【摘要】：完善全民基本醫(yī)保制度是推進(jìn)健康中國(guó)戰(zhàn)略的重要內(nèi)容。醫(yī)改進(jìn)入深水區(qū),利益格局深度調(diào)整不可避免,體制機(jī)制的矛盾更加突出。新醫(yī)改以來,多項(xiàng)政策文件均提出要探索建立"醫(yī)療保險(xiǎn)經(jīng)辦機(jī)構(gòu)與醫(yī)療機(jī)構(gòu)、藥品供應(yīng)商的談判機(jī)制",以發(fā)揮"醫(yī)療保障對(duì)醫(yī)療服務(wù)和藥品費(fèi)用的制約作用,形成激勵(lì)與約束并重的支付制度"。本文采用定性研究的方法,運(yùn)用治理理論探討醫(yī)療服務(wù)價(jià)格談判機(jī)制,主張?jiān)诙喾絽⑴c、伙伴關(guān)系、協(xié)同共治、協(xié)調(diào)各方利益的原則下,各方主體在多元協(xié)商的框架下將矛盾沖突顯性化,互動(dòng)博弈透明化,利益訴求談判化,共同決策契約化,完成上述目標(biāo),實(shí)現(xiàn)"醫(yī)、患、保"三方共贏。
[Abstract]:Improving the basic health insurance system for all is an important part of the strategy of promoting a healthy China. Medical reform enters deep water area, benefit pattern depth adjustment is inevitable, the contradiction of system mechanism is more prominent. Since the new medical reform, many policy documents have proposed to explore the establishment of a "negotiation mechanism between medical insurance agencies and medical institutions and drug suppliers," in order to give play to the role of medical security in restricting medical services and drug costs. To form a payment system with equal emphasis on incentives and constraints. In this paper, we adopt the method of qualitative research to discuss the mechanism of medical service price negotiation by using governance theory, and advocate the principle of multi-party participation, partnership, co-treatment and coordination of the interests of all parties. Under the framework of pluralistic negotiation, the main bodies of each party make the contradiction and conflict explicit, the interactive game transparent, the interest appeal negotiated, the common decision made contractual, and achieve the above goal, and realize the "medical treatment, patient, and safeguard" tripartite win-win.
