算:ESR最佳篩選點(diǎn)為7.5 mm/h,CRP最佳篩選點(diǎn)為5 527.50μg/L。結(jié)論ESR及CRP可反映模型兔體內(nèi)結(jié)核菌感染情況及病情進(jìn)展,造模術(shù)后56 d檢測值可作為篩選兔脊柱結(jié)核動物模型的可靠指標(biāo)。
[Abstract]:Objective To observe the changes of erythrocyte sedimentation rate (ESR) and C reactive protein (CRP) in the construction of the spinal tuberculosis model in New Zealand rabbits. Methods 45 New Zealand rabbits were randomly divided into the model group (n=25) and the control group (n=20). The model group was bored in the 4 and 5 lumbar intervertebral spaces and filled with gelatin sponge, in which the suspension of H37Rv tuberculosis standard strain was immersed in 0.1 ml, and the control group was injected with saline 0.1 ml. Observe the general condition of rabbits, and by imaging, pathology, microbiology evaluation model; observed the rabbits in two groups before and after operation (1, 3, 5, 7, 10, 14, 28, 42, 56 d) changes at different time points of ESR and CRP on ESR and CRP; correlation with spinal tuberculosis infection. Results the experiment was completed in 21 rabbits in 56 D model group after operation, and 18 in the control group. Through imaging, pathology and microbiology evaluation, the positive rate of model infection in spinal tuberculosis was 76.19% (16/21), and the positive rabbits were all at least in accordance with a positive standard of modeling.
