【摘要】：隨著陜西省人口老齡化趨勢(shì)日益嚴(yán)峻,由醫(yī)療服務(wù)體系與養(yǎng)老服務(wù)體系各自為政產(chǎn)生的老年群體護(hù)理難、養(yǎng)老風(fēng)險(xiǎn)大等問題逐漸凸顯。轉(zhuǎn)變養(yǎng)老服務(wù)發(fā)展理念,整合醫(yī)療資源與養(yǎng)老資源,推進(jìn)醫(yī)養(yǎng)結(jié)合養(yǎng)老服務(wù)發(fā)展成為全國(guó)各地正在探索的熱點(diǎn)議題。目前我省醫(yī)養(yǎng)結(jié)合養(yǎng)老服務(wù)正處于初步發(fā)展階段,尚無(wú)具體實(shí)施意見,醫(yī)養(yǎng)結(jié)合養(yǎng)老機(jī)構(gòu)面臨著不同發(fā)展困境。本文運(yùn)用質(zhì)性研究中最大信息飽和法,調(diào)查陜西各地共15家醫(yī)養(yǎng)結(jié)合養(yǎng)老機(jī)構(gòu),收集政府相關(guān)人員、機(jī)構(gòu)管理者與工作人員以及入住老人三類群體的一手訪談資料。基于對(duì)醫(yī)養(yǎng)結(jié)合養(yǎng)老服務(wù)以及醫(yī)養(yǎng)結(jié)合養(yǎng)老機(jī)構(gòu)的明確界定,本文開展了關(guān)于醫(yī)養(yǎng)結(jié)合養(yǎng)老機(jī)構(gòu)發(fā)展現(xiàn)狀與困境的一系列研究。首先,通過梳理調(diào)研經(jīng)驗(yàn),將我省醫(yī)養(yǎng)結(jié)合養(yǎng)老機(jī)構(gòu)發(fā)展模式分為以以養(yǎng)老機(jī)構(gòu)為主體開展醫(yī)養(yǎng)結(jié)合養(yǎng)老服務(wù);以醫(yī)療機(jī)構(gòu)為主體開展醫(yī)養(yǎng)結(jié)合養(yǎng)老服務(wù)以及養(yǎng)老機(jī)構(gòu)與醫(yī)療機(jī)構(gòu)合作開展醫(yī)養(yǎng)結(jié)合養(yǎng)老服務(wù)三種,同時(shí)選取五個(gè)代表性案例進(jìn)行比較分析。其次,調(diào)查發(fā)現(xiàn)我省醫(yī)養(yǎng)結(jié)合養(yǎng)老機(jī)構(gòu)在理念認(rèn)識(shí)、運(yùn)營(yíng)機(jī)制、人才結(jié)構(gòu)、政策環(huán)境以及發(fā)展趨勢(shì)這五方面存在問題,并從政府、機(jī)構(gòu)、社會(huì)三個(gè)方面分析問題產(chǎn)生原因。最后,針對(duì)目前困境,筆者借鑒國(guó)內(nèi)外醫(yī)養(yǎng)結(jié)合養(yǎng)老服務(wù)相關(guān)經(jīng)驗(yàn),提出以下對(duì)策:理念上正確認(rèn)識(shí)醫(yī)養(yǎng)結(jié)合;政府建立健全政策體系、推進(jìn)相關(guān)配套制度研究;機(jī)構(gòu)各種模式創(chuàng)新發(fā)展、培養(yǎng)醫(yī)養(yǎng)結(jié)合復(fù)合型人才,希望為我省醫(yī)養(yǎng)結(jié)合養(yǎng)老機(jī)構(gòu)發(fā)展建設(shè)提供經(jīng)驗(yàn)參考。
[Abstract]:With the aging trend of the population in Shaanxi Province becoming increasingly severe, the elderly group nursing problems caused by the medical service system and the old-age service system have become difficult, and the pension risks have become more and more serious. The concept of the development of the old-age service has been changed. The integration of medical resources and old-age resources to promote the development of health care combined with old-age care services has become a hot topic in all parts of the country. At present our province is in the initial stage of development. There is no specific implementation opinion, medical and nursing combined pension institutions are facing different development difficulties. This paper uses the maximum information saturation method in qualitative research to investigate a total of 15 medical and nursing combined pension institutions in various parts of Shaanxi. Collect relevant government personnel, institutional managers and staff, as well as the three groups of the elderly first-hand interviews. Based on the medical care combined with old-age care services and medical care combined with pension institutions clearly defined. This paper has carried out a series of studies on the current situation and difficulties of the development of medical and nursing combined pension institutions. First of all, through combing the research experience. This paper divides the development mode of medical and nursing combined old-age care institutions into old-age care institutions as the main body to carry out the health care combined with old-age care services; Medical institutions as the main body to develop the combination of medical care and old-age care services and pension institutions and medical institutions to cooperate in the development of health care combined with old-age care services three, while selecting five representative cases for comparative analysis. Secondly. The investigation found that there are five problems in the concept understanding, operation mechanism, talent structure, policy environment and development trend of the medical and nursing combined pension institutions in our province, and from the government, institutions. Finally, in view of the current predicament, the author draws lessons from the experience of the medical and nursing combination of old-age service at home and abroad, and puts forward the following countermeasures: conceptually, correctly understand the combination of medical care and support; The government should establish and perfect the policy system and promote the research of relevant supporting systems; It is hoped that the innovation and development of various models of institutions and the training of compound talents in combination of medical care and support can provide an empirical reference for the development and construction of medical and nursing combined pension institutions in our province.
