【摘要】：隨著市場(chǎng)經(jīng)濟(jì)體制的建立與發(fā)展，與之配套的社會(huì)保障制度也在不斷完善。作為受眾范圍最廣的醫(yī)療保障制度，其改革與發(fā)展直接影響著群眾的基本生活，成為百姓關(guān)注的焦點(diǎn)、熱點(diǎn)問題。 在我國(guó)，各省市都根據(jù)本地實(shí)際建立了城鎮(zhèn)職工基本醫(yī)療保險(xiǎn)制度。天津市也結(jié)合本市具體情況建立了符合經(jīng)濟(jì)社會(huì)發(fā)展現(xiàn)狀的醫(yī)療保險(xiǎn)制度，并不斷改革完善。本文通過對(duì)天津市城鎮(zhèn)職工基本醫(yī)療保險(xiǎn)的改革發(fā)展進(jìn)行梳理，將其發(fā)展歷程分為探索、起步、延伸和完善四個(gè)階段，通過發(fā)放調(diào)查問卷、訪談的方式，對(duì)醫(yī)療保險(xiǎn)群眾滿意度進(jìn)行評(píng)價(jià)，發(fā)現(xiàn)其在實(shí)際操作過程中出現(xiàn)的很多問題，，主要包括：醫(yī)療費(fèi)用增長(zhǎng)過快，醫(yī)�；饓毫薮�；定點(diǎn)醫(yī)院運(yùn)行效率不高，看病難、看病貴問題突出；基層醫(yī)院地位不保，基礎(chǔ)功能發(fā)揮不強(qiáng)；對(duì)患小病人員及健康人員缺乏激勵(lì)引導(dǎo)機(jī)制；社會(huì)誠(chéng)信低下，以藥養(yǎng)醫(yī)風(fēng)氣盛行；經(jīng)辦機(jī)構(gòu)管理創(chuàng)新不足，服務(wù)水平有待提升的問題。針對(duì)這些問題，文章結(jié)合外省市醫(yī)療保險(xiǎn)制度建設(shè)的有益經(jīng)驗(yàn)，對(duì)存在問題進(jìn)行研究分析，提出要推進(jìn)門診統(tǒng)籌、創(chuàng)新制度管理、探索發(fā)展商業(yè)醫(yī)療保險(xiǎn)等完善天津市城鎮(zhèn)職工基本醫(yī)療保險(xiǎn)制度的具體措施與方法。
[Abstract]:With the establishment and development of market economy system, the social security system is improving.As the most extensive medical security system, its reform and development directly affect the basic life of the masses and become the focus and hot issues of the people.In China, provinces and cities have established a basic medical insurance system for urban workers according to the actual local conditions.Tianjin also established a medical insurance system in line with the current situation of economic and social development, and continued to reform and perfect.By combing the reform and development of basic medical insurance for urban workers in Tianjin, this paper divides its development process into four stages: exploring, starting, extending and perfecting, issuing questionnaires and interviews.The evaluation of the satisfaction of the masses of medical insurance found that many problems appeared in the actual operation process, including: the rapid growth of medical expenses, the huge pressure of medical insurance fund, the low operating efficiency of designated hospitals, and the difficulty of seeing doctors.The problem of expensive medical treatment is prominent; the status of grass-roots hospitals is not guaranteed, the basic functions are not played well; the lack of incentive and guidance mechanism for patients with minor illnesses and health personnel; the low social integrity, the prevailing atmosphere of keeping doctors with drugs; and the lack of innovation in the management of administrative institutions,Service level needs to be improved.In view of these problems, the article combines the beneficial experience of the medical insurance system construction in other provinces and cities, carries on the research and analysis to the existing problems, proposes to promote the out-patient coordination, innovates the system management,To explore the development of commercial medical insurance and other measures to improve the basic medical insurance system for urban workers in Tianjin.
