【摘要】：目的探討超聲輔助下頸內(nèi)靜脈穿刺在嬰幼兒完全植入式靜脈輸液港(TIVAP)植入術(shù)中的效果。方法回顧性分析2009年1月至2016年7月在上海交通大學醫(yī)學院附屬兒童醫(yī)學中心(單中心)治療的446例TIVAP植入術(shù)患兒臨床資料,患兒平均年齡1.3歲(4個月~2.8歲)。比較傳統(tǒng)頸內(nèi)靜脈盲穿刺和超聲輔助下頸內(nèi)靜脈穿刺耗時、一次穿刺成功率、穿刺并發(fā)癥發(fā)生率,并作統(tǒng)計學分析。結(jié)果446例患兒中頸內(nèi)靜脈盲穿刺265例,平均需時7.6 min,一次穿刺成功201例(75.85%),發(fā)生穿刺并發(fā)癥15例(5.66%);超聲輔助下頸內(nèi)靜脈穿刺181例,平均需時4.2 min,一次穿刺成功176例(97.24%),發(fā)生穿刺并發(fā)癥3例(1.70%)。結(jié)論嬰幼兒TIVAP植入術(shù)時采用超聲輔助下頸內(nèi)靜脈穿刺,可縮短手術(shù)時間,提高一次穿刺成功率并減少穿刺并發(fā)癥發(fā)生,是一種安全有效、簡單可行的技術(shù),值得臨床推廣。
[Abstract]:Objective to investigate the effect of ultrasound assisted internal jugular venipuncture (IVAP) on infantile infantile total implanted intravenous infusion (IVAP). Effect of implantation. Methods retrospective analysis was made at the Children's Medical Center affiliated to the School of Medicine of Shanghai Jiaotong University from January 2009 to July 2016 (single center). The clinical data of 446 children with TIVAP implantation were analyzed. The average age of the children was 1.3 years (4 months to 2.8 years old). Compared with the traditional internal jugular vein blind puncture and ultrasound assisted internal jugular vein puncture, the success rate of single puncture and the incidence of complications were compared. Results there were 265 cases of internal jugular vein blind puncture in 446 cases, the average time was 7.6 mins, 201 cases were successful in one puncture (75.85). Complications of puncture occurred in 15 cases. Ultrasound assisted internal jugular vein puncture was performed in 181 cases, with an average time of 4.2 minutes. 176 cases were successful in one puncture. Conclusion Ultrasound-assisted internal jugular vein puncture can shorten the operation time of TIVAP implantation in infants. It is a safe, effective, simple and feasible technique to improve the success rate of single puncture and reduce the complications of puncture, which is worth popularizing in clinic.
