常州市原戚墅堰區(qū)醫(yī)養(yǎng)結(jié)合養(yǎng)老服務(wù)研究

【摘要】：“醫(yī)養(yǎng)結(jié)合”是集醫(yī)療、護理、康復(fù)、保健、臨終關(guān)懷和養(yǎng)老等多種功能于一體的醫(yī)養(yǎng)結(jié)合、全程托護式的新型養(yǎng)老護理模式。又是一種有病就治病,沒病就療養(yǎng),將醫(yī)療和養(yǎng)老有效結(jié)合的新型養(yǎng)老服務(wù)模式。此種模式不僅僅包括了傳統(tǒng)意義上的生活照料養(yǎng)老服務(wù),即日常的文化娛樂,常規(guī)生活照料,精神慰藉等服務(wù)。而且更加注重老年人的醫(yī)療保健,康復(fù)訓(xùn)練,疾病診治,以及臨終關(guān)懷等專業(yè)醫(yī)護服務(wù)。這種醫(yī)療和養(yǎng)護相結(jié)合的模式,對于老年群體中的弱勢群體,即失能及半失能老人尤為重要,能有效提高失能及半失能老年人的生活質(zhì)量。本論文通過從常州市原戚墅堰區(qū)民政、衛(wèi)生、統(tǒng)計等部門調(diào)閱大量資料和數(shù)據(jù),分析常州市原戚墅堰區(qū)老年人的人口比例及患病情況,同時對常州市原戚墅堰區(qū)社會福利中心、居家養(yǎng)老服務(wù)中心、愛心護理院等養(yǎng)老機構(gòu)進行調(diào)查研究,提出醫(yī)養(yǎng)結(jié)合養(yǎng)老服務(wù)的新研究方式,使其應(yīng)用在老年人中的大部分失能、半失能等弱勢群體,這不僅是對傳統(tǒng)的養(yǎng)老服務(wù)的肯定,也是改變傳統(tǒng)養(yǎng)老服務(wù)中只重“養(yǎng)”卻不重“醫(yī)”的養(yǎng)老服務(wù)現(xiàn)狀。原戚墅堰區(qū)從一開始推行醫(yī)養(yǎng)結(jié)合新型養(yǎng)老服務(wù)就得到社區(qū)居民的高度關(guān)切和歡迎,關(guān)鍵的一點是醫(yī)養(yǎng)結(jié)合養(yǎng)老服務(wù)是以社區(qū)居民健康實際出發(fā),以居民的健康需求為導(dǎo)向,堅持社區(qū)衛(wèi)生的公益性、不以盈利為目的的服務(wù)功能。醫(yī)養(yǎng)結(jié)合通過醫(yī)療資源和養(yǎng)老資源的有效整合,合理使用了社區(qū)資源,真正體現(xiàn)了社區(qū)衛(wèi)生為居民提供有效、經(jīng)濟、方便、綜合、連續(xù)的集醫(yī)療、預(yù)防、保健、康復(fù)、健康教育等服務(wù)的特點。醫(yī)養(yǎng)結(jié)合服務(wù),大有可持續(xù)發(fā)展前景。本文研究的主題是常州市原戚墅堰區(qū)醫(yī)養(yǎng)結(jié)合養(yǎng)老服務(wù)。主要運用了福利多元化理論及需求理論作為本文的理論支撐,并研究了國內(nèi)外養(yǎng)老服務(wù)現(xiàn)狀。結(jié)合養(yǎng)老服務(wù)的相關(guān)理論,尤其是醫(yī)養(yǎng)結(jié)合養(yǎng)老發(fā)展理論,對影響醫(yī)養(yǎng)結(jié)合養(yǎng)老發(fā)展服務(wù)的各個因素進行分析和比較,研究不同因素對其的影響和貢獻。通過對常州市原戚墅堰區(qū)醫(yī)養(yǎng)結(jié)合養(yǎng)老服務(wù)發(fā)展現(xiàn)狀、存在問題繼續(xù)分析,在借鑒國內(nèi)外其它地區(qū)醫(yī)養(yǎng)結(jié)合養(yǎng)老服務(wù)發(fā)展經(jīng)驗基礎(chǔ)上對常州市原戚墅堰區(qū)醫(yī)養(yǎng)結(jié)合養(yǎng)老服務(wù)現(xiàn)狀發(fā)展提出對策。并有針對性的從加入事業(yè)投入、醫(yī)護人員培養(yǎng)、政府政策支持改革,擴大資金來源等方面來保障老年人對“醫(yī)養(yǎng)結(jié)合”的有效需求,從而有效推進養(yǎng)老服務(wù)業(yè)的行業(yè)化發(fā)展,加大政府監(jiān)督管理強度,加強養(yǎng)老服務(wù)業(yè)標(biāo)準(zhǔn)化管理、多管齊下,保證政府和社會對“醫(yī)養(yǎng)結(jié)合”養(yǎng)老服務(wù)的有效供給和持續(xù)發(fā)展。
[Abstract]:The combination of medical care and nursing care is a new type of nursing mode which integrates medical, nursing, rehabilitation, health care, hospice care and old-age care. It is also a new mode of old-age service which combines medical treatment with old-age care effectively. This model not only includes the traditional sense of care for the aged, that is, daily cultural entertainment, routine life care, spiritual comfort and other services. And more attention to health care, rehabilitation training, disease diagnosis and treatment, and hospice care and other professional health care services. The combination of medical treatment and maintenance is particularly important for the vulnerable groups of the elderly, that is, the disabled and the semi-disabled, and can effectively improve the quality of life of the disabled and semi-disabled. Through reading a lot of data from the departments of civil affairs, health and statistics in Yuanqishuyan District of Changzhou City, this paper analyzes the population proportion and disease situation of the elderly in Yuanqishuyan District, Changzhou City. At the same time, the social welfare center of Yuanqishuyan