【摘要】：目的以重慶市某三甲醫(yī)院老年科住院老年患者為例,了解住院老年患者在日常生活能力、抑郁、焦慮、社會支持方面存在的問題,分析這4個方面的影響因素和關(guān)聯(lián)性。更好的幫組住院老年患者治療,找出提升住院老年患者生活質(zhì)量的方法,得出相應(yīng)的結(jié)果,為醫(yī)院應(yīng)對住院老年患者治療和護(hù)理提供參考借鑒,也可以給幫組住院老年患者的有關(guān)機(jī)構(gòu)提供參考和依據(jù)。方法本課題采取了隨機(jī)抽樣的方法,通過國際上通用的問卷和自制的問卷獲得有關(guān)數(shù)據(jù)信息。在某醫(yī)院老年科根據(jù)住院號隨機(jī)抽樣,采用問卷調(diào)查的方式了解不同性別、年齡、職業(yè)等人口學(xué)特征下住院老年患者日常生活能力、抑郁、焦慮、社會支持方面存在的信息,從分析調(diào)查數(shù)據(jù)中總結(jié)歸納出一般性的結(jié)論。結(jié)果共發(fā)放問卷400份:回收有效問卷375份,應(yīng)答率為93.75%;其中,男性219(41.60%)人,女性156(59.40%)人。焦慮方面:375份有效問卷中住院老年患者焦慮狀況總分24~66(38.46±1.94)分。住院老年患者的焦慮與日常生活能力、抑郁和社會支持的相關(guān)系數(shù)分別為0.2507;0.8910;0.3032。住院老年患者的焦慮與日常生活能力、抑郁和社會支持密切相關(guān)�；橐鰻顩r、家庭關(guān)系、是否獨(dú)居、文化程度是住院老年患者焦慮影響因素。抑郁方面:375例住院老年患者中,抑郁的住院老年患者123例,占總?cè)藬?shù)的32.80%。sds分值為21-30分者為30例,占總數(shù)的8.00%;11-20分者93例,占總數(shù)的24.80%;10分以下者252例,占總數(shù)的67.20%。住院老年患者的抑郁與住院老年患者日常生活能力、焦慮和社會支持密切相關(guān)。疾病、婚姻狀況、住院時間、自理能力、醫(yī)療費(fèi)用、業(yè)余愛好是住院老年患者抑郁影響因素。社會支持方面:375份有效問卷中,ssrs問卷得分在小于33分的占20.80%,33~45分之間的占74.40%,大于45分占4.80%。住院老年患者的社會支持與住院老年患者的抑郁焦慮和日常生活能力密切相關(guān)。年齡、文化程度、是否獨(dú)居、家人關(guān)系是住院老年患者社會支持的影響因素。日常生活能力方面:375例調(diào)查對象中,能完全獨(dú)立(adl評分為100分)活動功能的維持率,男性為24.10%,女性為31.40%;男女老年住院病人中adl受損率,女性為59.60%,男性為75.90%。女性住院老年患者的完全獨(dú)立能力要比男性好。住院老年患者各年齡段的adl受損率分別:60-69歲為95.40%,70-79歲為91.40%,80-89歲為87.20%,90歲及以上為98.55%;adl受損率在各年齡階段間有明顯差異。住院老年患者的日常生活能力與住院老年患者的抑郁焦慮和社會支持密切相關(guān)。疾病、喪偶、經(jīng)濟(jì)條件、年齡等均是adl的影響因素。結(jié)論通過對住院老年患者日常生活能力、抑郁、焦慮、社會支持的研究。發(fā)現(xiàn)住院老年患者日常生活能力的影響因素主要是疾病、喪偶、經(jīng)濟(jì)條件、年齡;住院老年患者抑郁的影響因素主要是疾病、婚姻狀況、住院時間、治療效果、自理能力、醫(yī)療費(fèi)用、業(yè)余愛好;住院老年患者焦慮的影響因素主要是婚姻狀況、家庭關(guān)系、是否獨(dú)居、文化程度;住院老年患者社會支持的影響因素主要是年齡、文化程度、是否獨(dú)居、家人關(guān)系。不難發(fā)現(xiàn),對住院老年患者日常生活能力、抑郁、焦慮、社會支持影響的共同因素主要有年齡、家庭關(guān)系、文化程度。并且通過相關(guān)性分析我們發(fā)現(xiàn),住院老年患者日常生活能力、抑郁、焦慮、社會支持這四個方面都存在相關(guān)關(guān)系,所以只要影響其中任何一項都會對其他三項造成相應(yīng)的影響。因而在考慮住院老年患者日常生活能力、抑郁、焦慮、社會支持的影響因素的時候,都要考慮疾病、喪偶、經(jīng)濟(jì)條件、年齡、婚姻狀況、住院時間、治療效果、自理能力、醫(yī)療費(fèi)用、業(yè)余愛好、文化程度、是否獨(dú)居、家人關(guān)系這些因素的任何一項,不能單純的只考慮其中的一項或者幾項,要綜合的考慮這些影響因素。本研究對醫(yī)院老年科住院老年患者的治療和護(hù)理有參考意義,同時也可以給幫組住院老年患者的有關(guān)機(jī)構(gòu)提供參考和依據(jù)。
[Abstract]:Objective to geriatrics, General Hospital of Chongqing city as an example to understand the hospitalized elderly patients, hospitalized elderly patients in daily life, depression, anxiety, social support problems, analysis of the impact of the 4 factors and relevance. Better help the hospitalized elderly patients with treatment, to identify ways to enhance the quality of life in elderly inpatients patients, the corresponding results, provide a reference for the treatment and nursing of elderly patients in the hospital to deal with, can also help in the group of elderly patients with the relevant agencies to provide reference and basis. This paper adopts the method of random sampling, and the data obtained by the international general information questionnaire and self-made questionnaire. Department of geriatric hospital according to the hospital, random sampling, questionnaire is used to investigate the different gender, age, ability of daily life in the elderly hospitalized under occupation and other demographic characteristics, Depression, anxiety, social support information, from the analysis of survey data summarized the general conclusion. Results a total of 400 questionnaires, 