【摘要】:繳費(fèi)意愿是提高農(nóng)民工基本醫(yī)療保險(xiǎn)參保率的重要標(biāo)準(zhǔn)。農(nóng)民工醫(yī)保繳費(fèi)意愿越強(qiáng)烈,參保積極性也就越高,也能夠整體上提高我國(guó)農(nóng)民工基本醫(yī)療保險(xiǎn)的覆蓋率。我國(guó)農(nóng)民工在城市從事的多數(shù)是環(huán)境較差、條件艱苦、勞動(dòng)時(shí)間長(zhǎng)、疾病風(fēng)險(xiǎn)較大的工作,對(duì)自身健康消耗極大。然而,多數(shù)農(nóng)民工缺乏健康維護(hù)意識(shí),對(duì)基本醫(yī)療保險(xiǎn)的認(rèn)識(shí)程度也不夠深入,大多以消耗健康來(lái)賺取收入,不利于農(nóng)民工長(zhǎng)久的勞動(dòng)生產(chǎn),F(xiàn)階段,農(nóng)民工的整體覆蓋率較低,我國(guó)醫(yī)保制度區(qū)域分割和城鄉(xiāng)分割的特點(diǎn),又阻礙了農(nóng)民工獲取相應(yīng)的醫(yī)療保障待遇,這顯然和我國(guó)社會(huì)保障制度發(fā)展的目標(biāo)相違背。因此,我們有必要對(duì)農(nóng)民工的醫(yī)保繳費(fèi)意愿進(jìn)行研究,以提高農(nóng)民工基本醫(yī)療保險(xiǎn)的參保率和保障水平,推動(dòng)全國(guó)統(tǒng)一勞動(dòng)力市場(chǎng)建設(shè)。本研究首先對(duì)選題的研究背景和意義進(jìn)行了闡釋,回顧了國(guó)內(nèi)外學(xué)者對(duì)農(nóng)民工基本醫(yī)療保險(xiǎn)繳費(fèi)意愿研究的文獻(xiàn)綜述,并介紹了本文的理論基礎(chǔ),即Grossman健康需求理論和福利經(jīng)濟(jì)學(xué)理論,然后對(duì)本文關(guān)鍵名詞進(jìn)行了概念界定。其次,文章梳理了農(nóng)民工基本醫(yī)療保險(xiǎn)的政策歷程,對(duì)農(nóng)民工的醫(yī)保參,F(xiàn)狀和繳費(fèi)能力進(jìn)行分析,并立足于訪談法研究影響農(nóng)民工醫(yī)保繳費(fèi)能力的潛在因素。再次,本文立足于實(shí)地調(diào)研數(shù)據(jù),實(shí)證研究了農(nóng)民工基本醫(yī)療保險(xiǎn)繳費(fèi)意愿的影響因素,重點(diǎn)考察的是醫(yī)保報(bào)銷方便程度和健康變量對(duì)農(nóng)民工醫(yī)保繳費(fèi)意愿的影響。最后,文章基于實(shí)證研究結(jié)果,提出了政策建議。本文研究發(fā)現(xiàn),我國(guó)農(nóng)民工基本醫(yī)療保險(xiǎn)的參,F(xiàn)狀為整體參保率低、同時(shí)參保率存在行業(yè)和區(qū)域差異;農(nóng)民工的收入和消費(fèi)水平與醫(yī)保繳費(fèi)能力存在關(guān)聯(lián)性。且實(shí)證研究結(jié)果表明,醫(yī)療保險(xiǎn)報(bào)銷程度越方便顯著提高了農(nóng)民工的繳費(fèi)意愿;健康狀況越差則農(nóng)民工的繳費(fèi)意愿越低;接受過職業(yè)培訓(xùn)的農(nóng)民工有較強(qiáng)的繳費(fèi)意愿;農(nóng)民工的收入和醫(yī)保繳費(fèi)意愿呈現(xiàn)顯著的正相關(guān)關(guān)系;工作環(huán)境越差、轉(zhuǎn)換城市工作次數(shù)越多則繳費(fèi)意愿越低;家庭耕地面積越多的農(nóng)民工其繳費(fèi)意愿越大。為了更好提高農(nóng)民工的醫(yī)保繳費(fèi)意愿,增強(qiáng)農(nóng)民工的醫(yī)保繳費(fèi)能力,本文建議采取以下措施:第一,建立統(tǒng)一的基本醫(yī)療保險(xiǎn)異地報(bào)銷制度。可建立區(qū)域間醫(yī)保聯(lián)網(wǎng)系統(tǒng)、建立規(guī)范的醫(yī)保信息管理體系、加快城鄉(xiāng)基本醫(yī)療保險(xiǎn)一體化建設(shè),以推動(dòng)統(tǒng)一的基本醫(yī)療保險(xiǎn)異地報(bào)銷制度建設(shè)。第二,強(qiáng)化對(duì)外出農(nóng)民工的健康教育。政府部門應(yīng)建立農(nóng)民工健康教育培訓(xùn)班,農(nóng)民工個(gè)人應(yīng)努力學(xué)習(xí)健康知識(shí),社會(huì)輿論應(yīng)加大全民健康宣傳,推動(dòng)建立人人都維護(hù)健康的社會(huì)大環(huán)境。第三,加強(qiáng)財(cái)政對(duì)農(nóng)民工基本醫(yī)療保險(xiǎn)個(gè)人繳費(fèi)部分的補(bǔ)助。政府應(yīng)該對(duì)困難農(nóng)民工個(gè)人繳費(fèi)實(shí)行補(bǔ)助,進(jìn)而促進(jìn)困難農(nóng)民工享有醫(yī)療保障待遇。第四,設(shè)立農(nóng)民工基本醫(yī)療保險(xiǎn)多種繳費(fèi)檔次。將醫(yī)保繳費(fèi)按照一定的檔次進(jìn)行劃分,能夠讓農(nóng)民工獲得足夠的選擇空間,應(yīng)進(jìn)一步落實(shí)企業(yè)的繳費(fèi)責(zé)任,擴(kuò)大覆蓋面,建立繳費(fèi)檔次轉(zhuǎn)接制度,從而有效保障農(nóng)民工的醫(yī)療保障權(quán)益。
[Abstract]:The willingness to pay the fee is an important standard to improve the insurance rate of basic medical insurance for migrant workers. The stronger the willingness of migrant workers to pay for medical insurance, the higher the enthusiasm of the migrant workers, the higher the coverage rate of the basic medical insurance for migrant workers in our country. However, the majority of migrant workers lack the awareness of health maintenance, and the understanding of basic medical insurance is not deep enough. Most of them make income from the consumption of health, which is not conducive to the long-term labor production of migrant workers. At this stage, the overall coverage rate of peasant workers is low, and the regional division of medical insurance system in China is divided. The characteristics of cutting and urban and rural division have hindered the migrant workers from getting the corresponding medical treatment, which is obviously contrary to the goal of the development of social security system in our country. Therefore, it is necessary to study the willingness to pay for medical insurance for migrant workers so as to improve the insurance rate and the level of insurance