耐甲氧西林金黃色葡萄球菌SCCmec基因分型及耐藥性分析

【摘要】:目的分析耐甲氧西林金黃色葡萄球菌(MRSA)SCCmec基因型別及耐藥性,為不同感染型別菌株的有效治療及耐藥性控制提供指導(dǎo)。方法收集本院2016年1-12月患者各類送檢臨床標(biāo)本,分離MRSA,對(duì)菌株進(jìn)行SCCmec基因的多重PCR擴(kuò)增,K-B法分析SCCmec基因型菌株耐藥情況,根據(jù)標(biāo)準(zhǔn)判定結(jié)果。結(jié)果本院2016年1-12月患者各類臨床標(biāo)本中共分離出MRSA 282株,攜帶SCCmec基因151株,占53.55%,其中143株攜帶SCCmecⅢ型基因,8株攜帶SCCmecⅡ型基因。攜帶SCCmecⅢ型基因MRSA病區(qū)分布:ICU病房81株(占56.64%),外科病房39株(占27.27%),內(nèi)科病房16株(占11.19%),門診病房7株(占4.90%)。攜帶SCCmecⅡ型基因MRSA病區(qū)分布:ICU病房5株(占62.50%),外科病房2株(占25.00%),內(nèi)科病房1株(占12.50%),門診病房分離的MRSA菌株未檢出SCCmecⅡ型。攜帶SCCmecⅢ型基因的MRSA標(biāo)本來(lái)源:痰液93株(占65.03%),血液37株(占25.87%),腹水9株(占6.29%),尿液3株(占2.10%),傷口分泌物1株(占0.70%)。攜帶SCCmecⅡ型基因MRSA標(biāo)本來(lái)源:痰液5株(占62.50%),血液3株(占37.50%),患者腹水、尿液標(biāo)本以及傷口分泌物標(biāo)本分離的MRSA菌株未檢出SCCmecⅡ型。攜帶SCCmecⅢ型MRSA對(duì)氨芐西林、阿莫西林、哌拉西林、頭孢拉啶、頭孢吡肟和利福平的耐藥率分別為100.00%、67.83%、53.85%、60.13%、26.57%和8.39%,對(duì)萬(wàn)古霉素敏感;攜帶SCCmecⅡ型MRSA對(duì)氨芐西林、阿莫西林、哌拉西林、頭孢拉啶、頭孢吡肟和利福平的耐藥率分別為100.00%、75.00%、37.50%、25.00%、12.50%和12.50%,對(duì)萬(wàn)古霉素敏感。結(jié)論 MRSA臨床分離株基因型以SCCmecⅢ型為主,主要分離自醫(yī)院的ICU病房及患者的痰液和血液標(biāo)本。臨床治療MRSA感染時(shí)應(yīng)首選萬(wàn)古霉素,但應(yīng)合理選用,避免耐藥性發(fā)展。
