【摘要】：目的總結(jié)分析乳腺Paget病的臨床表現(xiàn)及超聲圖像特征。方法以術(shù)后組織病理學(xué)診斷為標(biāo)準(zhǔn),對(duì)34例乳腺Paget病的臨床資料、超聲表現(xiàn)進(jìn)行分析,總結(jié)乳腺Paget病的臨床表現(xiàn)和超聲聲像圖特征。結(jié)果 34例患者中,單一Paget病7例,合并浸潤(rùn)性導(dǎo)管癌16例,合并導(dǎo)管原位癌7例,同時(shí)合并浸潤(rùn)性導(dǎo)管癌和導(dǎo)管內(nèi)癌3例,合并其他病理類(lèi)型癌1例;有Paget病典型臨床表現(xiàn)的24例,無(wú)Paget病典型臨床表現(xiàn)的10例。24例有Paget病典型臨床表現(xiàn)的患者中,超聲提示乳頭或乳暈區(qū)回聲異常11例,乳腺腫物14例,微鈣化14例;10例無(wú)Paget病典型臨床表現(xiàn)的患者中,超聲提示乳頭或乳暈區(qū)回聲異常3例,乳腺腫物9例,微鈣化6例。14例超聲提示乳頭或乳暈區(qū)回聲異常的患者中,彩色多普勒超聲均提示乳頭局部血流豐富。結(jié)論乳腺Paget病主要表現(xiàn)為乳頭或乳暈區(qū)皮膚的改變,高頻超聲可敏感識(shí)別乳頭及乳暈區(qū)回聲異常。對(duì)于超聲發(fā)現(xiàn)的乳腺病灶,應(yīng)對(duì)其乳頭及乳暈區(qū)進(jìn)行仔細(xì)掃查,避免Paget病漏診。
[Abstract]:Objective to summarize the clinical manifestation and ultrasound image feature analysis of Paget's disease of the breast. Methods the postoperative histopathological diagnosis as the standard, the clinical data of 34 cases with Paget disease of the breast, analyze the ultrasonic manifestation of Paget's disease of the breast, summarize the clinical manifestations and ultrasonographic features. Results of the 34 patients, 7 cases of single Paget disease, with 16 cases of infiltrating ductal carcinoma, 7 cases with ductal carcinoma in situ, combined with invasive ductal carcinoma and ductal carcinoma in 3 cases, combined with other pathological types of carcinoma in 1 cases; 24 cases had typical clinical manifestations of Paget disease, 10 cases of.24 patients with typical clinical manifestations of Paget disease with typical clinical manifestations of Paget disease the patients, ultrasound echo nipple or areola of breast abnormalities in 11 cases, 14 cases of breast tumor, micro calcification in 14 cases; 10 cases of patients with Paget disease without typical clinical manifestations in the nipple or areola of breast ultrasound echo abnormalities in 3 cases, 9 cases of breast tumor, micro calcification in 6 cases.14 Cases of ultrasound echo nipple or areola of breast abnormalities in patients with color Doppler ultrasound showed papillary local rich blood. Conclusion Paget disease of the breast is mainly the nipple or areola skin changes, the high-frequency ultrasonic echo sensitive identification of the nipple and areola area. The ultrasound found abnormal breast lesions, the nipple and areola of breast careful scanning, avoid Paget disease misdiagnosis.
