【摘要】:目的:探討兔顳下頜關(guān)節(jié)(TMJ)關(guān)節(jié)突缺損模型建立后實(shí)現(xiàn)關(guān)節(jié)置換的方法,闡明碳纖維增強(qiáng)型聚醚醚酮(CFR-PEEK)材料制作的TMJ假體的置換修復(fù)效果,為臨床應(yīng)用CFR-PEEK人造關(guān)節(jié)置換TMJ提供實(shí)驗(yàn)依據(jù)。方法:13只健康成年日本大耳白兔按照隨機(jī)數(shù)字法分為實(shí)驗(yàn)組(n=6)、陽性對照組(n=4)和陰性對照組(n=3)。實(shí)驗(yàn)組兔切除右側(cè)TMJ關(guān)節(jié)突,建立關(guān)節(jié)缺損模型,同期植入CFR-PEEK人造關(guān)節(jié);陽性對照組兔切除右側(cè)TMJ關(guān)節(jié)突,建立關(guān)節(jié)缺損模型,不植入CFR-PEEK人造關(guān)節(jié);陰性對照組兔不做任何處理。通過CT影像學(xué)檢查評估人造關(guān)節(jié)植入后的固位和替代修復(fù)效果。連續(xù)記錄13周各組兔體質(zhì)量并進(jìn)行比較。結(jié)果:建立的兔TMJ關(guān)節(jié)突缺損模型能夠成功植入CFR-PEEK人造關(guān)節(jié)。CT影像學(xué)檢查,經(jīng)三維重建和體外顯影處理后可見植入CFR-PEEK人工關(guān)節(jié)固位良好,代替正常關(guān)節(jié)發(fā)揮作用;13周兔體質(zhì)量觀察,與陽性對照組比較,實(shí)驗(yàn)組兔體質(zhì)量增加(P0.05);實(shí)驗(yàn)組和陰性對照組兔體質(zhì)量均明顯增加,但2組比較差異無統(tǒng)計(jì)學(xué)意義(P0.05)。實(shí)驗(yàn)組和陰性對照組兔體質(zhì)量均正常增長,陽性對照組兔體質(zhì)量增長較為緩慢,甚至停止增長或者負(fù)增長。結(jié)論:植入CFR-PEEK人造關(guān)節(jié)修復(fù)關(guān)節(jié)突缺損能夠有效恢復(fù)兔正常咀嚼功能,CFR-PEEK人造關(guān)節(jié)成功應(yīng)用于TMJ缺損修復(fù),有望成為一種重建TMJ關(guān)節(jié)突的理想材料。
[Abstract]:Objective: To investigate the temporomandibular joint (TMJ) method to achieve the establishment of facet joint replacement defect model, clarify the carbon fiber reinforced polyetheretherketone (CFR-PEEK) replacement repair effect of TMJ prosthesis materials, and provide the experimental basis for the clinical application of CFR-PEEK artificial joint replacement TMJ. Methods: 13 healthy adult Japanese white rabbits were divided into experimental group (n=6), positive control group (n=4) and negative control group (n=3) according to the random number method. In the experimental group, the right TMJ articular process was removed, and a joint defect model was established. CFR-PEEK artificial joint was implanted at the same time. The right TMJ joint process was removed in the positive control group, and the joint defect model was established, without CFR-PEEK artificial joint, and the negative control group did not do any treatment. CT imaging examination was used to evaluate the retention and replacement effect of artificial joint implantation. The mass of rabbits in each group was recorded for 13 weeks and compared. Results: the model of rabbit TMJ joint defect could be successfully implanted into the artificial joint of CFR-PEEK. CT imaging, the 3D reconstruction and in vitro after developing CFR-PEEK artificial joint implants visible good retention, instead of the normal joint function; 13 weeks of observation Tuti quality, compared with the positive control group, the experimental group increased body weight (P0.05); experimental group and negative control group rabbits body weight were significantly increased. But the difference between the 2 groups was not statistically significant (P0.05). The quality of the rabbits in the experimental group and the negative control group increased normally. The growth of the rabbit body in the positive control group was slower, even stopped growth or negative growth. Conclusion: implantation of CFR-PEEK artificial joint to repair the joint defect can effectively restore the normal masticatory function of rabbits. CFR-PEEK artificial joint has been successfully applied to TMJ defect repair, and is expected to become an ideal material for reconstruction of TMJ articular process.
