【摘要】：目的:以手術(shù)發(fā)現(xiàn)和病理結(jié)果為診斷金標(biāo)準(zhǔn),探討高頻彩色多普勒超聲(CDFI)在急性足踝部外傷性病變的應(yīng)用。方法:對2013年1月~2015年9月期間,在新疆醫(yī)科大學(xué)第二附屬醫(yī)院經(jīng)住院及門診就診的56位77例臨床疑診為足及踝部急性外傷性病變患者行高頻彩超檢查,觀察其聲像圖改變及特征,并以手術(shù)及病理結(jié)果為金標(biāo)準(zhǔn),評價(jià)高頻彩超對足及踝部軟組織外傷性病變的診斷能力,其中50例疑似為韌帶、肌腱、神經(jīng)、關(guān)節(jié)等軟組織損傷病例同時(shí)行MRI檢查,比較高頻彩超與MRI對足踝部軟組織外傷性病變的檢查結(jié)果;另外21例疑診合并有足踝部骨折病例同時(shí)行X-Ray檢查,對高頻彩超與X-Ray對足踝部骨折診斷一致性進(jìn)行評價(jià)。結(jié)果:高頻彩超對50例足及踝部軟組織外傷性病變診斷的靈敏度、特異度、陽性似然比、陰性似然比、陽性預(yù)測值、陰性預(yù)測值分別為70.7%、77.8%、3.18、0.38、93.5%、36.8%;高頻彩超與MRI對足及踝部軟組織損傷的檢查結(jié)果及分別對韌帶、肌腱、神經(jīng)及關(guān)節(jié)損傷的檢查結(jié)果比較,無明顯統(tǒng)計(jì)學(xué)差異(P0.05)。高頻彩超與X-Ray對21例足踝部骨折診斷一致性評價(jià)比較,Kappa值=0.871(一致性極佳)。結(jié)論:高頻彩色多普勒超聲對足及踝部急性外傷性病變具有較高的診斷價(jià)值,可為臨床醫(yī)師診治足踝部急性外傷提供客觀的影像學(xué)診斷依據(jù),并作為足及踝部外傷后常規(guī)的影像學(xué)檢查方法。
[Abstract]:Objective: to investigate the application of high frequency color Doppler ultrasound (CDFI) in the diagnosis of acute traumatic foot and ankle lesions. Methods: from January 2013 to September 2015, a total of 56 patients with suspected acute traumatic foot and ankle lesions in the second affiliated Hospital of Xinjiang Medical University were examined by high frequency color Doppler ultrasonography. To evaluate the diagnostic ability of high frequency color ultrasound for traumatic lesions of soft tissue of foot and ankle, 50 cases were suspected to be ligaments, tendons and nerves. MRI examination was performed in all cases of soft tissue injury such as joint, and the results of high-frequency color Doppler ultrasonography and MRI in detecting traumatic lesions of soft tissue of ankle were compared, and X-Ray examination was performed in 21 cases with suspected fracture of ankle and foot at the same time. To evaluate the consistency of high frequency color Doppler ultrasound and X-ray in diagnosis of ankle fracture. Results: the sensitivity, specificity, positive likelihood ratio, negative likelihood ratio, positive predictive value of high frequency color Doppler ultrasound in the diagnosis of 50 cases of traumatic lesions of soft tissue of foot and ankle were analyzed. The negative predictive values were 70.7 / 77.8 and 3.18 / 0.38 / 93.5 respectively. There was no significant difference between high frequency color Doppler ultrasonography and MRI in detecting soft tissue injury of foot and ankle, and in ligament, tendon, nerve and joint injury. Evaluation of diagnostic consistency of 21 cases of foot and ankle fractures with high frequency color Doppler ultrasound and X-Ray the Kappa value was 0.871 (excellent consistency). Conclusion: high frequency color Doppler ultrasound has high diagnostic value for acute traumatic lesions of foot and ankle, and can provide objective imaging diagnostic basis for clinicians in the diagnosis and treatment of acute trauma of foot and ankle. And as a routine imaging examination method after foot and ankle trauma.
