醫(yī)養(yǎng)融合的內(nèi)在沖突及其制度化途徑

【摘要】:醫(yī)養(yǎng)融合是通過對(duì)醫(yī)療資源和養(yǎng)老資源進(jìn)行系統(tǒng)化、科學(xué)化配置和管理,為老年人提供集醫(yī)療護(hù)理和生活照料為一體的綜合養(yǎng)老服務(wù),實(shí)質(zhì)上是對(duì)現(xiàn)有傳統(tǒng)醫(yī)療資源和養(yǎng)老資源的細(xì)分整合、轉(zhuǎn)型升級(jí)、創(chuàng)新和延續(xù)傳統(tǒng)醫(yī)療服務(wù)和養(yǎng)老服務(wù)。然而在當(dāng)前我國醫(yī)療服務(wù)和養(yǎng)老服務(wù)"分而治之"狀態(tài)下,在醫(yī)養(yǎng)融合中客觀存在著利益訴求沖突、付出與回報(bào)之間沖突以及合法性基礎(chǔ)沖突等問題,由此造成諸多發(fā)展困境。從組織社會(huì)學(xué)角度上看,造成這些內(nèi)在沖突的原因在于醫(yī)療機(jī)構(gòu)和養(yǎng)老機(jī)構(gòu)資源的非對(duì)稱性、合作的交易成本較高以及具有不同的合法性基礎(chǔ)和壓力。文章基于制度化合作的思路,嘗試提出化解這些內(nèi)在沖突的關(guān)鍵在于盡快構(gòu)建一個(gè)相對(duì)合理、比較系統(tǒng)、便于操作且易于被各方面認(rèn)可的制度化合作框架。
[Abstract]:The integration of medical and nursing care is a comprehensive old-age service for the elderly, which integrates medical care and life care through systematic, scientific allocation and management of medical resources and old-age resources. In essence, it is the subdivision and integration of the existing traditional medical resources and pension resources, transformation and upgrading, innovation and continuation of traditional medical services and pension services. However, in the current state of "divide and rule" of medical service and old-age service in our country, there are objectively many problems in the integration of medical and nursing, such as the conflict of interest demand, the conflict between pay and return, and the conflict of legitimacy basis, which leads to many development dilemmas. From the point of view of organizational sociology, the reasons for these internal conflicts lie in the asymmetry of resources between medical institutions and pension institutions, the high transaction cost of cooperation, and the different legitimacy and pressure of cooperation. Based on the idea of institutionalized cooperation, this paper tries to put forward that the key to resolve these internal conflicts lies in the construction of a relatively reasonable, systematic, easy to operate and easy to be recognized by all aspects of the institutionalized cooperation framework as soon as possible.
