【摘要】：人與人之間的交流互動是人類生活的基本形式,人們出于各種各樣的目的進行交流互動。醫(yī)患溝通作為最具有代表性的互動行為,與人們生活息息相關(guān),但很少有學(xué)者從社會學(xué)角度進行分析解。本文借助美國社會學(xué)家戈夫曼互動理論,對醫(yī)患會話進行深入研究。研究對象是醫(yī)生與病人會診時的對話,具體研究方法是通過搜集門診會話錄音進行轉(zhuǎn)寫,翻譯；以戈夫曼的互動理論為框架進行分析,考察分析醫(yī)生病人會話中的互動理論的體現(xiàn)形式及其反應(yīng)出的醫(yī)患溝通中存在的問題。 主要參照戈夫曼的三個重要理論觀點：擬劇論、游戲論和儀式論對搜集到的醫(yī)患會話進行分析。經(jīng)過研究發(fā)現(xiàn)：(1)在農(nóng)村的家庭診所中,醫(yī)生與病人在會話過程中,需在熟人、醫(yī)患兩組身份平臺上進行轉(zhuǎn)換。(2)不同場合下,病人在面對醫(yī)生時采取不同的印象管理策略。(3)醫(yī)患雙方在醫(yī)學(xué)信息,機構(gòu)身份不對稱的情況下,表現(xiàn)出對社會服務(wù)互動原則不同的遵循情況。醫(yī)生常違反平等與禮貌原則,以居高臨下的姿態(tài)命令病人；而病人則比較尊重和順從醫(yī)生的安排即使在威脅到面子的情況下,也不愿采取反抗行為。結(jié)合實際的研究結(jié)論,本文提出了三條建議以供醫(yī)院醫(yī)生參考,構(gòu)建更加良好的醫(yī)患會話模式,促進醫(yī)患平等、和諧溝通。
