【摘要】：目的及意義耳鳴、耳聾是耳鼻喉科常見(jiàn)病,中醫(yī)治療耳鳴耳聾具有一定優(yōu)勢(shì)。中醫(yī)學(xué)對(duì)耳鳴耳聾的認(rèn)識(shí)歷史悠久,治療方式多樣,極具特色�，F(xiàn)代中醫(yī)學(xué)對(duì)耳鳴耳聾的研究,或是理法方藥單獨(dú)討論,不夠全面系統(tǒng);或是試驗(yàn)研究,參差不齊;或是實(shí)驗(yàn)驗(yàn)證,偏于論證。所以對(duì)古代文獻(xiàn)中耳鳴耳聾的學(xué)術(shù)源流進(jìn)行梳理,可以為臨床提供理論與實(shí)踐指導(dǎo)。研究方法主要采用傳統(tǒng)文獻(xiàn)學(xué)研究方法、斷代史研究法、歸納總結(jié)法及比較研究法。首先,手檢與機(jī)檢相結(jié)合,通過(guò)《中國(guó)中醫(yī)古籍總目》及《中華醫(yī)典》初步篩選搜集耳鳴耳聾相關(guān)古代文獻(xiàn)資料。然后從先秦兩漢時(shí)期到清代,從史學(xué)角度整理不同時(shí)期的文獻(xiàn)資料,從時(shí)間上把握其源流及發(fā)展。著重探討總結(jié)各個(gè)時(shí)期的學(xué)術(shù)發(fā)展特點(diǎn)把握其傳承發(fā)展關(guān)系。最后,對(duì)相關(guān)問(wèn)題進(jìn)行討論,必要時(shí)須借助史學(xué)、文化學(xué)等領(lǐng)域的研究方法和內(nèi)容。研究成果及意義1.系統(tǒng)梳理先秦至清代醫(yī)籍中耳鳴耳聾的學(xué)術(shù)源流首先,對(duì)耳鳴耳聾的定義及病名歷史沿革進(jìn)行探討。其次,分五個(gè)時(shí)期介紹先秦到清代對(duì)耳鳴耳聾的認(rèn)識(shí)。先秦至兩漢時(shí)期是中醫(yī)理論的奠基時(shí)期,經(jīng)典醫(yī)籍尤其是《黃帝內(nèi)經(jīng)》對(duì)耳鳴耳聾的論述一直為后世沿用。這個(gè)時(shí)期,耳鳴耳聾的病因病機(jī)及治法基本成型,病機(jī)主要從邪氣、臟腑、經(jīng)脈、陰陽(yáng)、氣血、運(yùn)氣等方面論述。治法以針刺為主,針?biāo)幗Y(jié)合。晉唐時(shí)期,對(duì)病機(jī)的探討開(kāi)始深入但仍以風(fēng)邪與腎虛為主,孫思邈從臟腑寒熱虛實(shí)角度進(jìn)行辨證,提出毒聾之說(shuō)。治法則以針刺和外治法為主,關(guān)注養(yǎng)生導(dǎo)引術(shù),形式內(nèi)容豐富。宋金元時(shí)期,醫(yī)分門戶。劉河間從熱證論述,提出水衰火實(shí)之說(shuō),發(fā)耳聾治肺之先聲;李東垣強(qiáng)調(diào)脾胃的重要性;朱丹溪倡滋陰降火之論,論治雜病的學(xué)術(shù)思想豐富了耳鳴耳聾的病機(jī)。此外,出現(xiàn)了陳無(wú)擇的三因分類和嚴(yán)用和的從心腎立論。這一時(shí)期內(nèi)治外治并用,方藥內(nèi)容豐富,是一個(gè)創(chuàng)新發(fā)展的時(shí)期。明清時(shí)期,醫(yī)家承金元之說(shuō)對(duì)耳鳴耳聾病因病機(jī)進(jìn)行深入探討,溫病學(xué)派立論無(wú)關(guān)少陽(yáng),治療更加重視顧護(hù)正氣,耳鳴耳聾病因病機(jī)系統(tǒng)逐漸完善。同時(shí),西學(xué)東漸促使中西醫(yī)匯通派產(chǎn)生,對(duì)耳鳴耳聾的認(rèn)識(shí)更加客觀理性。2.對(duì)耳鳴耳聾的一些相關(guān)理論進(jìn)行探討目前國(guó)內(nèi)從肺、三焦及膽論治耳鳴耳聾的醫(yī)家還很少,但臨床療效明顯。從肺治耳聾、從三焦及膽立論治療耳鳴耳聾的源流頗為久遠(yuǎn),對(duì)其進(jìn)行探討,可以為臨床實(shí)踐提供幫助。目前對(duì)耳鳴耳聾相關(guān)古籍的引用有些是不正確的,所以需要進(jìn)行追本溯源,對(duì)其進(jìn)行糾正。另外,對(duì)于耳鳴耳聾腎虛的病因病機(jī)多數(shù)醫(yī)家以補(bǔ)為主,但古人早已提出耳鳴慎補(bǔ),耳聾忌純用補(bǔ)澀劑的觀點(diǎn),并且古籍中記載的關(guān)于耳鳴耳聾的養(yǎng)生導(dǎo)引術(shù)對(duì)于預(yù)防和治療耳鳴耳聾有很好的作用。筆者在梳理文獻(xiàn)資料時(shí)發(fā)現(xiàn)一些現(xiàn)代教材未論述或極少論述的內(nèi)容,遂對(duì)其中部分內(nèi)容進(jìn)行整理。通過(guò)對(duì)耳鳴耳聾古代文獻(xiàn)的梳理可以讓人們對(duì)本病的認(rèn)識(shí)更加系統(tǒng),避免以偏概全,對(duì)臨床實(shí)踐和理論研究都有意義。
