【摘要】：目的:構(gòu)建我國護士長風險管理能力評價指標體系,為臨床護士長風險管理能力評價提供科學的依據(jù)。并對指標體系進行初步應用,了解護士長風險管理能力的現(xiàn)況,分析影響護士長風險管理能力的主要因素。方法:采用文獻分析法、專家訪談法和Delphi專家咨詢法初步構(gòu)建指標體系的內(nèi)容維度,運用用層次分析法確定權(quán)重。在構(gòu)建的指標體系基礎(chǔ)上編制護士長風險管理能力的自評問卷,對山西2所三級甲等綜合醫(yī)院的100名護士長進行了預調(diào)查,分析信效度。在較好信效度的基礎(chǔ)上,對山西省8所三級甲等綜合醫(yī)院的285名護士長進行實測,分析護士長風險管理能力的影響因素。結(jié)果:1.初步構(gòu)建護士長風險管理能力指標體系兩輪專家咨詢的積極系數(shù)分別為0.82、0.86,共18位專家提出了修改意見,說明專家積極性高。專家咨詢結(jié)果顯示判斷依據(jù)(Ca)=0.922,熟悉程度(Cs)=0.855,權(quán)威程度(Cr)=0.889,專家權(quán)威程度較高;兩輪專家咨詢的變異系數(shù)(CV)均0.25。第二輪專家咨詢一級指標、二級指標的協(xié)調(diào)系數(shù)分別為:0.498、0.197,說明專家意見的協(xié)調(diào)性較一致。最終確立4個一級指標,其權(quán)重從高到低順序為護理風險識別能力(0.396)、護理風險處理能力(0.396)、護理風險評估能力(0.122)、護理風險管理效果評價能力(0.086)以及12個二級指標和35個三級指標的護士長風險管理能力測評指標體系。2.護士長風險管理能力問卷的信效度檢驗信度:總體的Cronbach'sα系數(shù)0.960,分半信度為0.884;效度:探索性因子分析結(jié)果表明在提取4個公因子后,累計方差貢獻率為64.97%。經(jīng)過最大方差正交旋轉(zhuǎn)后,每個一級指標均有單獨的因子來反映,指標結(jié)構(gòu)與預想的基本一致。3.護士長風險管理能力指標體系的初步應用通過單因素分析得出:影響護士長風險管理能力的因素有年齡、工作時間、學歷、職稱、繼續(xù)教育。多重線性回歸結(jié)果顯示:工作時間、學歷、繼續(xù)教育是影響護士長風險管理能力的主要因素,均呈正相關(guān)。結(jié)論:1.確立護士長風險管理能力評價指標體系,包括4個一級指標、12個二級指標、35個三級指標。2.編制護士長風險管理能力自評問卷,具有良好的信效度。3.影響護士長風險管理能力的因素有年齡、工作時間、學歷、職稱、繼續(xù)教育,其中工作時間、學歷、繼續(xù)教育有顯著影響。
[Abstract]:Objective: to construct the evaluation index system of head nurses' risk management ability in China, to provide scientific basis for the evaluation of head nurses' risk management ability, and to apply the index system preliminarily to understand the present situation of head nurses' risk management ability. Methods: literature analysis, expert interview and Delphi expert consultation were used to construct the content dimension of the index system. The weight was determined by AHP. Based on the established index system, the self-assessment questionnaire of head nurses' risk management ability was compiled, and 100 head nurses in 2 general hospitals of Grade 3A in Shanxi Province were investigated in advance. On the basis of good reliability and validity, 285 head nurses in 8 general hospitals of Grade 3A in Shanxi Province were measured. Results: 1. The positive coefficient of two rounds of expert consultation on the risk management ability index system of head nurses was 0.820.86 respectively. A total of 18 experts put forward suggestions for modification. The result of expert consultation shows that the judgment is based on the degree of familiarity with the degree of familiarity is 0.855, the degree of authority is equal to 0.889, and the degree of authority is higher, and the coefficient of variation of the two rounds of expert consultation is 0.25.First class index of the second round of expert consultation, The coordination coefficient of the secondary index is respectively 0. 498 / 0.197, which indicates that the coordination of the expert opinion is consistent. Finally, four primary indicators are established. The order of weight from high to low is nursing risk identification ability (0.396g), nursing risk processing ability (0.396g), nursing risk assessment ability (0.122), nursing risk management effect evaluation ability (0.086) and nurses Changfeng with 12 second class indexes and 35 third grade indexes. The reliability and validity of the head nurse risk management ability questionnaire: the overall Cronbach's 偽 coefficient was 0.960and the split-half reliability was 0.884; the validity: the exploratory factor analysis showed that after the extraction of four common factors, The cumulative variance contribution rate is 64.97. After orthogonal rotation of the maximum variance, each primary index has a separate factor to reflect, The primary application of the index system of the head nurse's risk management ability is obtained by single factor analysis: the factors influencing the head nurse's risk management ability are age, working time, educational background, title, etc. Continuous education. Multiple linear regression results show that: working hours, academic qualifications, continuing education are the main factors that affect the head nurses' risk management ability, all of them are positively correlated. Conclusion: 1. Establish the evaluation index system of head nurses' risk management ability. The self-evaluation questionnaire of head nurses' risk management ability has good reliability and validity. The factors influencing the head nurses' risk management ability are age, working time, academic background, title, etc. Continuing education, including working hours, academic qualifications, continuing education has a significant impact.