District in Changzhou City is also studied. The family pension service center, caring nursing home and other old-age care institutions carried out investigation and research, and put forward a new research method of combining medical care with old-age care, so that it could be applied to the majority of the disabled, semi-disabled and other vulnerable groups among the elderly. This is not only the affirmation of the traditional old-age service, but also the change of the present situation of the old-age service which only emphasizes "raising" but not "medical" in the traditional old-age service. From the beginning of the original Qishuyan District, the community residents have been highly concerned about and welcomed the implementation of the new type of old-age care service. The key point is that the combination of medical care and support for the aged is based on the actual health of the community residents and is guided by the residents' health needs. Adhere to the public welfare of community health, not to profit for the purpose of service functions. Through the effective integration of medical resources and old-age resources, the combination of medical treatment and nursing has made rational use of community resources, which truly reflects that community health provides effective, economical, convenient, comprehensive and continuous medical treatment, prevention, health care and rehabilitation for the residents. The theme of this study is the combination of medical care and old-age care services in Yuanqishuyan District, Changzhou City. This paper mainly uses the theory of welfare diversification and demand theory as the theoretical support, and studies the current situation of old-age service at home and abroad. Combined with the related theory of old-age service, especially the theory of medical and nursing combined with old-age development, this paper analyzes and compares the factors that affect the development of old-age care service, and studies the influence and contribution of different factors to it. By analyzing the current situation of medical and nursing combined old-age service development in Yuanqishuyan District, Changzhou City, the existing problems continue to be analyzed. Based on the experience of other regions at home and abroad, this paper puts forward some countermeasures for the development of medical and nursing combined old-age service in Yuanqishuyan District, Changzhou City. And aiming at ensuring the elderly's effective demand for "combination of medical care and support" from the aspects of joining in career investment, training of medical and nursing personnel, government policy supporting reform, expanding the source of funds, and so on, so as to effectively promote the development of the old-age service industry. Strengthen the intensity of government supervision and management, strengthen the standardized management of the old-age service industry, multi-pronged, ensure the effective supply and sustainable development of the "combination of medical care and support" services between the government and the society.