375 valid questionnaires, the response rate was 93.75%; among them, 219 men (41.60%), 156 female (59.40%). Social support: 375 valid questionnaires, the questionnaire scores less than 33 points in SSRs accounted for 20.80%, 33~45 accounted for between 74.40%, more than 45 points accounted for 4.80%. of hospitalized elderly patients is closely related to social support and hospitalization of elderly patients with depression and anxiety and the ability of daily life. Age, education level, living alone that family relationship are the influence factors of social support in elderly patients in hospital. The ability of daily life: 375 sample, can be completely independent (ADL score 100) to maintain the rate function, male 24.10%, female 31.40% of men and women; the damage rate of ADL in hospitalized elderly patients, 59.60% for female, male 75.90%. female hospitalized elderly patients with completely independent ability than men. Hospitalized elderly patients of all ages were 60-69 years: the damage rate of ADL was 95.40%, 70-79, 91.40%, 80-89, 87.20%, 90 years old and above 98.55%; the damage rate of ADL had significant difference in different age stages. Among hospitalized elderly patients is closely related with the daily life ability of hospitalized elderly patients with depression anxiety and social support. The disease, widowed, economic conditions, age and other factors are ADL. Conclusion the hospitalized elderly patients with depression, daily life. Study on anxiety and social support. The influencing factors of ability of daily life in elderly patients is the main disease, widowed, economic conditions, age; influencing factors of hospitalized elderly patients with depression is the main disease, marital status, living Hospital time, treatment effect, self-care ability, medical expenses, hobbies; influencing factors of hospitalization in elderly patients with anxiety is mainly marital status, family relationship, whether living alone, cultural level; influencing factors of social support in elderly patients hospitalized are age, education level, living alone, family relationship. It is not difficult to find, the ability of hospital and the daily life of elderly patients with depression, anxiety and social support are the main factors affecting the common age, family relationships, cultural degree. And through the correlation analysis we found that the hospitalized elderly patients in daily living, depression, anxiety, social support, these four aspects are correlated, so long as the impact of any one of them will the other three caused a corresponding impact. So considering the hospital capacity, daily life of elderly patients with depression, anxiety, when the influence of social support factors, should consider the disease, death Occasionally, economic conditions, marital status, age, duration of hospitalization, treatment effect, self-care ability, medical expenses, hobbies, cultural degree, whether living alone, any family relationship between these factors, not only consider one or several, to consider these factors. This study reference to the hospital geriatric hospital treatment and nursing care of elderly patients, but also to help hospitalized elderly patients group of relevant institutions to provide reference and basis.
