【摘要】：當(dāng)前,探索穩(wěn)定、可行的長(zhǎng)期護(hù)理保險(xiǎn)制度是我國(guó)亟待完成的任務(wù)。社會(huì)長(zhǎng)期護(hù)理保險(xiǎn)在我國(guó)是新生制度,亟待總結(jié)國(guó)內(nèi)外的相關(guān)經(jīng)驗(yàn)。首先綜述國(guó)際上長(zhǎng)期護(hù)理保障制度理論和實(shí)踐中的模式,發(fā)現(xiàn)雖然長(zhǎng)期護(hù)理保障制度按籌資來(lái)源可分為財(cái)政支持、社會(huì)保險(xiǎn)、私營(yíng)保險(xiǎn)和依托醫(yī)療保障計(jì)劃籌資四種,但大多國(guó)家實(shí)行混合制度;繼而總結(jié)了國(guó)際長(zhǎng)期護(hù)理保險(xiǎn)制度的發(fā)展趨勢(shì)。隨后,選取德國(guó)、荷蘭、日本、韓國(guó)四個(gè)國(guó)家作為典型案例,對(duì)四國(guó)長(zhǎng)期護(hù)理保險(xiǎn)制度的制定背景和政策細(xì)節(jié)(保險(xiǎn)人、被保險(xiǎn)人、繳費(fèi)機(jī)制、財(cái)務(wù)結(jié)構(gòu)、給付對(duì)象、護(hù)理等級(jí)分類、等級(jí)評(píng)定機(jī)構(gòu)和流程、待遇給付方式和內(nèi)容)進(jìn)行比較研究和歸納。在此基礎(chǔ)上,按照同樣邏輯,比較研究國(guó)內(nèi)先行自發(fā)探索長(zhǎng)期護(hù)理保險(xiǎn)制度的青島、上海、長(zhǎng)春和南通四個(gè)城市的制度政策細(xì)節(jié)。最后,認(rèn)為我國(guó)在長(zhǎng)期護(hù)理保險(xiǎn)制度實(shí)踐中應(yīng)考慮如下問題:應(yīng)該建立面向全民的長(zhǎng)期護(hù)理保障制度;應(yīng)依附于基本醫(yī)療保險(xiǎn),由醫(yī)療保險(xiǎn)經(jīng)辦機(jī)構(gòu)承擔(dān)長(zhǎng)期護(hù)理保險(xiǎn)經(jīng)辦業(yè)務(wù);大部分地區(qū)短期內(nèi)應(yīng)建立職工和城鄉(xiāng)居民不同的籌資和待遇體系;待遇確定機(jī)構(gòu)應(yīng)為多方面利益代表聯(lián)合組成的委員會(huì),適宜采取經(jīng)辦機(jī)構(gòu)派員入戶評(píng)估、醫(yī)生審查、委員會(huì)復(fù)核的三段式方式;待遇供給方面,短期內(nèi)應(yīng)以老年人為主要服務(wù)對(duì)象,進(jìn)而考慮失能人群、再考慮失智人群,有條件的地方可以全覆蓋;待遇內(nèi)容設(shè)計(jì)的原則應(yīng)該是強(qiáng)調(diào)�；�、鼓勵(lì)家庭履行照護(hù)義務(wù)、鼓勵(lì)社區(qū)護(hù)理服務(wù)、強(qiáng)調(diào)前期預(yù)防保健和康復(fù)。
[Abstract]:At present, exploring a stable and feasible long-term nursing insurance system is an urgent task in our country. Social long-term nursing insurance is a freshman system in our country, and it is urgent to sum up the relevant experience at home and abroad. First of all, this paper summarizes the theory and practice model of long-term nursing security system in the world, and finds that although the long-term nursing security system can be divided into four kinds according to the source of financing: financial support, social insurance, private insurance and financing based on medical security scheme. However, most countries have a mixed system; Then it summarizes the development trend of international long-term nursing insurance system. Then, four countries, Germany, Netherlands, Japan and South Korea, are selected as typical cases, and the background and policy details of the long-term care insurance system in the four countries (insurer, insured, payment mechanism, financial structure, object of payment) are selected as typical cases. The classification of nursing grade, the organization and process of grade evaluation, the way and content of treatment and payment) were compared and summarized. On this basis, according to the same logic, this paper makes a comparative study on the institutional policy details of Qingdao, Shanghai, Changchun and Nantong, which spontaneously explore the long-term nursing insurance system in China. Finally, it is considered that the following problems should be considered in the practice of long-term nursing insurance system in China: a long-term nursing security system for the whole people should be established; It should be attached to the basic medical insurance, and the medical insurance agencies should undertake the business of long-term nursing insurance. In most areas, different financing and treatment systems for workers and urban and rural residents should be established in the short term. The treatment determination organization should be a committee composed of representatives of various interests, and it is appropriate to adopt the three-stage way of sending staff to evaluate, examine by doctors and review by the committee. In terms of treatment and supply, the elderly should be taken as the main service object in the short term, and then the disabled population should be considered, and then the mentally retarded population should be considered. The principle of treatment content design should be to emphasize the basic protection, to encourage the family to fulfill the duty of care, to encourage community nursing services, and to emphasize preventive health care and rehabilitation in the early stage.
