精神分裂癥患者面孔整體加工與執(zhí)行功能的相關(guān)性

【摘要】:目的:正常成年人對面孔的加工復(fù)雜而精細(xì),有別于其他物體,存在整體加工的傾向,在人際交往中有不可忽視的作用;為實(shí)現(xiàn)某一特定目標(biāo)而采取的多重認(rèn)知加工協(xié)同操作的執(zhí)行能力,使得諸多繁雜的日;顒涌梢皂樌M(jìn)行。精神分裂癥患者存在廣泛的認(rèn)知缺陷,其視覺加工和執(zhí)行功能的損傷嚴(yán)重影響到其日常生活和社會交往活動。患者執(zhí)行功能受損已被國內(nèi)外眾多研究所證實(shí),而目前對面孔整體加工能力的研究尚少,這一領(lǐng)域正逐步發(fā)展為學(xué)者們關(guān)注的熱點(diǎn)問題。精神分裂癥患者對面孔的加工究竟是整體加工還是局部加工,這是一個(gè)值得探討的問題,面孔整體加工和執(zhí)行功能同為認(rèn)知的一部分,二者間是否存在相關(guān)性,尚未有明確的研究證實(shí)。因此,本研究比較精神分裂癥患者與正常人對面孔的整體加工是否存在差異,并探討面孔整體加工能力與以認(rèn)知靈活性和工作記憶為指標(biāo)的執(zhí)行功能間的相關(guān)性。 方法:采用方便取樣,選取安徽省精神衛(wèi)生中心就診的56名未用藥精神分裂癥患者和所在社區(qū)的54名正常被試進(jìn)行測試,采用陽性與陰性癥狀量表(PANSS)評估患者的精神癥狀,合成面孔任務(wù)(CFT)評估被試的面孔整體加工情況,統(tǒng)計(jì)合成面孔效應(yīng)分?jǐn)?shù)(CFEs)、面孔識別的正確率和平均反應(yīng)時(shí);同時(shí)做威斯康辛卡片分類任務(wù)(WCST)和數(shù)字廣度測驗(yàn)(DS)評估,以持續(xù)性錯(cuò)誤數(shù)和DS得分作為分析被試執(zhí)行功能的指標(biāo);對面孔整體加工和執(zhí)行功能相關(guān)性作Spearman相關(guān)分析。 結(jié)果:患者組CFT的合成面孔效應(yīng)分?jǐn)?shù)(CFEs)[(-2.75±0.72)分]、面孔識別正確率[(68.13±17.26)%]和DS得分[(19.73±4.38)分]低于正常組[分別為:(0.14±0.58)分、(83.61±10.48)%和(24.33±2.08)分],CFT的平均反應(yīng)時(shí)[(7015±3685)ms]和WCST的持續(xù)性錯(cuò)誤數(shù)[(27.45±15.67)個(gè)]高于正常組[分別為:(4095±1883)ms和(11.93±8.87)個(gè)],差異有統(tǒng)計(jì)學(xué)意義(P0.05~0.01);患者組SM和SA情況下的正確率和平均反應(yīng)時(shí)差異均無統(tǒng)計(jì)學(xué)意義(P0.05),,正常組SM情況下的正確率高于SA,差異有統(tǒng)計(jì)學(xué)意義(r=2.364,P=0.020);CFT正確率與PANSS總分(r=-0.491,P0.01)和陰性癥狀分(r=-0.312,P0.05)呈負(fù)相關(guān),DS得分與PANSS總分(r=-0.299,P0.05)呈負(fù)相關(guān),其余各項(xiàng)與PANSS得分的相關(guān)性均無統(tǒng)計(jì)學(xué)意義;患者組CFT的正確率與WCST的持續(xù)性錯(cuò)誤數(shù)(r=-0.339,P0.01)和DS得分(r=0.314,P0.05)顯著相關(guān),CFT的CFEs和平均反應(yīng)時(shí)與WCST的持續(xù)性錯(cuò)誤數(shù)和DS得分的相關(guān)性無統(tǒng)計(jì)學(xué)意義(P0.05)。 結(jié)論:精神分裂癥患者對面孔的整體加工受損,有局部加工傾向;患者面孔識別的正確率低于正常人,可能與其認(rèn)知靈活性和工作記憶等執(zhí)行功能受損有關(guān);患者面孔加工策略的傾向性與執(zhí)行功能的關(guān)系有待進(jìn)一步探討。
[Abstract]:Objective: normal adult face processing is complex and fine, different from other objects, there is a tendency of overall processing, which can not be ignored in interpersonal communication; In order to achieve a specific goal, the execution ability of multi-cognitive processing cooperative operation makes a lot of complex daily activities can be carried out smoothly. Schizophrenia patients have a wide range of cognitive defects, their visual processing and executive function damage seriously affect their daily life and social interaction activities. The study of patients' executive function impairment has been confirmed by many researches at home and abroad, but at present, there are few researches on the whole face processing ability, and this field is gradually developing into a hot issue that scholars pay attention to. It is worth discussing whether the face processing in schizophrenic patients is a whole process or a partial processing. The whole face processing and the executive function are both part of cognition, and whether there is a correlation between the two. No clear research has confirmed it. Therefore, this study compared whether there were differences in the overall face processing between the schizophrenic patients and the normal people, and explored the correlation between the overall face processing ability and the executive function, which was characterized by cognitive flexibility and working memory. Methods: 56 untreated schizophrenic patients in Anhui Mental Health Center and 54 normal subjects in their community were tested by convenient sampling. The positive and negative symptoms scale (PANSS) was used to evaluate the mental symptoms of the patients. The synthetic face task (CFT) was used to evaluate the overall face processing of the subjects, and the scores of face effect (CFE), the correct rate and average response time of face recognition, and the Wisconsin card sorting task (WCST) and the number span test (DSDS) were analyzed. The persistent error number and DS score were used as the indicators to analyze the executive function of the subjects, and the Spearman correlation analysis was made on the correlation between the overall face processing and the executive function. Results: the scores of synthetic face effect scores (CFES), correct rate of face recognition [68.13 鹵17.26%] and DS score (19.73 鹵4.38) of CFT in the patients group were lower than those in the normal group (83.61 鹵10.48% and 24.33 鹵2.08, respectively). The mean response time of CFT was higher than that of the normal group [68.13 鹵17.26%] and WCST (27.45 鹵15.67). There was significant difference in the accuracy rate and average reaction time of SM and SA in the normal group (4.095 鹵1883)ms and 11.93 鹵8.87, respectively), but the correct rate in SM and SA in the normal group was higher than that in the control group, and the difference was statistically significant. The correct rate of PANSS was negatively correlated with the total score of PANSS (r = 0.491P 0.01) and the score of negative symptoms (r = 0.312P 0.05). The DS score was negatively correlated with the total score of PANSS (r = 0.299n, P 0.05). The correlation between other items and PANSS score was not statistically significant. There was no significant correlation between the correct rate of CFT and the number of persistent errors in WCST (P 0.01) and the scores of DS (r = 0.314P 0.05). There was no significant correlation between CFEs and mean response time of CFT and the number of persistent errors in WCST and the scores of DS. There was no significant correlation between the accuracy rate of CFT and the number of persistent errors in WCST and DS scores. Conclusion: the patients with schizophrenia have the tendency of local processing, and the correct rate of face recognition is lower than that of normal people, which may be related to the impairment of their cognitive flexibility and working memory. The relationship between the tendency of face processing strategy and executive function needs to be further explored.
