文章摘要：目的評(píng)價(jià)西醫(yī)常規(guī)治療基礎(chǔ)上中藥保留灌腸治療肝性腦病的療效。方法計(jì)算機(jī)檢索Cochrane圖書(shū)館、Pub Med、EMbase、維普、中國(guó)知網(wǎng)、中國(guó)生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫(kù)和萬(wàn)方數(shù)據(jù)庫(kù)中關(guān)于西醫(yī)常規(guī)治療基礎(chǔ)上中藥保留灌腸（試驗(yàn)組）對(duì)比西醫(yī)常規(guī)治療（對(duì)照組）治療肝性腦病的隨機(jī)對(duì)照試驗(yàn)（RCT）,并追溯納入文獻(xiàn)的參考文獻(xiàn),檢索時(shí)限為各數(shù)據(jù)庫(kù)從建庫(kù)至2013年11月。由2名研究者按照文獻(xiàn)納入和排除標(biāo)準(zhǔn)獨(dú)立進(jìn)行文獻(xiàn)篩選、資料提取、質(zhì)量評(píng)價(jià)并交叉核對(duì)后,采用Rev Man 5.1軟件進(jìn)行Meta分析。結(jié)果最終納入26個(gè)RCT,共計(jì)1 691例患者。根據(jù)干預(yù)時(shí)間的長(zhǎng)短進(jìn)行亞組分析,7 d為1個(gè)療程。Meta分析結(jié)果顯示：干預(yù)時(shí)間≤1個(gè)療程,試驗(yàn)組的總有效率高于對(duì)照組,兩組差異有統(tǒng)計(jì)學(xué)意義[RR=1.34,95%CI（1.25,1.44）,P〈0.000 01];干預(yù)時(shí)間〉1個(gè)療程,試驗(yàn)組的總有效率高于對(duì)照組,兩組差異有統(tǒng)計(jì)學(xué)意義[RR=1.34,95%CI（1.21,1.48）,P〈0.000 01]。結(jié)論西醫(yī)常規(guī)治療基礎(chǔ)上中藥保留灌腸對(duì)于治療肝性腦病有顯著療效。由于納入分析的研究質(zhì)量差異較大,因此有必要進(jìn)行大樣本、多中心、隨機(jī)、雙盲的對(duì)照試驗(yàn),以進(jìn)一步證實(shí)其臨床療效。

Abstr：Objective To evaluate the efficacy of western conventional treatment based on traditional Chinese medicine retention enema on patients with hepatic encephalopathy.Methods Such databases as the Cochrane Library,PubMed,EMbase,VIP,CNKI,CBM and WanFang Database were searched from the inception to November 2013 to collect randomized controlled trials（RCTs） about Western conventional treatment based on traditional Chinese medicine retention enema to treat hepatic encephalopathy,and the references of the included literature were also retrieved.Two reviewers independently screened the literature according to the inclusion and exclusion criteria,extracted the data,and evaluated and cross-checked the methodological quality.Then meta-analysis was conducted using RevMan 5.1 software.Results A total of 26 RCTs involving 1 691 patients were included.The subgroup analyses based on the length of intervention time showed that when the course of treatment intervention was less than one,the overall effective rate of the Chinese herbal retention enema group was higher than that of the control group,with a significant difference[RR=134,95%CI（1.25,1.44）,P〈0.000 01];after more than one course of treatment intervention,the overall effective rate of the Chinese herbal retention enema group was higher than that of the control group,with a significant difference[RR=134,95%CI（1.21,1.48）,P〈0.000 01].Conclusions On the basis of available evidence,the western medicine treatment based on traditional Chinese medicine retention enema for hepatic encephalopathy has a certain effect.However,the heterogeneity among the included studies is large.It is necessary to design multicenter,strictly randomized and doubleblind test controlled trials with large samples to validate these conclu
