【摘要】：江西省自1999年開始實施城鎮(zhèn)職工基本醫(yī)療保險制度改革以來,隨著2003年、2007年新型農(nóng)村合作醫(yī)療制度及城鎮(zhèn)居民基本醫(yī)療保險制度的先后建立,逐步建立了包括城鄉(xiāng)居民醫(yī)療救助在內(nèi)的江西省基本醫(yī)療保障制度,實現(xiàn)了三項基本醫(yī)療保險在制度上的全覆蓋。然而在醫(yī)療保障領(lǐng)域,我國城鄉(xiāng)間基本醫(yī)療保險的差異是一直就存在的,江西省作為“中部崛起”城市之一,在整體經(jīng)濟欠發(fā)達及突出的“二元”經(jīng)濟特征等情況下,基本醫(yī)療保險制度也呈現(xiàn)典型的“二元”特征。城鄉(xiāng)分割的“二元”醫(yī)保不但阻礙“人人享有健康”的全民醫(yī)保的進程,也帶來了許多社會問題,國內(nèi)外不少學(xué)者在如何整合三項基本醫(yī)療保險制度的問題上已經(jīng)做了相關(guān)研究和努力。本文從三項基本醫(yī)療保險制度的基本理論出發(fā),針對基本醫(yī)療保險制度內(nèi)部缺陷和外部環(huán)境的問題,從醫(yī)療保險基金結(jié)余、財政投入等角度入手來對基本醫(yī)療保險城鄉(xiāng)整合進行研究。 本文通過規(guī)范分析法,首先,從江西省目前三項基本醫(yī)療保險制度的基本情況出發(fā),以公共品理論作為探討三項醫(yī)療保險制度城鄉(xiāng)整合的理論基礎(chǔ),并對城鄉(xiāng)基本醫(yī)療保險一體化的基本內(nèi)涵進行了概述,提出了加快整合三項基本醫(yī)療保險制度是解決“看病難”、“看病貴”,構(gòu)建城鄉(xiāng)一體化社會保障體系,實現(xiàn)醫(yī)保公平、社會公正和構(gòu)建和諧社會的需要。然后,在此基礎(chǔ)上,對江西省三項基本醫(yī)療保險制度的實施現(xiàn)狀進行了論述,介紹了三項基本醫(yī)療保險之間存在的制度差異,并從制度內(nèi)部差異和制度發(fā)展外部環(huán)境兩個方面分析了三項基本醫(yī)療保險制度存在的問題。再次,通過借鑒試點地區(qū)在整合城鄉(xiāng)基本醫(yī)療保險制度,推進三項基本醫(yī)療保險制度一體化方面取得的成功經(jīng)驗,確立了江西省整合三項基本醫(yī)療保險制度的基本思路,即要打破戶籍、部門和地域界限,通過統(tǒng)一基本醫(yī)療保險管理體制,建立省內(nèi)自由流動的醫(yī)療保險制度,實現(xiàn)人群全覆蓋的基本醫(yī)療保險制度,實現(xiàn)這一目標的第一步就是要實現(xiàn)新型農(nóng)村合作醫(yī)療制度與城鎮(zhèn)居民基本醫(yī)療保險制度的整合。明確了完善公共衛(wèi)生服務(wù)體系后,就加快整合江西省三項基本醫(yī)療保險制度,實現(xiàn)江西省醫(yī)療制度進一步公平、公正和服務(wù)的均等化,提出了相應(yīng)的政策措施建議。 在文章最后的總結(jié)部分,得出江西省三項基本醫(yī)療保險制度整合研究的基本結(jié)論：在不斷擴大參保覆蓋面的基礎(chǔ)上,內(nèi)部制度調(diào)整和外部環(huán)境優(yōu)化相結(jié)合,加之各種保障措施來給江西省三項基本醫(yī)療保險制度整合保駕護航,促進制度整合穩(wěn)定健康發(fā)展,并對構(gòu)建和完善我國社會保障體系提出了預(yù)期設(shè)想。
[Abstract]:Since the implementation of the reform of the basic medical insurance system for urban workers in Jiangxi Province in 1999, with the establishment of the new rural cooperative medical insurance system and the urban residents' basic medical insurance system in 2003 and 2007, The basic medical security system of Jiangxi Province, including the medical assistance for urban and rural residents, was gradually established, and the full coverage of the three basic medical insurance systems was realized. However, in the field of medical security, the difference of basic medical insurance between urban and rural areas in China has always existed. Jiangxi Province, as one of the "rising cities in the central region", is in the condition of underdevelopment of the overall economy and prominent "dual" economic characteristics, etc. The basic medical insurance system also presents the typical "duality" characteristic. The "dual" medical insurance system, which is divided between urban and rural areas, not only hinders the process of universal health insurance for all, but also brings many social problems. Many scholars at home and abroad have made relevant research and efforts on how to integrate the three basic medical insurance systems. Based on the basic theories of three basic medical insurance systems, aiming at the internal defects of the basic medical insurance system and the problems of the external environment, this paper starts with the balance of the medical insurance fund. Financial investment and other angles to study the basic medical insurance urban and rural integration. In this paper, through normative analysis, first of all, starting from the three basic medical insurance systems in Jiangxi Province, taking the theory of public goods as the theoretical basis for the urban-rural integration of the three medical insurance systems, It also summarizes the basic connotation of the integration of urban and rural basic medical insurance, and points out that speeding up the integration of the three basic medical insurance systems is to solve the problem of "difficult to see a doctor", "expensive to see a doctor", to construct a social security system of integration of urban and rural areas, and to realize the fairness of medical insurance. Social justice and the need to build a harmonious society. Then, on this basis, the paper discusses the current situation of the implementation of the three basic medical insurance systems in Jiangxi Province, and introduces the differences between the three basic medical insurance systems. The problems of the three basic medical insurance systems are analyzed from the internal differences and the external environment of institutional development. Thirdly, through the successful experience of integrating the urban and rural basic medical insurance system and promoting the integration of the three basic medical insurance systems in the pilot areas, the author establishes the basic ideas of Jiangxi Province to integrate the three basic medical insurance systems. That is, to break the boundaries of household registration, departments and regions, to establish a medical insurance system of free flow in the province through the unification of the basic medical insurance management system, and to realize the basic medical insurance system with full coverage of the population. The first step to achieve this goal is to realize the integration of the new rural cooperative medical system and the basic medical insurance system of urban residents. After perfecting the public health service system, the author puts forward the corresponding policy and measures to accelerate the integration of the three basic medical insurance systems in Jiangxi Province, to realize the further fairness, justice and equalization of the medical care system in Jiangxi Province. In the last part of the article, we draw the basic conclusion of the three basic medical insurance system integration research in Jiangxi province: on the basis of expanding the coverage of participating insurance, the internal system adjustment and the external environment optimization are combined. In addition, all kinds of safeguard measures are used to guarantee the integration of three basic medical insurance systems in Jiangxi Province, to promote the stable and healthy development of the system integration, and to put forward a tentative plan for the construction and improvement of our social security system.
