【摘要】：伴隨著人口老齡化進(jìn)程的不斷深化,健康老齡化以及老年人醫(yī)療費用的問題得到了廣泛關(guān)注。由于受到數(shù)據(jù)等各方面因素的限制,我國國內(nèi)關(guān)于分健康狀態(tài)老年人的醫(yī)療費用研究相對較少。本文結(jié)合我國第六次人口普查數(shù)據(jù)和“中國健康與養(yǎng)老追蹤調(diào)查(CHARLS)"項目搜集的有關(guān)老年人醫(yī)療費用的數(shù)據(jù)測算分健康狀態(tài)老人的醫(yī)療費用,針對老齡化程度的日益嚴(yán)重下老年人的醫(yī)療衛(wèi)生服務(wù)需求和醫(yī)療費用的逐漸加大的問題,以及目前我國醫(yī)療保險制度實際報償額度尤其是農(nóng)村醫(yī)療保險非常低下的問題,提出政府應(yīng)出資為60歲以上老年人提供額外的自付費用報銷。依據(jù)老年人在“不能自理”狀態(tài)下支付的醫(yī)療費用占總費用的比例,對自付費用報銷比例設(shè)定如下標(biāo)準(zhǔn)：60-69歲人群醫(yī)療費用自付部分的25%納入報銷范圍,70-79歲人群醫(yī)療費用自付部分的35%納入報銷范圍,80-89歲人群醫(yī)療費用自付部分的50%納入報銷范圍,90以上歲人群醫(yī)療費用自付部分的65%納入報銷范圍。同時本文最后根據(jù)對Charls數(shù)據(jù)分析中發(fā)現(xiàn)的問題提出了相應(yīng)的醫(yī)療保險制度優(yōu)化政策建議。
[Abstract]:With the deepening of population aging process, the problem of healthy aging and elderly medical expenses has been paid more and more attention. Due to the limitation of data and other factors, there is relatively little research on the medical cost of the elderly in China. Based on the data of the sixth population Census of China and the data collected by the Chinese Health and Old-age tracking Survey (CHARLS) project on the medical expenses of the elderly, this paper calculates the medical costs of the healthy elderly. In view of the problem of increasing demand for medical and health services and medical expenses of the elderly under the increasingly serious situation of aging, and the problem that the actual compensation amount of medical insurance system in our country, especially the rural medical insurance, is very low at present, It is suggested that the government should provide additional reimbursement for out-of-pocket expenses for people over 60 years of age. According to the proportion of medical expenses paid by the elderly to the total expenses under the condition of "not being able to take care of themselves", the following criteria were set for the proportion of out-of-pocket expenses reimbursed: 25% of the out-of-pocket medical expenses of the 60-69 age group were included in the reimbursement scope. 35% of the out-of-pocket medical expenses of people aged 70-79 years were included in the reimbursement scope, 50% of the medical expenses out-of-pocket portion of the 80-89 years old people were included in the reimbursement scope, and 65% of the medical expenses out-of-pocket portion of the people aged over 90 years were included in the reimbursement scope. At the same time, according to the problems found in the analysis of Charls data, this paper puts forward the corresponding policy recommendations on the optimization of medical insurance system.
