【摘要】：目的:本研究希望通過整理古代文獻(xiàn),整理古籍中選用期門穴的相關(guān)條文,以探討其主治病癥,取穴依據(jù)和取穴時(shí)機(jī),了解其診治思路,并期望總結(jié)運(yùn)用期門穴的規(guī)律和特色,探討其在臨床上的應(yīng)用。此外,本研究也希望透過搜索近代中外文獻(xiàn),進(jìn)行統(tǒng)計(jì)與分析,了解期門穴在現(xiàn)代應(yīng)用的情形,對(duì)照古今主治病癥,提升期門穴臨床應(yīng)用更開闊更深入的可能性。本研究還整理了期門穴的穴名、定位、刺灸法、針灸注意事項(xiàng),安全性及其防治針灸意外等內(nèi)容,為針灸適宜病癥研究提供腧穴研究的示范,為期門穴臨床應(yīng)用和科研提供理論依據(jù)。研究方法與步驟:以文獻(xiàn)研究法為主,佐以統(tǒng)計(jì)分析法。古代文獻(xiàn)主要查自湖南電子音像出版社所出版的《中華醫(yī)典》第五版電子叢書。本版中華醫(yī)典是目前搜集最完整的版本。資料搜集時(shí)按期門穴和肝募穴名稱進(jìn)行檢索、摘錄。通過計(jì)算機(jī)檢索及人工閱讀,搜集期門相關(guān)的古代文獻(xiàn)條文后,根據(jù)訂定文獻(xiàn)條文的納入標(biāo)準(zhǔn)和條文排除標(biāo)準(zhǔn)將文獻(xiàn)條文錄入EXCEL數(shù)據(jù)庫。先將文獻(xiàn)內(nèi)容根據(jù)單穴主治和配伍主治分成二大類,再進(jìn)一步按照臨床適宜病癥中醫(yī)內(nèi)科、外科、婦科、兒科、眼科、耳鼻喉科等進(jìn)行統(tǒng)計(jì)歸類;接著運(yùn)用頻數(shù)統(tǒng)計(jì)法對(duì)期門穴治療病癥進(jìn)行統(tǒng)計(jì)�，F(xiàn)代文獻(xiàn)主要從中國知網(wǎng),萬方數(shù)據(jù)庫,維普期刊網(wǎng)中和Google學(xué)術(shù)搜尋查取。其中重復(fù)的臨床文獻(xiàn)研究做了統(tǒng)一篩選。同樣的將文獻(xiàn)內(nèi)容先根據(jù)單穴主治和配伍主治分類,再進(jìn)一步按照臨床適宜病癥分類整理。為了確保檢索的全面性,根據(jù)不同的數(shù)據(jù)庫特點(diǎn)采用關(guān)鍵詞檢索,主要包括:期門穴,肝募穴,肝膽病,文獻(xiàn)研究,適應(yīng)病等。檢索詞的選擇來源主要有國家規(guī)劃教材《針灸學(xué)》、《刺法灸法學(xué)》等。英文檢索詞采用"Qimen","literature review","Liver-Mu point"。成果:經(jīng)過歸納整理,總結(jié)出古代文獻(xiàn)中期門穴適宜病癥。結(jié)果:在古代文獻(xiàn)中,期門單穴治療病癥共計(jì)62種,腧穴配伍治療病癥共計(jì)62種,除去重復(fù)的21種病癥;最后得出期門穴在古代醫(yī)書中的總適宜病癥103種。而在現(xiàn)代文獻(xiàn)中,應(yīng)用期門穴單穴治療病癥共計(jì)3種,腧穴配伍治療病癥共計(jì)24種,除去重復(fù)的1種病癥;最后得出期門穴在現(xiàn)代臨床應(yīng)用的適宜病癥為26種。對(duì)照古今期門適應(yīng)癥病可以發(fā)現(xiàn),古代文獻(xiàn)適應(yīng)病癥多達(dá)100種,現(xiàn)代臨床應(yīng)用搜集到的有26種,大多集中在消化系統(tǒng)的肝膽腸胃系疾病以及婦科疾病�？芍P(guān)于期門穴主治應(yīng)用仍有很大發(fā)展的空間。
