【摘要】：新型農(nóng)村合作醫(yī)療(以下簡(jiǎn)稱新農(nóng)合)制度不僅是凸顯我國(guó)醫(yī)療現(xiàn)狀的重要載體,也是構(gòu)建多元化農(nóng)村醫(yī)療保障體系的重要組成部分,其不斷發(fā)展和完善對(duì)提高農(nóng)民的健康水平、緩解農(nóng)民“看病難、看病貴”等問(wèn)題具有重要意義。而新型農(nóng)村合作醫(yī)療基金作為新農(nóng)合制度可持續(xù)發(fā)展的關(guān)鍵,其運(yùn)行好壞不僅是衡量基金管理制度是否完善的重要標(biāo)準(zhǔn),也關(guān)乎著參合農(nóng)民的切身利益。本文首先在梳理新農(nóng)合基金運(yùn)行績(jī)效相關(guān)文獻(xiàn)的基礎(chǔ)上,以公共財(cái)政支出理論、政府績(jī)效管理理論、利益相關(guān)者理論為理論依據(jù),并在邏輯模型和政府績(jī)效評(píng)價(jià)“4E”準(zhǔn)則的框架下,以及德?tīng)柗品ǖ闹笜?biāo)篩選后,構(gòu)建了一套包括經(jīng)濟(jì)發(fā)展水平、籌資水平、管理水平和參合受益水平4個(gè)一級(jí)指標(biāo)和農(nóng)民人均純收入、人均籌資額等12個(gè)二級(jí)指標(biāo)的新農(nóng)合基金運(yùn)行績(jī)效評(píng)價(jià)指標(biāo)體系。其次,運(yùn)用因子分析法對(duì)甘肅省2014年14個(gè)市州新農(nóng)合基金運(yùn)行情況進(jìn)行分析,并提取了“基金利用”、“基金籌資”和“參合受益”三個(gè)公因子。根據(jù)數(shù)據(jù)分析結(jié)果,發(fā)現(xiàn)甘肅省新農(nóng)合基金整體上運(yùn)行較好,但各市(州)之間基金運(yùn)行不平衡,相互之間還存在差異;新農(nóng)合基金的良性運(yùn)行和當(dāng)?shù)亟?jīng)濟(jì)發(fā)展水平、農(nóng)民的支出能力和意愿、政府財(cái)政補(bǔ)助、基金的監(jiān)管程度等因素密切相關(guān)。最后,在相關(guān)數(shù)據(jù)分析和甘肅省新農(nóng)合基金運(yùn)行現(xiàn)存問(wèn)題的基礎(chǔ)上,本文從立足地區(qū)經(jīng)濟(jì)發(fā)展特色,促進(jìn)醫(yī)療衛(wèi)生資源合理配置;穩(wěn)步提升財(cái)政補(bǔ)助標(biāo)準(zhǔn),優(yōu)化基金籌資結(jié)構(gòu);加強(qiáng)新農(nóng)合基金監(jiān)管和合理使用;提高實(shí)際補(bǔ)償比和參合受益水平等方面嘗試性地提出對(duì)策建議。
[Abstract]:The new rural cooperative medical system (hereinafter referred to as NCMS) is not only an important carrier to highlight the current medical situation of our country, but also an important part of constructing a diversified rural medical security system. Its continuous development and perfection is of great significance to improve the health level of farmers and alleviate the problems of "difficult and expensive to see a doctor". As the key to the sustainable development of the new rural cooperative medical system, the operation of the new rural cooperative medical fund is not only an important standard to measure whether the fund management system is perfect, but also related to the vital interests of participating farmers. On the basis of combing the literature about the performance of the new rural cooperative fund, this paper takes the public finance expenditure theory, the government performance management theory, the stakeholder theory as the theoretical basis. Under the framework of the logical model and the "4e" criterion of government performance evaluation, and after the selection of indicators by Delphi method, a set of economic development level and financing level are constructed. The evaluation index system of the performance of the NRCF is composed of four first-level indexes and 12 second-level indexes, such as the management level and the benefit level of participation and cooperation, and the per capita net income of farmers and the amount of capital raised per capita. Secondly, the paper analyzes the operation of NCMS in 14 cities and states of Gansu province in 2014 by using factor analysis method, and extracts three common factors of "fund utilization", "fund financing" and "benefit from participation and cooperation". According to the results of data analysis, it is found that the new rural cooperative fund in Gansu Province is running well as a whole, but the operation of the fund is unbalanced among the cities (states), and there are still differences between them, and the benign operation of the new rural cooperative fund and the level of local economic development are still different. The expenditure ability and willingness of farmers, government financial subsidy, fund supervision and other factors are closely related. Finally, based on the analysis of relevant data and the existing problems in the operation of the new rural cooperative fund in Gansu province, this paper promotes the rational allocation of medical and health resources on the basis of the characteristics of regional economic development, steadily raises the financial subsidy standard, and optimizes the fund financing structure. To strengthen the supervision and rational use of the new rural cooperative fund, to raise the actual compensation ratio and the level of benefit from the participation and cooperation, and so on, to put forward some countermeasures and suggestions.
