【摘要】：精神分裂癥是一種嚴(yán)重的精神疾病,臨床癥狀主要表現(xiàn)為陰性癥狀、陽性癥狀和認(rèn)知功能障礙,在精神分裂癥的多種認(rèn)知功能障礙中,執(zhí)行功能紊亂被認(rèn)為是最核心和最持久的缺陷。執(zhí)行功能(Executive function,簡稱EF)是一種高級的認(rèn)知過程,是指在完成一些比較復(fù)雜的認(rèn)知任務(wù)時(shí),個(gè)體對達(dá)成特定目的的認(rèn)知過程進(jìn)行靈活的調(diào)節(jié)和優(yōu)化,并在此基礎(chǔ)上對認(rèn)知過程進(jìn)行有意識的控制,從而使認(rèn)知系統(tǒng)協(xié)調(diào)有序的運(yùn)行,順利的實(shí)現(xiàn)既定目標(biāo)。執(zhí)行功能缺陷會(huì)影響患者的生活、工作和學(xué)習(xí)等,對精神分裂癥患者的執(zhí)行功能的研究有重大的意義。 研究目的：本研究根據(jù)Miyake的理論,將執(zhí)行功能劃分為三個(gè)成分：記憶、抑制和轉(zhuǎn)換能力,旨在探究精神分裂癥患者是否在這三個(gè)子成分上存在缺陷。 研究方法：根據(jù)Miyake的三成分的理論假設(shè),分別設(shè)計(jì)三種任務(wù)來測量執(zhí)行功能的三種成分。其中,用韋氏記憶量表(WMS Ⅲ)的空間廣度任務(wù)測量記憶能力,工具為空間廣度板；用維度變化卡片分類任務(wù)(DCCS)測量轉(zhuǎn)換能力；用箭頭任務(wù)(Arrows)測量抑制能力；并且用綜合的Dots任務(wù)同時(shí)測量記憶、抑制和轉(zhuǎn)換能力。嚴(yán)格匹配精神分裂癥組和對照組的性別、年齡、文化程度等基本人口學(xué)特征。根據(jù)不同的任務(wù)的特點(diǎn),采用E-prime軟件編制Arrows任務(wù)、Dccs任務(wù)和Dots任務(wù)并用筆記本電腦呈現(xiàn),計(jì)算機(jī)自動(dòng)記錄反應(yīng),而空間廣度任務(wù)則人工進(jìn)行,并一一記錄反應(yīng)。最后用SPSS16.0分析數(shù)據(jù)。 研究結(jié)論： (1)精神分裂癥患者的空間記憶力受到嚴(yán)重?fù)p傷。 (2)精神分裂癥患者在箭頭任務(wù)上沒有表現(xiàn)出抑制功能的損傷。 (3)精神分裂癥患者在維度變化卡片分類任務(wù)上表現(xiàn)出轉(zhuǎn)換能力的缺陷。 (4)精神分裂癥患者的整體執(zhí)行功能是受到損傷的,而且可以通過訓(xùn)練提高精神分裂癥患者的執(zhí)行功能。
[Abstract]:Schizophrenia is a serious mental disease. The clinical symptoms are mainly negative symptoms, positive symptoms and cognitive dysfunction. Executive dysfunction is considered to be the most central and persistent defect. Executive function (EFF) is a high-level cognitive process, which refers to the completion of some more complex cognitive tasks. The individual adjusts and optimizes the cognitive process to achieve a specific purpose, and on this basis controls the cognitive process consciously, so that the cognitive system can operate in a coordinated and orderly manner. The deficiency of executive function will affect the life, work and study of patients, which is of great significance to the study of the executive function of patients with schizophrenia. Objective: according to Miyake's theory, the executive function was divided into three components: memory, inhibition and conversion. Methods: according to the theoretical hypothesis of three components of Miyake, three tasks were designed to measure the three components of executive function. The ability of conversion was measured with dimensional change card sorting task (DCCS); with arrowhead task (Arrows); with comprehensive Dots task simultaneously measuring memory, inhibition and conversion ability. The gender and age of schizophrenia group and control group were strictly matched. According to the characteristics of different tasks, E-prime software is used to compile Arrows task, DCCS task and Dots task and present them with notebook computer. The computer automatically records the response, while the spatial breadth task is manually carried out. And record the reaction one by one. Finally, use SPSS16.0 to analyze the data. The study concluded that:. 1) the spatial memory of schizophrenic patients is severely impaired. (2) schizophrenic patients showed no inhibitory effects on arrowhead tasks. Schizophrenic patients showed a deficit in their ability to change their cards on dimensional change card sorting tasks. 4) the overall executive function of schizophrenic patients is impaired and can be improved by training.
