【摘要】：目的對處于HIV RNA窗口期的獻(xiàn)血者做追蹤、隨訪和驗證。方法分別使用第3、4代酶聯(lián)免疫試驗(ELISA)方法(試劑)、免疫印跡試驗(WB),以及核酸檢測技術(shù)(NAT)定性和定量等方法,對在血液篩查中檢出的1例獻(xiàn)血者HIV窗口期不同時間(根據(jù)獻(xiàn)血者的配合度間隔4-8 d)的血液標(biāo)本做追蹤和隨訪檢測。采用ELISA方法以雙試劑檢測抗-HIV;同時采用全自動NAT單檢分析系統(tǒng),對該例血液標(biāo)本做了HBV、HCV、HIV 3項NAT聯(lián)檢和鑒別試驗。結(jié)果該例獻(xiàn)血者標(biāo)本首次檢測:ELISA抗-HIV非反應(yīng)性,NAT定性試驗反應(yīng)性;隨后NAT定量試驗和抗-HIV確認(rèn)試驗(WB)也同為陰性。采用雙試劑ELISA和WB確認(rèn)方法對獻(xiàn)血者追蹤、隨訪檢測4次,至3周后確認(rèn)為血清學(xué)抗-HIV轉(zhuǎn)陽,獲得到了1個完整的血清學(xué)轉(zhuǎn)化結(jié)果。結(jié)論首次追蹤、隨訪檢測得以確認(rèn)1例重慶地區(qū)病毒載量較高的HIV RNA窗口期獻(xiàn)血者;NAT檢測有利于阻截該HIV RNA窗口期獻(xiàn)血者的血液流向臨床。
[Abstract]:Objective to follow up, follow up and verify the blood donors in the window of HIV RNA. Methods qualitative and quantitative methods were used in the third and fourth generation enzyme linked immunosorbent assay (Elisa) (Reagent, Western blotting, and Nucleic acid detection), respectively. The blood samples of one blood donor at different time (4-8 days interval according to the donor's matching degree) in HIV window period detected by blood screening were tracked and followed up. The ELISA method was used to detect anti-HIVs and the automatic NAT single detection and analysis system was used to detect and identify the three items of NAT in the blood samples of this case. Results the blood donor sample was tested for the first time for the qualitative reaction of anti-HIV non-reactive Nat test, and then the NAT quantitative test and anti-HIV confirmation test were also negative. Double reagent ELISA and WB confirmations were used to track the blood donors. Four follow-up tests were carried out. After 3 weeks, the results were confirmed as serological anti-HIV positive, and a complete serological transformation result was obtained. Conclusion A case of HIV RNA blood donor with high viral load in Chongqing area can be confirmed by follow-up examination for the first time. The detection of Nat is beneficial to blocking the blood flow of the donor during the HIV RNA window period.