for the basic medical insurance of migrant workers, and to promote the national unified work. This study first explains the research background and significance of the topic, reviews the literature review of scholars at home and abroad on the willingness to pay for basic medical insurance for migrant workers, and introduces the theoretical basis of this paper, namely the Grossman health demand theory and the theory of welfare economics, and then the key nouns of this paper have been carried out. Secondly, the article combed the basic medical insurance policy of migrant workers, analyzed the status of medical insurance participation and payment ability of migrant workers, and based on the interview method to study the potential factors affecting the medical insurance payment ability of migrant workers. Again, this paper, based on the field survey data, empirically studied the basic medical insurance payment of migrant workers. The influence factors of the willingness to pay are focused on the effect of the convenience of medical insurance reimbursement and the influence of health variables on the willingness to pay for medical insurance of migrant workers. Finally, based on the results of the empirical research, the article puts forward the policy suggestions. The income and consumption level of migrant workers are related to the payment ability of medical insurance. The results of the empirical study show that the more convenient the medical insurance reimbursement is, the better the willingness to pay for the migrant workers; the worse the health condition is, the lower the willingness of the migrant workers to pay. There is a significant positive correlation between the income of migrant workers and the willingness to pay for medical insurance; the worse the working environment, the more the conversion of urban work, the lower the willingness to pay, the greater the willingness to pay for the more rural migrant workers in the family. In order to better improve the willingness to pay for medical insurance and enhance the medical insurance payment ability of migrant workers, this paper suggests that the migrant workers' medical insurance payment ability will be enhanced. The following measures are taken as follows: first, establish a unified basic medical insurance foreign reimbursement system, establish an interregional medical insurance network system, establish a standardized medical insurance information management system, accelerate the integration of urban and rural basic medical insurance, in order to promote the construction of a unified basic medical insurance reimbursement system. Second, strengthen the health of migrant workers. Education. Government departments should set up a training class for health education for migrant workers. The individual should work hard to learn health knowledge. Public opinion should increase public health publicity and promote the establishment of a healthy social environment for all. Third, strengthen the financial subsidy to the personal contribution of basic medical insurance for migrant workers. The personal payment of migrant workers should be subsidized to promote the medical insurance treatment for the migrant workers. Fourth, the basic medical insurance of migrant workers is set up. The medical insurance payment can be divided in accordance with a certain grade, which can make the migrant workers get enough space. We should carry out the payment responsibility of the enterprises, expand the coverage and establish the coverage. The transfer system of payment grades can effectively protect the rights and interests of medical insurance for migrant workers.
