面向時(shí)變需求的急診室醫(yī)生周排班研究

【摘要】:急診室病人的到達(dá)率隨時(shí)間動(dòng)態(tài)變化,醫(yī)院現(xiàn)行的固定時(shí)間排班模式并不能滿足病人的時(shí)變需求,因此出現(xiàn)病人到達(dá)高峰時(shí)段醫(yī)生嚴(yán)重不足,病人等待時(shí)間過(guò)長(zhǎng),而在病人到達(dá)率低的時(shí)段醫(yī)生工作時(shí)間利用率不高的現(xiàn)象。針對(duì)此問(wèn)題,以周為單位求解醫(yī)護(hù)人員的排班問(wèn)題。首先基于排隊(duì)論,提出一種可以估計(jì)動(dòng)態(tài)需求系統(tǒng)中病人等待時(shí)間的解析方法,然后考慮病人和醫(yī)生雙方因素,在一系列規(guī)則約束下建立數(shù)學(xué)模型,以期得到最小化病人等待時(shí)間的靈活排班方案,在此基礎(chǔ)上設(shè)計(jì)了變鄰域搜索算法求解該問(wèn)題。通過(guò)數(shù)值實(shí)驗(yàn),驗(yàn)證了所提出模型和算法對(duì)醫(yī)院管理工作的意義。
[Abstract]:The arrival rate of the patients in the emergency room changes with time. The current fixed time schedule mode can not meet the time-varying needs of the patients. Therefore, there is a serious shortage of doctors during the peak period, and the waiting time of the patients is too long. But when the patient arrival rate is low, the utilization rate of doctor's working time is not high. In order to solve this problem, we take week as the unit to solve the scheduling problem of medical staff. First of all, based on the queuing theory, This paper presents an analytical method to estimate the waiting time of patients in dynamic demand systems. Then considering the factors of both patients and doctors, a mathematical model is established under a series of rules. Based on this, a variable neighborhood search algorithm is designed to solve the problem. The significance of the proposed model and algorithm to hospital management is verified by numerical experiments.
