【摘要】：新農(nóng)合醫(yī)療費用的快速增長引起了廣泛關(guān)注。本文利用江西省87個縣2008—2010年數(shù)據(jù),基于SDA結(jié)構(gòu)分解方法將新農(nóng)合費用增長分解為規(guī)模效應(yīng)、流動效應(yīng)以及費用膨脹效應(yīng),對新農(nóng)合醫(yī)療費用增長的動因及其一般規(guī)律進(jìn)行分析。研究發(fā)現(xiàn),2009年江西省新農(nóng)合費用增長主要是因為新增住院次數(shù)的增加,反映出醫(yī)療需求的釋放;2010年住院費用大幅上漲則更多體現(xiàn)在流動效應(yīng)和費用膨脹效應(yīng)的增加,反映出參合農(nóng)民醫(yī)療需求層次的提高;收入水平、次均住院費用大小及其偏離度是影響新農(nóng)合費用增長結(jié)構(gòu)的主要因素。研究結(jié)論表明,現(xiàn)階段新農(nóng)合醫(yī)療費用控制制度應(yīng)體現(xiàn)差異化,縣級醫(yī)療重在費用控制,鄉(xiāng)級醫(yī)療重在提升服務(wù)水平和醫(yī)療質(zhì)量。
[Abstract]:The rapid increase in medical expenses of NCMS has attracted wide attention. Based on the data of 87 counties in Jiangxi Province from 2008 to 2010, the cost growth of NCMS is decomposed into scale effect, flow effect and cost expansion effect based on SDA structure decomposition method. This paper analyzes the causes and general rules of the increase of medical expenses in NCMS. The study found that the increase in the cost of NCMS in Jiangxi Province in 2009 was mainly due to the increase in the number of new hospitalizations, which reflected the release of medical needs; the sharp increase in hospitalization costs in 2010 was more reflected in the increase of the flow effect and the cost expansion effect. The level of income, average hospitalization cost and its deviation are the main factors that influence the structure of new rural cooperative expenses. The research results show that the new rural cooperative medical expense control system at present should reflect the differentiation, the county level medical treatment should focus on the cost control, the township level medical treatment should enhance the service level and the medical treatment quality.
