【摘要】：目的:通過檢測Hcy,同時檢測AT-III及DD含量在AL患者化療前后改變情況,研究Hcy與AT-III、DD之間的相關(guān)性,探討Hcy在AL合并易栓癥中的意義。方法:AL組共30例,自2015年11月至2017年2月在青海大學附屬醫(yī)院診斷初治AL患者;其中包括23例AML(非M3)和7例ALL。AL組根據(jù)化療后結(jié)局,分為兩組:AL緩解組20例;AL未緩解組:10例。對照組30例,為健康體檢者。檢測AL組化療前、經(jīng)標準方案化療后(恢復(fù)期)Hcy、AT-III和DD表達水平;結(jié)合文獻發(fā)現(xiàn)上述指標與高腫瘤負荷可能有一定聯(lián)系,因此我們檢測化療前LDH含量及血常規(guī),收集記錄WBC;對照組于就診時檢測Hcy、AT-III及DD。應(yīng)用SPSS21.0進行數(shù)據(jù)的統(tǒng)計分析,各組間比較采用單因素方差分析;同組化療前后比較采用配對樣本t檢驗;指標間相關(guān)性為直線相關(guān)分析。檢驗水準α=0.05。結(jié)果:1.AL組化療前Hcy、DD表達水平均較對照組高,但AT-III含量較對照組低。2.化療前AL未緩解組Hcy、DD含量較緩解組高,而AT-III較緩解組低。3.化療后AL緩解組Hcy、DD含量較化療前低,而AT-III含量較化療前高;AL未緩解組化療后Hcy、AT-Ⅲ、DD含量較化療前無明顯差異。4.在化療前AL患者中,Hcy與AT-III兩者存在直線相關(guān)關(guān)系,為負相關(guān)(r=-0.419,P=0.021);Hcy與DD兩者存在直線相關(guān)關(guān)系,為正相關(guān)(r=0.612,P=0.000);Hcy與LDH兩者存在直線相關(guān)關(guān)系,為正相關(guān)(r=0.407,P=0.025);Hcy與WBC不存在相關(guān)性(r=0.340,P=0.066)。結(jié)論:1.AL患者化療前Hcy、AT-III、DD表達異常,并發(fā)易栓癥的風險增加。2.AL患者化療前Hcy水平分別與AT-Ⅲ、DD和LDH存在線性相關(guān)關(guān)系。3.AL未緩解組化療前后Hcy、AT-Ⅲ及DD水平變化不明顯,仍應(yīng)加強對AL化療后未緩解患者合并易栓癥的重視;而AL緩解組化療后Hcy、AT-Ⅲ及DD恢復(fù)正常水平,合并易栓癥的風險逐漸降低。
[Abstract]:Objective: to study the correlation between Hcy and AT-IIIN DD by detecting the changes of AT-III and DD in AL patients before and after chemotherapy, and to explore the significance of Hcy in AL patients with thrombotic disease. Methods from November 2015 to February 2017, 30 patients with primary AL were diagnosed in the affiliated Hospital of Qinghai University, including 23 AMLs (non-M3) and 7 ALL.AL patients. According to the outcome of chemotherapy, they were divided into two groups: 20 cases of non-remission group (n = 20) and 10 cases of non-remission group (n = 10). The control group (30 cases) was healthy. Before and after chemotherapy, the expression levels of Hcyl AT-III and DD were detected in AL group (convalescent stage), and combined with the literature, it was found that the above indexes might be related to high tumor load, so we detected LDH content and blood routine before chemotherapy. The records of WBCwere collected and the control group was examined for Hcytin AT-III and DD. SPSS 21.0 was used to carry out statistical analysis of data, single factor ANOVA was used for comparison among groups, matched sample t test was used for comparison before and after chemotherapy in the same group, and linear correlation analysis was used among the indexes. The test level is 偽 0. 05. Results 1. The expression of Hcyn DD in AL group was higher than that in control group before chemotherapy, but AT-III was lower than that in control group. Before chemotherapy, the level of Hcyn DD in AL without remission group was higher than that in remission group, but AT-III was lower than that in remission group. After chemotherapy, the content of Hcytin DD in AL remission group was lower than that before chemotherapy, and the content of AT-III was higher than that before chemotherapy.
