【摘要】：目的:探討大腸濕熱證管狀腺瘤與P53、bFGF表達的相關性。方法:隨機選取就診福建省第二人民醫(yī)院門診/病房行電子結腸鏡檢查發(fā)現(xiàn)息肉并行息肉摘除術,經病理診斷確診為大腸管狀腺瘤伴IN并符合納入標準的患者60例,按中醫(yī)辨證分為研究組(大腸濕熱證組)、對照組(脾胃虛弱證組)各30例,并設立正常對照組20例(來源于本院體檢中心)。通過免疫組織化學方法檢測研究組、對照組及正常對照組腸黏膜組織中P53及bFGF的表達水平,統(tǒng)計上述3組P53、bFGF表達并進行對比分析,探討它們之間的相關性。結果:1、對大腸濕熱證組、脾胃虛弱證組和正常對照組的年齡、性別進行比較,P均0.05,無統(tǒng)計學意義,三組間的性別、年齡無顯著差異,具有可比性。對研究組和對照組的瘤變程度進行比較,P0.05,兩中醫(yī)證型間腺瘤瘤變程度具有可比性。2、統(tǒng)計大腸管狀腺瘤P53、bFGF的表達與性別、年齡、上皮內瘤變程度的關系,發(fā)現(xiàn)P53、bFGF的表達在性別、年齡上無差異(P0.05);在低高級別上皮內瘤變程度中的表達差異經檢驗有統(tǒng)計學意義(P0.05),且成正相關。3、P53、bFGF在大腸濕熱證組、脾胃虛弱證組表達均明顯高于正常對照組,差異有統(tǒng)計學意義(P0.05);大腸濕熱證組中P53、bFGF表達與脾胃虛弱證組比較差異無統(tǒng)計學意義(P0.05)。4、大腸管狀腺瘤同一高級別上皮內瘤變大腸濕熱證P53表達高于脾胃虛弱證,具統(tǒng)計學意義(P0.05);不同瘤變程度大腸濕熱證組、脾胃虛弱證組中bFGF表達均無差異(P0.05)。5、大腸濕熱證組和脾胃虛弱證組中P53、bFGF在高級別上皮內瘤變中的表達均明顯高于低級別上皮內瘤變,具有統(tǒng)計學意義(P均0.05)。結論:1、大腸管狀腺瘤高級別上皮內瘤變中P53表達與大腸濕熱證可能存在相關。2、P53、bFGF在大腸管狀腺瘤組織中的表達明顯高于正常腸黏膜組織,且高級別上皮內瘤變中表達明顯升高,提示P53及bFGF在腺瘤惡變過程中可能起重要作用。
[Abstract]:Objective: to investigate the relationship between tubular adenoma and p53 in large intestine damp-heat syndrome. Correlation of bFGF expression. Methods: polyps were detected by electronic colonoscopy in outpatient / ward of the second people's Hospital of Fujian Province. Sixty patients with tubular adenoma of large intestine with IN and according to the inclusion criteria were divided into two groups according to TCM syndrome differentiation (30 cases in the group of damp-heat syndrome of large intestine and 30 cases in the group of deficiency of spleen and stomach). The expression levels of p53 and bFGF in intestinal mucosa of the study group, the control group and the normal control group were detected by immunohistochemical method. The expression of P53 bFGF in the above three groups was analyzed and compared, and the correlation between them was discussed. Results: 1. The age of the group with damp-heat syndrome of large intestine, the group of deficiency of spleen and stomach and the normal control group. Gender comparison P 0.05, no statistical significance, three groups of gender, age no significant difference, comparable. The study group and the control group of the degree of neoplasia were compared P0.05. The expression of P53 bFGF in colorectal tubular adenoma was correlated with sex, age and degree of intraepithelial neoplasia. The expression of bFGF was not different in sex and age (P 0.05). The difference of expression in the low and high grade intraepithelial neoplasia was statistically significant (P 0.05), and the positive correlation was found in the large intestine dampness and heat syndrome group. The expression of spleen and stomach deficiency syndrome group was significantly higher than that of normal control group, and the difference was statistically significant (P 0.05). The expression of P53 bFGF in the large intestine damp-heat syndrome group was not significantly different from that in the spleen and stomach asthenia group. The expression of p53 in large intestine damp-heat syndrome was higher than that in spleen and stomach asthenia group (P 0.05). There was no difference in the expression of bFGF in the groups with different degrees of tumorigenesis of dampness and heat of large intestine and deficiency of spleen and stomach, and p53 in the group of damp-heat of large intestine and the group of deficiency of spleen and stomach. The expression of bFGF in high grade intraepithelial neoplasia was significantly higher than that in low grade intraepithelial neoplasia (P < 0.05). The expression of p53 in high grade intraepithelial neoplasia of colorectal tubular adenoma may be associated with damp-heat syndrome of large intestine. The expression of P53 bFGF in colorectal tubular adenoma is significantly higher than that in normal intestinal mucosa. The expression of p53 and bFGF may play an important role in the malignant transformation of adenoma.
