相關(guān)自身免疫疾病中血清IgG亞型分布特點(diǎn)研究

【摘要】:目的:分析IgG亞型在相關(guān)自身免疫性疾病中的分布特點(diǎn)。方法:本研究共納入自治區(qū)人民醫(yī)院風(fēng)濕免疫科2015-2016確診類風(fēng)濕關(guān)節(jié)炎患者160例、系統(tǒng)性紅斑狼瘡100例、強(qiáng)直性脊柱炎104例,原發(fā)性干燥綜合征初治患者70例。所有患者均符合國際指南診斷標(biāo)準(zhǔn)。健康對(duì)照組患者50例。采用散射比濁法檢測(cè)各受試者血清IgG1、IgG2、IgG3、IgG4水平;比較分析各種自身免疫性疾病患者血清IgG亞型水平與健康對(duì)照者有無差異,尤其是IgG4水平是否升高,并結(jié)合各自身免疫性疾病患者的臨床資料,分析具有不同臨床特征的患者IgG亞型分布特點(diǎn)及其相關(guān)性。結(jié)果:1.通過比較自身免疫性疾病患者與健康對(duì)照者血清IgG亞型發(fā)現(xiàn):pSS患者血清IgGl、IgG3、總IgG水平顯著高于對(duì)照組;RA患者血清IgGl、IgG3水平顯著高于對(duì)照組;SLE患者血清IgGl、IgG3、總IgG水平顯著高于對(duì)照組;AS患者血清血清IgGl、IgG4水平顯著高于對(duì)照組。2.通過對(duì)各自身免疫性疾病患者血清IgG亞型進(jìn)行相互比較發(fā)現(xiàn):pSS組患者血清IgGl、總IgG水平顯著高于其他各疾病組,AS組患者血清IgG2、IgG4水平顯著高于其他各疾病組,RA組患者血清IgG3水平其他各疾病組。3.通過對(duì)各自身免疫性疾病不同亞組間患者血清IgG亞型進(jìn)行比較發(fā)現(xiàn):IgG亞型在具有不同臨床特征患者中的分布有明顯不同。pSS組患者中有抗SSA、SSB抗體陽性、ESR升高者其血清IgGl水平明顯高于無上述特征者;RA組患者中繼發(fā)肺間質(zhì)纖維化患者其血清IgG4水平明顯升高;SLE組患者中補(bǔ)體下降、ESR增快者血清IgGl水平顯著升高,有腎受累者血清各IgG亞型均有顯著下降,而且以IgG2降低更為明顯;AS組患者中提示炎癥反應(yīng)的CRP、IL-6升高伴隨IgG3增高。結(jié)論:1血清IgG亞型在pSS、RA、SLE、AS疾病患者中的分布明顯不同于健康人群。2血清IgG亞型在pSS、RA、SLE、AS疾病患者間的分布有著顯著區(qū)別。3血清IgG4水平與類風(fēng)濕關(guān)節(jié)炎患者肺間質(zhì)纖維化存在一定相關(guān)性。4血清IgG4水平在強(qiáng)直性脊柱炎患者中明顯升高,可能與既往使用生物制劑相關(guān)。
[Abstract]:Objective: to analyze the distribution of IgG subtypes in autoimmune diseases. Methods: a total of 160 patients with rheumatoid arthritis and 100 patients with systemic lupus erythematosus were enrolled in this study. 104 cases of ankylosing spondylitis and 70 cases of primary Sjogren's syndrome. All the patients were in accordance with the international standard of diagnosis and 50 cases of healthy control group. To compare and analyze the difference of serum IgG subtype level between patients with various autoimmune diseases and healthy controls, especially whether the level of IgG4 increased, and combined with the clinical data of the patients with autoimmune diseases. The distribution and correlation of IgG subtypes in patients with different clinical characteristics were analyzed. Results: 1.Through comparing the serum IgG subtypes of patients with autoimmune diseases with those of healthy controls, we found that the total IgG level in patients with IgG was significantly higher than that in controls. The serum IgG3 level of RA patients was significantly higher than that of control group, and the total IgG level was significantly higher than that of control group. 2. The serum IgG subtypes of patients with autoimmune diseases were studied. It was found that the serum IgGland total IgG levels of the patients with PSS were significantly higher than those of the patients with as and other diseases. The serum IgG3 levels of the patients with RA were significantly higher than those of the patients with RA and other diseases. Comparison of serum IgG subtypes in patients with different subgroups of immunological diseases. The distribution of IgG subtypes in patients with different clinical characteristics was significantly different. The serum IgGl level of patients with elevated anti-SSA-SSB antibody was significantly higher in PSS group. The level of serum IgG4 in patients with secondary pulmonary interstitial fibrosis was significantly higher than that in RA patients without the above characteristics. Serum IgG subtypes were significantly decreased in patients with renal involvement. Moreover, the decrease of IgG2 was more obvious in as group than that in as group. Conclusion the distribution of serum IgG subtype pSS1 in patients with as disease is significantly different from that in healthy people. 2. Serum IgG subtype in patients with pSSRAA SLEAS disease is significantly different from that in healthy people. There was a significant correlation between serum IgG4 level and pulmonary interstitial fibrosis in patients with rheumatoid arthritis. The level of serum IgG4 in patients with ankylosing spondylitis was significantly higher than that in patients with ankylosing spondylitis. May be related to the past use of biological agents.
