【摘要】：目的:評(píng)價(jià)中國精神分裂癥患者服藥依從性影響因素,為臨床提高患者服藥依從性提供支持和指導(dǎo)意見。方法:從中英文數(shù)據(jù)庫中檢索有關(guān)中國精神分裂癥患者服藥依從性影響因素的文獻(xiàn)進(jìn)行分析。通過英文檢索詞 "Schizophrenia*","Schizophrenic Disorder*","MedicationAdherence","Medication Persistence","Medication Compliance","Risk Factor*","Population*at Risk","China","Mainland China","Hong kong","Macao","Taiwan"和中文檢索詞"精神分裂癥"、"精神分裂性障礙""藥物治療依從性"、"服藥依從性""危險(xiǎn)因素"、"影響因素"對(duì)萬方數(shù)據(jù)庫、中國知網(wǎng)(CNKI)、中國生物醫(yī)學(xué)文獻(xiàn)數(shù)據(jù)庫(CBM)、Pubmed、Web of Science、Cochrane 和 Scopus 這 7 個(gè)國內(nèi)外數(shù)據(jù)庫進(jìn)行文獻(xiàn)檢索。同時(shí),對(duì)新期刊以及新文獻(xiàn)進(jìn)行手工檢索。經(jīng)質(zhì)量評(píng)估后,收集數(shù)據(jù)。兩名研究人員分別獨(dú)立進(jìn)行,不一致的信息由第三位研究人員再次核對(duì)。采用Review manger 5.3軟件對(duì)服藥依從性的影響因素進(jìn)行合并分析。采用優(yōu)勢(shì)比(odds ratio,OR)和標(biāo)準(zhǔn)化均數(shù)差值(standardized mean difference,SMD)對(duì)分類指標(biāo)和連續(xù)性指標(biāo)分別衡量。并且,對(duì)異質(zhì)性≥50%的因素采用合并值敏感性分析法,發(fā)表偏倚采用漏斗圖進(jìn)行檢驗(yàn)。結(jié)果:本研究最終納入9篇文獻(xiàn),合計(jì)分析了 12個(gè)指標(biāo),包括:患者年齡(歲)、性別、婚姻狀況、病程(年)、社會(huì)支持、藥物副作用、受教育程度(≥9年)、工作情況、支付能力、服用的藥物種數(shù)、醫(yī)患關(guān)系以及患者的自知力。本研究顯示4個(gè)因素與服藥依從性關(guān)聯(lián)有統(tǒng)計(jì)學(xué)意義,分別是社會(huì)支持:OR=3.19,95%CI:1.18～8.64,PQ0.00001,P=0.02;藥物副作用:OR=0.35,95%CI:0.18～0.68,PQ=0.0001,P P=0.002;支付能力:OR=1.92,95%CI:1.09～3.40,PQ=0.0006,P=0.02;患者自知力:OR=5.51,95%CI:1.42～21.42,PQ=0.05,P=0.01。結(jié)論:中國精神分裂癥患者服藥依從性的主要影響因素為:社會(huì)支持、藥物副作用、支付能力及患者自知力。
