【摘要】：目的:采用MR基于體素的形態(tài)學(xué)測(cè)量(VBM)技術(shù),探討椎間盤源性下腰痛患者大腦灰質(zhì)體積的變化。方法:2016年5月-10月對(duì)本院22例椎間盤源性下腰痛患者和22例健康志愿者行3.0T MRI掃描,掃描序列包括T_1WI、T_2WI和三維快速擾相梯度回波序列T_1WI,經(jīng)圖像后處理及統(tǒng)計(jì)分析,獲得兩組受試者大腦灰質(zhì)形態(tài)學(xué)差異的相關(guān)信息。結(jié)果:與對(duì)照組比較,下腰痛組患者大腦皮層多個(gè)腦區(qū)灰質(zhì)體積減少,包括右側(cè)額上回、左側(cè)額內(nèi)側(cè)回、左側(cè)直回、右側(cè)顳上回、右側(cè)顳中回、右側(cè)島葉以及右側(cè)額下回(P0.001,FWEc校正);無灰質(zhì)體積增加的區(qū)域。結(jié)論:慢性椎間盤源性下腰痛可引起廣泛的腦功能區(qū)灰質(zhì)體積的減少,這些腦區(qū)主要涉及感覺、認(rèn)知及情緒,一定程度上揭示了慢性椎間盤源性下腰痛神經(jīng)系統(tǒng)病變的發(fā)生機(jī)制。
[Abstract]:Aim: to investigate the changes of gray matter volume in the brain of patients with discogenic low back pain (LBP) by Mr morphometry based on voxel (VBM). Methods: from May to October 2016, 3.0T MRI scanning was performed on 22 patients with low back pain caused by intervertebral disc and 22 healthy volunteers. The scanning sequences included T 1WI T 2WI and 3D fast disturbing phase gradient echo T 1WI, and were analyzed by image postprocessing and statistical analysis. Information about the difference of gray matter morphology between the two groups was obtained. Results: compared with the control group, the volume of gray matter in multiple areas of cerebral cortex decreased in the low back pain group, including right superior frontal gyrus, left medial frontal gyrus, left straight gyrus, right superior temporal gyrus, right middle temporal gyrus. Right insular lobe and right inferior frontal gyrus (P0.001 FWEc correction); no area with increased gray matter volume. Conclusion: chronic discogenic low back pain can cause the reduction of gray matter volume in a wide range of brain functional areas, which are mainly involved in sensation, cognition and emotion. To some extent, it reveals the mechanism of neuropathy of chronic disc-derived low back pain.
