【摘要】：進入80年代,我國經(jīng)濟制度從計劃經(jīng)濟向社會主義市場經(jīng)濟逐步轉型,我國醫(yī)療保障制度也從過去福利型的公費、勞保醫(yī)療制度,轉變到社會化管理的基本醫(yī)療保險制度。期間,建立起的基本醫(yī)療保險制度適應了我國社會主義市場經(jīng)濟體制,在保障廣大人民群眾基本醫(yī)療需求方面發(fā)揮了重要的作用。但隨著社會的進一步發(fā)展,我國現(xiàn)有的基本醫(yī)療保險制度在取得顯著成績的同時,也暴露出不少問題。其中一個十分關鍵的問題是中國城鄉(xiāng)分割的二元保險制度已經(jīng)成為城鄉(xiāng)經(jīng)濟、社會統(tǒng)籌發(fā)展的嚴重制約因素。因此,探索城鎮(zhèn)居民與農村居民醫(yī)療保險制度統(tǒng)籌一體化,就成為我國醫(yī)藥衛(wèi)生體制改革中值得研究的新課題。 本研究立足廣西區(qū)情,綜合運用文獻研究法、實證研究法、問卷調查法、專家咨詢法等相關研究方法對廣西統(tǒng)籌城鄉(xiāng)居民基本醫(yī)療保險這一課題開展研究。一方面通過設計發(fā)放調查問卷,收集整理相關數(shù)據(jù),詳細分析廣西基本醫(yī)療保險的參保人群,繳費標準、保險待遇等情況,針對當前廣西城鎮(zhèn)居民醫(yī)療保險和新型農村合作醫(yī)療保險存在的突出問題進行深入剖析,找出廣西統(tǒng)籌城鄉(xiāng)居民醫(yī)療保險的難點。另一方面,根據(jù)經(jīng)濟基礎的相似性、人口結構的相似性等現(xiàn)實條件,選取成都和重慶兩市統(tǒng)籌城鄉(xiāng)居民基本醫(yī)療保險的經(jīng)驗做法加以借鑒,探尋廣西城鄉(xiāng)醫(yī)保資源的整合途徑,以此縮小人群之間、城鄉(xiāng)之間、區(qū)域之間的社會醫(yī)療保險補償差別,建立具有良性循環(huán)和穩(wěn)定特性的參保機制,增強社會醫(yī)療保險的共濟性、公平性和可持續(xù)性,為廣西衛(wèi)生事業(yè)的進一步發(fā)展提供參考。
[Abstract]:In the 1980s, the economic system of our country has gradually transformed from planned economy to socialist market economy, and the medical security system of our country has also changed from the welfare type of public expense and labor insurance medical system to the basic medical insurance system of socialized management. During this period, the basic medical insurance system has adapted to the socialist market economic system of our country and played an important role in protecting the basic medical needs of the broad masses of the people. However, with the further development of society, the existing basic medical insurance system in China has made remarkable achievements, but also exposed a lot of problems. One of the key problems is that China's urban-rural division of the dual insurance system has become a serious constraint on urban and rural economic and social development. Therefore, exploring the integration of medical insurance system between urban and rural residents has become a new topic worth studying in the reform of medical and health system. This study is based on the situation of Guangxi, comprehensive use of literature research, empirical research, questionnaire, expert consultation and other related research methods to Guangxi as a whole urban and rural residents of the basic medical insurance to carry out research. On the one hand, by designing and issuing questionnaires, collecting and collating relevant data, and analyzing in detail the insured population, payment standards and insurance benefits of basic medical insurance in Guangxi, Aiming at the prominent problems existing in the current urban and rural residents' medical insurance in Guangxi and the new rural cooperative medical insurance, the difficulties of Guangxi's overall planning of urban and rural residents' medical insurance are found out. On the other hand, according to the similarity of economic base, the similarity of population structure and other realistic conditions, this paper selects Chengdu and Chongqing to coordinate the experience of urban and rural residents' basic medical insurance for reference, and explores the integration of urban and rural health care resources in Guangxi. In order to reduce the difference of social medical insurance compensation between people, between urban and rural areas, between regions, to establish a mechanism of insurance participation with the characteristics of benign cycle and stability, and to enhance the mutual benefit, equity and sustainability of social medical insurance, To provide a reference for the further development of health care in Guangxi.
