【摘要】：研究目的:了解江西省社區(qū)老年人的抑郁情緒現(xiàn)況,探討其影響因素,針對其影響因素制定綜合心理干預(yù)方案,實施示范性心理干預(yù)并對效果進(jìn)行評估與反饋,為提高老年心理健康水平提供參考依據(jù)。研究方法:(1)定量調(diào)查:采用多級、整群、隨機(jī)的取樣方法,對1389名60歲以上的江西省社區(qū)老年人,進(jìn)行老年抑郁量表(Geriatric Depression Scale,GDS)、生活事件量表(Life Event Scale,LES)、日常生活能力量表(Activities of Daily Living Scale,ADL)、社會支持評定量表(Social Support Rating Scale,SRSS)、蒙特利爾認(rèn)知評估量表(Montreal Cognitive Assessment,Mo CA)和自編問卷等測評與分析。(2)心理干預(yù):隨機(jī)選取南昌市2個社區(qū)作為對照社區(qū)和干預(yù)社區(qū),每個社區(qū)均隨機(jī)抽取有抑郁情緒(GDS得分≥11分)的老年人為研究對象,兩組均開展一般健康教育,干預(yù)組在此基礎(chǔ)上實施綜合心理干預(yù)。研究結(jié)果:(1)江西省社區(qū)老年人抑郁情緒的檢出率(GDS≥11分)為39.38%(547/1389),GDS總分為(10.58±6.73)分;(2)單因素χ2分析顯示:年齡、家庭經(jīng)濟(jì)狀況、有無精神疾病家族史、生活事件、社會支持、認(rèn)知功能、日常生活能力、婚姻狀況、近半年軀體健康狀況和與他人居住情況等9個因素均與社區(qū)老年人抑郁情緒的發(fā)生有關(guān);(3)多因素Logistic回歸分析顯示:有精神疾病家族史、負(fù)性生活事件較多、日常生活能力下降、獨居、近半年患有軀體疾病等是老年人出現(xiàn)抑郁情緒的危險因素,良好的社會支持水平是老年人抑郁情緒發(fā)生的保護(hù)因素;(4)社區(qū)健康教育可以部分緩解老年人的抑郁情緒;(5)綜合心理干預(yù)組干預(yù)后老人抑郁情緒檢出率和抑郁程度較對照組老人均顯著的降低,差異具有統(tǒng)計學(xué)意義(P0.05)。研究結(jié)論:(1)江西省社區(qū)老年人抑郁情緒普遍存在且抑郁水平相對較高;(2)社區(qū)老年人抑郁的發(fā)生主要受精神疾病家族史、日常生活能力、居住方式、近半年存在軀體疾病和生活事件、社會支持等多種因素影響:(3)綜合心理干預(yù)可以有效的改善社區(qū)老年人的抑郁情緒。
