【摘要】：自2009年開始,中國(guó)進(jìn)入后新農(nóng)合時(shí)代,新農(nóng)合在增進(jìn)農(nóng)村醫(yī)療服務(wù)質(zhì)量,提高農(nóng)村居民健康素養(yǎng)方面效果突出,但面臨的阻力不斷增大。本文運(yùn)用Heckman雙階段模型和一手調(diào)研數(shù)據(jù)研究農(nóng)村居民對(duì)新農(nóng)合參與的影響因素、滿意度、支付意愿等重要問題。當(dāng)前參合居民對(duì)新農(nóng)合滿意度較高,可接受的最大參合費(fèi)用均值為185.55元·人/年。防大病、節(jié)省醫(yī)藥開支是農(nóng)村居民繼續(xù)參合的主要原因,參合費(fèi)用太高則成為參合者中斷參與新農(nóng)合的首要因素,而醫(yī)院診療費(fèi)用增幅過大、參合費(fèi)用上漲太快、大病報(bào)銷比例偏低、政策宣傳不夠等則是參合者的主要擔(dān)憂。個(gè)人特征變量對(duì)個(gè)體參與意愿與支付意愿選擇的影響比較微弱,家庭特征變量(家庭年收入、是否有欠款、是否通村公路、醫(yī)療距離、是否開通有限電視)和參與特征變量(參合時(shí)長(zhǎng)、新農(nóng)合滿意度、門診報(bào)銷是否超出補(bǔ)償上限、是否報(bào)銷過住院(手術(shù))費(fèi))等是決定參合者參與意愿與支付意愿的關(guān)鍵變量。本文最后給出了相關(guān)政策建議。
[Abstract]:Since 2009, China has entered the post-new era of rural cooperation, the new rural cooperation in improving the quality of rural medical services, improve the health literacy of rural residents outstanding effect, but the resistance is increasing. This paper uses Heckman two-stage model and primary survey data to study the factors influencing rural residents' participation in NCMS, satisfaction, willingness to pay and other important issues. At present, the satisfaction of participating residents to NCMS is higher, the average of maximum acceptable participation cost is 185.55 yuan per year. The main reason for rural residents to continue to participate in rural areas is to prevent serious illness and save medical expenses. The participation cost is too high and becomes the primary factor for participants to interrupt their participation in the New Rural Cooperative Cooperation (NCMS). However, the cost of hospital consultation and treatment has increased too much, and the participation cost has risen too quickly. Serious illness reimbursement rate is low, policy propaganda is insufficient is the main concern of participants. The influence of individual characteristic variables on individual willingness to participate and willingness to pay is relatively weak. Family characteristics variables (annual household income, whether there is any arrears, whether access to the village road, medical distance, etc.) The key variables to determine the participants' willingness to participate and pay are whether the limited TV is opened or not and the participation characteristic variables (the duration of participation, the satisfaction degree of NCMS, whether the outpatient reimbursement exceeds the upper limit of compensation, and whether the cost of hospitalization (operation) is reimbursed). At the end of this paper, some policy suggestions are given.
