【摘要】：本文是一項(xiàng)有關(guān)醫(yī)務(wù)社會(huì)工作介入癌末患者提供臨終關(guān)懷服務(wù)的實(shí)務(wù)性研究。隨著社會(huì)工作的發(fā)展,人們對(duì)醫(yī)務(wù)社會(huì)工作需求的加深,癌癥患者這個(gè)群體也成為醫(yī)務(wù)社會(huì)工作者的重要服務(wù)對(duì)象之一。對(duì)癌癥患者提供臨終關(guān)懷服務(wù)不但具有醫(yī)學(xué)上的意義,也符合倫理上的需求,同時(shí)更是社會(huì)工作要關(guān)注的問題,而醫(yī)務(wù)社會(huì)工作介入癌末患者臨終關(guān)懷更是一項(xiàng)新的嘗試,對(duì)癌癥患者及其家屬這類人群進(jìn)行人文關(guān)懷與提供專業(yè)服務(wù),也成為醫(yī)務(wù)社會(huì)工作者的重要服務(wù)內(nèi)容之一。 本研究采用文獻(xiàn)研究法、個(gè)案訪談法、案例研究法等研究方法,從微觀層面對(duì)一位癌末患者臨終個(gè)案進(jìn)行剖析,探究醫(yī)務(wù)社會(huì)工作者介入的途徑與干預(yù)技巧,從而為患者及其家屬提供臨終關(guān)懷與情緒輔導(dǎo)服務(wù),構(gòu)建服務(wù)資源網(wǎng)絡(luò)。 本文主要從癌癥患者及其家屬的角度出發(fā),以醫(yī)務(wù)社會(huì)工作和臨終關(guān)懷為切入點(diǎn),醫(yī)務(wù)社會(huì)工作者運(yùn)用專業(yè)工作方法和技巧,為患者及其家屬提供臨終關(guān)懷與情緒輔導(dǎo)服務(wù),減輕其心理、生理和社會(huì)壓力,對(duì)患者及其家屬進(jìn)行死亡教育,并協(xié)助其正確認(rèn)識(shí)生死,構(gòu)建社會(huì)支持網(wǎng)絡(luò),形成患者及其家屬-醫(yī)生-護(hù)士-醫(yī)務(wù)社會(huì)工作者-社區(qū)工作人員-慈善組織人員-志愿者這樣的一個(gè)健康的人文關(guān)懷網(wǎng)絡(luò),為患者及其家屬提供人文關(guān)懷和情感支持,陪伴患者走過生命的最后一程,協(xié)助家屬回歸正常的社會(huì)生活。 最后,筆者根據(jù)提供個(gè)案服務(wù)的全過程,對(duì)本研究中個(gè)案介入的不足進(jìn)行實(shí)踐反思,并引出醫(yī)務(wù)社會(huì)工作構(gòu)建癌末患者臨終關(guān)懷服務(wù)資源網(wǎng)絡(luò)的建議。筆者認(rèn)為,癌末患者臨終關(guān)懷服務(wù)是一個(gè)系統(tǒng)工程,它需要醫(yī)護(hù)人員、社會(huì)工作者、社工機(jī)構(gòu)、政府、社區(qū)、慈善組織、義工組織等不同的部門實(shí)現(xiàn)跨領(lǐng)域跨專業(yè)的合作,從從社會(huì)政策、專業(yè)發(fā)展、社會(huì)工作者資源整合運(yùn)用、團(tuán)隊(duì)合作、義工服務(wù)來(lái)構(gòu)建一個(gè)完善的臨終關(guān)懷服務(wù)資源網(wǎng)絡(luò),以期助益于癌末患者臨終關(guān)懷服務(wù)的有效開展。
[Abstract]:This paper is a practical study on medical social work intervention in end-cancer patients to provide hospice care services. With the development of social work, people's need for medical social work has deepened. The group of cancer patients has also become one of the important service objects of medical social workers. To provide hospice services to cancer patients is not only of medical significance, but also in line with the ethical needs. At the same time, social work should pay attention to the issue, and medical social work involved in terminal cancer care is a new attempt to carry out humanistic care and provide professional services to cancer patients and their families. Also become one of the important service contents of medical social worker. This study uses literature research method, case interview method, case study method and other research methods, from the microscopic level to a terminal cancer patient dying case analysis, explore the approach of medical social workers intervention and intervention skills. In order to provide hospice care and emotional counseling services for patients and their families, build a network of service resources. Starting from the angle of cancer patients and their families, this paper takes medical social work and hospice care as the breakthrough point, and medical social workers use professional working methods and skills. To provide hospice care and emotional counseling services for patients and their families, to reduce their psychological, physical and social pressure, to carry out death education for patients and their families, to help them understand life and death correctly, and to build social support network. Form a healthy humanistic care network of patients and their families, doctors, nurses, medical social workers, community workers, philanthropic organizations and volunteers. To provide humanistic care and emotional support for patients and their families, to accompany patients through the last journey of life, to help families return to normal social life. Finally, according to the whole process of providing case service, the author reflects on the deficiency of case intervention in this study. The author thinks that the terminal care service is a system engineering, it needs medical staff, social workers. Social work agencies, government, community, charitable organizations, volunteer organizations and other different departments to achieve cross-disciplinary cooperation, from social policy, professional development, social worker resources integration, teamwork. Volunteer service is used to construct a perfect network of hospice care resources, which is helpful to the effective development of terminal care services for terminal cancer patients.
