【摘要】:隨著200年新醫(yī)改在全國推廣以來,到目前為止,社會基本醫(yī)療保險體系基本完成。也就是包括城鎮(zhèn)職工醫(yī)療保險、城鎮(zhèn)居民醫(yī)療保險、新型農(nóng)村合作醫(yī)療保險在內(nèi)的基本醫(yī)保體系初步建立。根據(jù)2012年中國統(tǒng)計年鑒數(shù)據(jù)顯示,截至到12月底,參加社會基本醫(yī)療保險的人數(shù)已經(jīng)超過13億人,基本實現(xiàn)全覆蓋。而重大疾病所導致的大額的醫(yī)療費用依然是對每一個家庭的潛在威脅,因病致貧和因病返貧依然是每個家庭揮之不去的夢魘。 為了解決這一突出問題,2012年8月,國務(wù)院出臺了《關(guān)于開展城鄉(xiāng)居民大病保險工作的指導意見》(以下簡稱《指導意見》),這項文件的出臺為各地展開城鄉(xiāng)居民大病保障工作提供了政策性的依據(jù)!吨笇б庖姟分赋,在確;踞t(yī)保能夠穩(wěn)定運轉(zhuǎn)的同時,能夠?qū)⒊青l(xiāng)居民基本醫(yī)保的基金結(jié)余的一定比例作為大病保險的籌資來源。 想要大病保險長期的持續(xù)運行下去,,從根本上解決因病致貧,因病返貧的困境,首先也是最主要的就是要解決大病保險的籌資問題。本文首先從理論上將我國城鄉(xiāng)居民大病保險籌資模式界定為籌資來源、籌資標準以及統(tǒng)籌層次和范圍。然后通過對當前試點地區(qū)大病保險的四種典型模式分別研究其籌資模式的類型和可行性。最后一部分給出大病保險的政策性意見。本文結(jié)論主要有完善大病保險籌資渠道;建立大病保險基金獨立運作的平臺;合理確定不同地區(qū)大病保險的籌資標準;適時的擴大大病保險的統(tǒng)籌層次和范圍。
[Abstract]:Since the new medical reform was popularized in the whole country in 200 years, the basic social medical insurance system has been basically completed. That is, the basic medical insurance system, including urban worker medical insurance, urban resident medical insurance and new rural cooperative medical insurance, was initially established. By the end of December, the number of people participating in basic social medical insurance had exceeded 1.3 billion, according to data from the China Statistical Yearbook of 2012, basically achieving full coverage. However, the high cost of medical care caused by major diseases remains a potential threat to every family, and poverty caused by illness and return to poverty by illness remain a constant nightmare for every family. In order to address this outstanding problem, in August 2012, The State Council has issued the guiding opinion on the Development of serious illness Insurance for Urban and Rural residents (hereinafter referred to as "the guiding opinion"), which provides a policy basis for all localities to carry out the work of ensuring serious illness among urban and rural residents. While ensuring the stable operation of the basic medical insurance, we can take the proportion of the fund balance of the basic medical insurance of urban and rural residents as the source of financing for the serious illness insurance. Want the serious illness insurance to run continuously for a long time, fundamentally solve the predicament of poor caused by illness and return to poverty because of illness, the first and most important is to solve the financing problem of the serious illness insurance. Firstly, this paper defines the financing mode of urban and rural residents' serious illness insurance in theory as the source of financing, the standard of financing, the level and scope of overall planning. Then the paper studies the types and feasibility of the financing model of the four typical models of the major illness insurance in the current pilot area. The last part gives the policy advice of serious illness insurance. The main conclusions of this paper are to improve the financing channels of the serious illness insurance; to establish a platform for the independent operation of the insurance fund for serious illness; to rationally determine the financing standards of the serious illness insurance in different regions; to expand the overall planning level and scope of the serious illness insurance in good time.
