【摘要】：目的探討呼吸系統(tǒng)粘液表皮樣癌的臨床特征及診治現(xiàn)狀。方法對(duì)2016年在浙江大學(xué)醫(yī)學(xué)院附屬邵逸夫醫(yī)院收治的1例呼吸系統(tǒng)粘液表皮樣癌患者并復(fù)習(xí)國(guó)內(nèi)外相關(guān)文獻(xiàn),通過(guò)收集的共7個(gè)病例對(duì)該疾病進(jìn)行回顧性分析。結(jié)果本組患者的中位年齡40歲,年齡跨度為6-71歲,除2個(gè)患者為6歲兒童,1個(gè)患者為22歲青年外,其他均為成人且年齡大于40歲(57%)。7個(gè)病例中,男5例,女2例。本組5個(gè)病例有精確的腫塊大小記載,平均最大直徑為3.0cm。腫塊位于右肺的有5例,左肺的有1例,累及左肺上下兩葉,氣管內(nèi)1例。本組Ⅰ期患者4例,其中2例因淋巴結(jié)狀態(tài)未知而不能肯定。Ⅱ期患者1例。111期患者1例為T(mén)4N2M0。本組6例可手術(shù)患者均行手術(shù)治療,均為根治性手術(shù),術(shù)后所有患者均未行輔助治療,僅有1例晚期患者無(wú)法手術(shù),予姑息放化療治療。本組患者無(wú)病生存期均超過(guò)隨訪的6-12個(gè)月。本文報(bào)道的患者自行氣管腫瘤切除術(shù)以來(lái)存活已超過(guò)12個(gè)月。所有7位患者都無(wú)吸煙史,可以大致推斷吸煙不是呼吸系統(tǒng)粘液表皮樣癌的危險(xiǎn)因素。結(jié)論呼吸系統(tǒng)粘液表皮樣癌大氣道的侵犯非常典型,多數(shù)患者可表現(xiàn)為咳嗽、咯血、呼吸困難、發(fā)熱。術(shù)前診斷困難,容易誤診,特別是僅僅通過(guò)穿刺的診斷與肺腺癌很難區(qū)分。其確診需手術(shù)后病理結(jié)果證實(shí)。手術(shù)是治療本病最主要的方法,晚期的輔助治療可聯(lián)系放療及化療�？墒中g(shù)患者根治術(shù)后預(yù)后顯著好于其他不可手術(shù)切除的患者。
[Abstract]:Objective to investigate the clinical features, diagnosis and treatment of mucoepidermoid carcinoma of respiratory system. Methods A case of mucoepidermoid carcinoma of the respiratory system was treated in the affiliated run Shaw Hospital of Zhejiang University Medical College in 2016 and the related literatures were reviewed and analyzed retrospectively through the collection of 7 cases. Results the median age of the patients was 40 years, the age span ranged from 6 to 71 years old. Except 2 patients were 6 years old children and 1 patient was 22 years old young people, all the patients were adults and over 40 years old (57%). Among the 7 cases, 5 cases were males. Female 2 cases. The mean maximum diameter of the tumor was 3.0 cm.
